X was such a genius, he payed homage to kanye saying he was a genius to imply beforehand he can be the same way. the way he says "run my run my head" is too perfectly said to not sound exactly like "i'm out my mind" at the same time. definitely a double entendre. the man the song is about is clearly abusing the woman, he tells her to give him head, while in his head he's most likely thinking this is crazy, what am i doing? i'm out my mind.
