The origin is the top left corner of the original image. X and Y axis are
horizontal and vertical edges respectively. The distance is the minimal
algebraic distance from the origin to the detected line. The angle accuracy
can be improved by decreasing the step size in the theta array.

Hough transform generally is apply to detect circle i.g fruits

You are referring to Hough Circle Transform. The standard Hough transform for line detection is extensively used in many applications, especially EBSD where you detect straight lines precisely to infer materials' crystal structure.

I do not know how the lines in government forms look like and also I am not sure if they are in pdf format or images or something else. This tutorial can definitely help in detecting lines in images.
