Nah. It actually pretty funny people poking at the classic plastic surgery, but for anyone who's spent a bit of time in the gym/fitness industry and was seen Zac's body transformation since his younger days, there's a much more obvious explanation; Steroids. Hyperdevelopment in the mandible muscles (the jaw), is a text book symptom of persistent steroid use. There was a bit of chatter when he started bulking up, that he might have been juicing because of the density of muscle he put on, over what was a fairly short period of time, but one look at that mug and it's pretty much a cert. Look at any competitive bodybuilder who's juicing, and they have the same shovel face shape.

You don’t have to break your jaw to enlarge your jaw muscles. Muscle growth occurs as the body’s healing mechanism to repair torn muscles. It’s what happens with weight lifting; you damage the muscles by over stressing it, then with a couple days of recovery the body creates new mass. Efron experienced a trauma to his face, damaging the muscles which triggered the body’s repair mechanism. Most people, outside weight trainers, never over stress their face muscles to ever trigger the tear/repair system.

Granted, things that cause muscle damage (ie lifting weights) tend to also cause hypertrophy signaling, BUT muscle damage itself DOES NOT cause hypertrophy. For example, concentric only training (least damage) vs eccentric only training (most damage) produces pretty much identical hypertrophy.

HGH is used especially in people that have poor Bone Mineral Density, but in Zac's case its more visible because his bones were already healthy.
