I had one of these on Wednesday. Well I think I had one - it might have been two! I had carpal tunnel release and cubital tunnel release surgeries at the same time, and I’m pretty sure the three anaesthetist team did this in my armpit beforehand.
Do you perform that block before sugery or after? I guess our surgeons might not want it before operation because the catheter would be to close to the knee (desinfetion issues, etc.)
For interscalene block for more ‘experienced’ practitioners, out of plane approach achieves better results in my experience. Thanks for beautiful demonstration of the technique.
The VASTO adductor membrane (not “VASO adductor”) doesn’t exist in the apex of the femoral triangle. It appears where the adductor canal begins, by definition. Furthermore, the sub sartorial plexus runs beneath sartorius but above the VAM, and local anesthetic will not reliably traverse the VAM to block the SSP.
