What is the cause that lets LDL cholesterol into intima of blood vessels and arteries? Also if it occurs only when LDL cholesterol is high, why doesn't it happen when the level is optimal. Thank you!

The process begins with damage to the wall of the blood vessel. Optimal levels of LDL are meant to decrease risk of atherosclerosis, but it is not an absolute guarantee that atherosclerosis will not happen. It simply decreases the risk.
