I run 3 m8les a day max hr of 178 bpm. Is there anything I can ask my doctor to adjust on the crt device. I have a rbbb. I went from a EF of 30 to a EF of 55 with the new device. I would hate to get shocked for no reason?

Thanks for the explanation. This is very relevant to me. Resting BP 137/83. Run a 5K over 26-28 minutes. 1 hour after run and cleaning myself up reading 116/77.

Dear Professor Selig, thank you for your valuable expertise on coronary artery plaque and its possible regression with high-dose statin and lifestyle changes, etcmyocardial ischaemia. I am from India. I have been asymptomatic so far. I have a positive family history (mother had a stroke and died 30 years ago when I was young), father was free of any cardiovascular symptoms and died in 2020. I am probably a carrier of the 9p21.3 region gene associated with CAD and premature CAD, which is common in an Asian Indian population. Your advice would be much appreciated. What kind of exercise would you recommend in my case Your advice will be much appreciated. I had an angiography 2 weeks ago Coronary angiography: Left main stem unobstructed. Very nice video. What I notice is after cardio my systolic drops by a lot 20+mm but diastolic drops only slightly like 4-5mm. I see something similar in your data also. Any comments why that might be. It is a bit puzzling to me. So does it imply that the formula HRxSVxTPR is valid only for systolic but not for distolic?

Steve, I know it's been a while but this information has been amazing. I had super high BP due to bad eating for many many years. Even my banker said while looking at my Visa statement once... "Eno you eat sooooo bad!" and she was right. That was years ago and I kept eating badly. I had a belt/jeans on last year that cut off my circulation to the point that my legs and feet ballooned. I'm talking massive amounts of water. I could press on my legs and the dent would remain for a long time. So I got bigger pants. And after many months the ballooning has gone down 98.75% I'd say. I can press on my legs and the dent will last only a few seconds and I'm right down to the bone again on the shins. I sit at a computer all day too. So that doesn't help. Fast forward to going to the Dentist. They took my BP and it was high, like 206/130. I had to get a nod from the doctor before they could do the root canal. That was a Monday. On Friday after going to the lab getting blood work, unrein etc. I stopped by one of those drug store BP stations. 216/145. (still had my looser jeans on). After 5 hours at the hospital my BP was still high. At resting it came down to 183/125. So I took my jeans off for good the last 4-5 days and what do you know? BP down to (haha down to...) 186/102 at rest but when I exercise it's currently at 160/99 (still awful but better). That's how I got to your video. So without going on more and more about it I just want to tell anyone that might be reading this. I've been eating fast food for 20 years. Almost 3 times a day. I at my highest was 348Lbs. Currently at 332 working towards 300. I'm Fat skinny. My arms and legs look like they belong to an Olympic runner. (ok not that good but you get the idea) all my weight in front loaded on the belly, Chest and upper legs. Doc has me started on what's known as Water Pills. I take my first one in a few hours to start on that. Listed as Hydrochlorothiazide 25MG. 1 a day for the next 30 days. Fellow Big People here is the deal. I'm that guy that hates going to the doctor. Or Dentist. To busy with helping others to help myself. STOP IT. Poke me, Probe me. Stick a finger up me. What ever. We're on this rock only once and if you want to take a few more laps around the sun it's best to just go get looked at. - Don't wear tight pants or belts. What they say about getting up time to time is true. I think I did a good number on myself ignoring that. - It's never to late to try and work on it. If you fail, get back up and do it again. Keep going. - Screw it. I'd sooner die trying then being found days later dead. Get mad at the your system, make that heart work (as long as you clear it with your doc) - This message is for fatbutts like myself that spend too much time eating, not even trying to move or exercise. Do something now before you get into the same boat as me. You may feel ok but you're not. Passing out almost behind your wheel of your car is not a "I''ll get over it" moment. - Make the phone call. Pick up the phone and make the appointment to go see the doc. Shift your eating slowly. Do you currently do drive thru? Well replace the pop always with a water. Get the small fry vs the large. 1 paddy not 4 (don't judge me!) :) Look, I can go on and on about my screw ups but if you did get this far just know that if you are PSH as noted in the above video it really is a good thing. It's a key way to control your BP and while getting fit at the same time and who knows, you might not need BP meds. I'll bookmark this comment and in 6 months I'll give you all and update where I'm at. If it's been past 6 months reply to this commend with Update? and I should see it hit my phone. Wish you all the best. I think I get the award for the longest RU-vid comment. Sorry about the wall :)

4:46 - 'eccentric hypertrophy is increased external diameter' - I thought it was decreased external diameter as you get dilatation of chamber (increased cavity size, but relatively normal wall thickness due to sarcomerogenesis added in series & parallel? (Please clarify as I may not understand - Thanks)
@claraclara4725

Yes you're right
@marclink0

Eccentric hypertrophy does indeed mean the outside diameter increases. Highly aerobically trained athletes develop bigger hearts with normal wall thickness to allow higher cardiac outputs. Compare this to heart failure with reduced ejection fraction where external diameter is also increased but pathologically so

Thanks for posting and sharing this information Steve. I suffer from hypertension and have for many years and am on medication. I have found that doing some high intensity rowing on a rowing machine each morning causes a reduction in BP throughout the rest of the day. This works! Thanks.
