A block can have 'blood clot + fat & calcium deposit' as its components. The blood clot (if it is there) can be cleaned by a procedure called thrombus aspiration (suctioning of clot). The fat and calcium deposit (which is the most common scenario) is actually very much stuck to the blood vessel (artery) wall. Infact that becomes a part of the wall. So it can't be just 'cleaned' or sucked out from there. So in angioplasty we move it aside by the help of balloon and stent. While in bypass, we implant new blood vessels for blood supply to heart (in bypass nothing is done directly to the block itself).
