*The only feature we can truly copy from the N95 and Surgical Facemasks is the 1 time use and discard feature.*
A JAMA study (Sept 2019 jamanetwork.com/journals/jama...) comparing the N95 respirators vs Surgical Masks in preventing influenza virus from a cluster randomized control trial of healthcare professionals using these masks in an outpatient setting (medical clinic/doctor's office which should be similar to our exposure in the public, like stores, offices, etc.) showed no significant difference between N95 (7.2% infections) and surgical masks (8.2% infections).
With a 95% filtration of 0.3 micron particles, in theory this should prevent the flu virus from infecting the trained healthcare professionals when wearing these masks, yet there was 7.2% of flu infections amongst the HCP. How is this possible? Most likely the infection is occuring when handling the disposable masks (both surgical and N95) and some droplets with flu virus was on their hands when rubbing their nose, mouth and eyes which may have occurred before washing their hands.
That may explain the 7.2% and the extra 1% which is not significant may be accounted for by the rare case of direct inhalation of flu aerosol droplets.
For non-trained people using face masks along with hand washing, I worry that they spent so much time on making a "super-filtering" face mask that they will keep the face mask for multiple uses and actually increase their risk from the larger droplets (more than 8% assuming immediately discarding the mask) while gaining a very minimal advantage on the aerosol inhalation risk. Even the cloth masks can be the washed, but the filter material is hard to wash. I hope that makes sense.
jamanetwork.com/journals/jama...
The main goal of “non-medical face covering” is to 1. keep the nose and mouth covered to prevent the an unwittingly infected user from openly spraying sneeze or cough droplets into the air which may land on surfaces or a person’s hand and 2. prevent a non-infected user from accidentally touching their nose or mouth with their own hand after receiving Coronavirus droplets from a hand shake, door knob, ATM or elevator button, etc.
Non-medical face coverings or even surgical face masks will not protect the user from Coronavirus aerosol (floating microdroplets) from being inhaled. Medical experts are reporting that the aerosol route of contracting Coronavirus is much lower than the droplet transmission from a contagious Coronavirus carrier with or without symptoms or inanimate objects (fomites) which that person has touched with a contaminated hand (sneeze or cough into their hand.)
The Lancet Journal April 2, 2020: Stability of SARS-CoV-2 in different environmental conditions. Chin A, Chu JT, Mahen RAP, Hui KPY, Yen HL, Chan MCW, et alolan. They studied how long the Covid-19 Coronavirus survived on different surfaces and environmental conditions. www.thelancet.com/journals/la...
A microdroplet (5 ul) of Coronavirus was placed on various surfaces then tested for recovery of the virus after various amounts of time.
Tissue and printer paper: No detectable virus after 3 hours.
Wood and cloth: No detectable virus after 2 days
Glass and banknotes: No detectable virus after 4 days
Stainless steel and plastic: No detectable virus after 7 days.
Surprisingly, on the outer layer of a surgical mask a detectable level of virus could still be present (~0.1% of original virus in the micro droplet.)

Check out this Explanatory RU-vid Video link at the bottom: What face masks actually do against coronavirus. It’s not meant to be a medical article (I have references to medical articles with links in the description which no one is obviously reading.) It’s something a 5 year old would understand. If you want to explain what’s happening to your children and why people are wearing masks, washing hands and social distancing. Pause the video at 7 : 19 on the VOX video and look at the diagram with all the circles with triangles around them. Most are blue (healthy), other are yellow (infected but no symptoms and still contagious.) Remember that image, then start the video from the beginning. This link starts the video right before that dot and triangle diagram. ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-P27HRClMf2U.html

April 7, 2020- Dr. Erickson said homemade masks are better than no mask at all. Watch Dr. Erickson’s news interview in the link below. bakersfieldnow.com/newsletter-daily/doctor-explains-the-importance-and-effectiveness-of-wearing-a-mask BAKERSFIELD, Calif. (KBAK/KBFX) - April 7, 2020 The Center for Disease Control is recommending people wear a mask when out in public during the pandemic. Dr. Dan Erickson, an emergency room physician and co-owner of Accelerated Urgent Care explains why wearing mask matters and how to know which one to get. He said N-95 masks earned their name because they help filter out about 95 percent of the airborne particles. “It helps decrease the spread of disease,” he said. Dr. Erickson said surgical masks act as a barrier, but they do not filter out the small airborne particles. “It’s the main purpose it’s just a barrier to decrease the spread of respiratory droplets, so you’re wearing it for yourself, but you’re also wearing it for other people in your proximity. Who would be showered by respiratory droplets,” he said. Dr. Erickson said homemade masks are better than no mask at all. “But also you don’t want to get a false sense of protection from it, meaning you still want to keep your six feet from people because it does not filter out the small viruses and bacteria.” He also encourages people to wear it out when they are out in the public, including when grocery shopping or at the gas station. The CDC and Dr. Erickson went in the news to tell us that they recommend wearing masks in public. I was pretty sure people are having a hard time getting decent cheap face masks without getting gouged or ripped off online (you order a bunch of “amazingly protective mask” and they charge your card, but the shipment keeps getting delayed or never comes.). So what do you do when you are waiting for the “best masks”? ¯\_(ツ)_/¯

Dr, Yang Justification for your work(!) I watch a daily vlog by a fellow Englishman, Dr John Campbell. He provides excellent statistical analysis of the global situation (he was predicting the current situation in early January) and has been a vocal advocate of "face coverings" to prevent viral shedding. In today's vlog, he referenced a report by Nature Science, which detailed their comparison of aerosol shedding from individuals infected by among other things, Coronavirus (common cold) You may find his review of the (much longer) report interesting. Commencing 21:34 ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-DJX6o6cVO30.html Conclusion, any mask is of great benefit, reducing viral shedding to effectively zero. ....... Here's a link to the source article www.nature.com/articles/s41591-020-0843-2 Keep up the good work! (I'm now experimenting furiously with materials at my disposal!)

@Sara Shepard Following your logic, even *walking* "causes stress on the lungs" I would suggest that anyone with that degree of restriction will not be either walking, or wearing a mask in public areas? Furthermore, moving while applying a stress, AKA "Exercise".... is, by all accounts good for you! The amount of CO2 retained in the confines of a mask? Negligible compared to lung capacity. Bacteria? Let's confirm this..... From the wearer? Who already had the bacteria present...... Hmmmm. Seems that bacteria would be restricted from infecting others! Excellent!

I made this to go to the store the other day. When I got back into my car - still wearing the mask - I opened a container of disinfecting wipes and was wiping my hands and keys and etc. It wasn't until I took the mask off that I noticed the strong smell of the wipes. So that was reassuring. Thank you.

I just made 2 masks following your instructions. One paper towel with a shop cloth inside instead of tissue and the other shop cloth with tissue inside. Both work and fit well. I prefer the chain elastic method, my ears are small and too weak to hold the elastics. These masks will do in a pinch. I'm going to write in small lettering, "If you can read this, you are standing too close". Thank you so much for this vid, Dr. Yang. Sandy (Canada)

Hi Sandy, I love it! “If you can read this, you are standing too close”. Priceless. LOL Maybe we should add that as a gag feature on the next mask. I’ll give you credit. I will need to do it “dead pan” to see if anyone catches it. Thanks for the feedback, Don’t forget to share the with people who may not have access to masks or are on the fence whether or not to comply. I suspect some people want to do it, but their masks from Amazon are taking weeks, and they tried the handkerchief no-sew and find it too bulky and suffocating.

There are some who have medical problems and are concerned but on the whole I think most people will wear a mask.

In case anyone is interested, to make one of these masks with fabric I just cut a pc. of cloth to the same size as the paper towel. Then I did everything else the same, adding the paper strip, paper towels and tissue to the cloth, & folding it all the same. That gives me an extra layer, but it is still light & easy to breathe thru'. Also, I fold my ends to the inside for a little neater apprearance, and nothing scratches my face. Love this pattern! I can whip one up in 5 minutes or less! Thanks again! Oh, another thing is.... with the tissue next to the face like this, it can be changed out easily. That's a plus for me (or anyone with allergies) who has a tendency to drip in their old age! LOL

This is the best mask design ever. I've sewn 38 masks so far for family and friends, and it was a herculean effort. From sourcing the materials to all the steps from cutting the pattern, then marking the fabric then cutting the fabric pieces then sewing the pieces (inside out mind you, cause that's how you sew) . I was able to cut down my time from 45 mins to 25 mins per mask but still... what an effort. Then came along this video. Dr.Yang wasn't kidding about 2 minutes. And I think this mask is very effective after testing it out too. Thank you for sharing. I hope this isn't a first and last time I'll be thanking a Yang in the near future! ;)

After I made this, I tried it using cloth and it's so comfortable. I replaced the 2 paper towels and Kleenex with a square bandana folded into thirds, then followed the video folding instructions. (Hot iron helps with the folds and general smoothness.) Instead of rubber bands you can use hair ties, but I cut two sections/loops of pantyhose legs. (If they are big enough they can each loop around a ponytail, leaving the ears free.) I replaced the staples with a couple thread stitches by hand, but small binder clips work too. I then chose to trim the excess bandana because it is longer than paper towels. Held in place extra with my glasses, which it did not fog up.

Yes indeed. If you wear glasses it is a safety issue. Fogging of the lenses is annoying and very dangerous if you are walking, jogging or cycling with a mask, as you cannot see where you are going. Also, if it us just you driving your own car, you don't need to have a mask on unless you go in and out of the vehicle a lot (i.e. delivery driver). You need to be able to see. If you find it too hard to breathe, try leaving it a bit looser at the back of the mask where anything that goes out will move toward you or in your hood rather directly away from you.

Hi James, You’re welcome. So happy to hear that you like it. You’re feedback means a lot since you also know what it is like to wear a mask for many hours. Most people are still getting used to it. People who wear masks as part of their jobs, realize comfort is very important. The more comfortable it is, the less you touch it. Imagine me in a 5 hour surgery wearing an uncomfortable mask or poorly fitting one that keeps slipping or gets sucked into my lips like a one way valve. This version is very wide, and a commenter from France said he was laughed at. I think the original version ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-NCF7lCHMnII.html with a tissue paper insert (Kleenex antiviral if anyone can really get those) and the Blue shop towels on a roll, can actually pass as a procedure mask without people noticing it is a DIY.

I used the doctor's pattern to make a mask so I could get a haircut. Instead of using rubber bands, I shortened the sides a bit and used medical tape to attach it to the sides of my face so my stylist could easily wash, cut and style my short hair. Thanks!

You do know that, certainly at such close quarters, being carried in tiny water droplets on your breath when you exhale, the virus is more likely to enter your body through the membranes in your eyes rather than breathing it in? That’s why medical staff are wearing the full face visors as well! I don’t think a haircut is worth catching this nasty virus for.

This kind of mask is the best fit for my face! Thank you! Now im just choosing which kind of cloth to use. Btw, can you please make a video/review on copper masks? One brand design has a hole on the chin area of the mask claiming for the user to breathe easily. And the copper material would kill 99% of germs.

Congrats, Dr. Yang. I've also been surprised by other articles showing a very close performance of surgical and n95 masks, both better than cloth ones. The issue of properly handling it and being a one use only piece is great too. Indeed the aerosol risk is comparatively minor. Respect, from a colleague.

Of all the no-sew masks I've made and worn these last two months, this pattern is my hands-down favorite. Heartfelt thanks to you, Dr. Yang! Below, minor tweaks based on materials I had on hand... Am using blue Shop Towels for the body of the mask, which I'd bought for another mask tutorial. In place of the printer-paper strip, am inserting an additional strip of Shop Towel, cut to size. After creasing and unfolding the towel, I tape a "nose-grip" no more than a quarter of an inch below the center crease. For the grip, I use an 8-inch length of pipe cleaner folded in half to make a 4-inch double wire. Then, before taping the wire onto the towel, I use needle-nose pliers to bend the two exposed, pointy tips backwards into a tiny flat loop, thereby avoiding an unsightly, mask-induced cheek wound. : ) I complete the mask as per your tutorial. Having mostly stayed inside these last few months, I haven't had to wear a mask for longer than an hour. (And I don't wear any mask more than once.) But the day may come when I have to be out all day. You recommend putting on a *new* mask after 3-4 hours. What happens to a mask after 3-4 hours to render it kaput? All the best to you, Dr. Yang, and again, thanks!
@GeorgeYangMDNYC

Nice job with your mask build. I think that depending on exposure to people, there may be a possible accumulation of potential virus on the outside of the mask. In a school setting without social distancing, I can imagine this happening. The 3-4 hour idea came from a news piece about how the Taiwan school children are still going to school and what measures they are taking. It includes how they eat lunch in a crowded classroom. ru-vid.comUgypWivl4DizEgpPxOl4AaABCQ?app=desktop The morning mask is tossed out and hands are washed prior to eating. After lunch, they don’t want the children handling the used mask (where do they store them?) and putting it back on, so they wear a fresh mask. A proper surgical mask should only cost 20-40 cents. It should be like kleenex tissues. As a surgeon, we have worn the same mask for 12+ hour long surgeries during my residency So it should be fine from a usability standpoint.

@@GeorgeYangMDNYC *_"...possible accumulation of potential virus on the outside of the mask."_* That makes sense, and I'll be sure pack extra masks if I need to be out all day. I'm a facial plastic surgeon based in New York City. My office has been closed since March 16. The hospital did not need me on the frontline, but requested all of the voluntary faculty to shutdown their practices and not see any patients. The doctors who are paid by the hospital still went to work, but our private practices were deemed not essential. Initial projects before the DIY disposable face masks was making Face shields for the frontline doctors and nurses who I know. They work at the hospitals and when paid by the hospital, even if you are not normally a COVID-19 doctor, they are paying your salary and will ask you to help where appropriate. For private practice doctors, we are in the same situation as the small businesses. We have no income and can only wait to restart. In the meantime, try to help in anyway I can. You can see the face shield work on my Instagram: instagram.com/georgeyangmd/ I had a funny thought about my DIY face shield project. The shield is made of "plastic" and it covers the "face." So, I'm a "Facial" "Plastic" Surgeon. Go figure. ¯\_(ツ)_/¯ Since 2013, I've wanted to make educational RU-vid videos and videos about my practice, but never had the time to actually make the videos, because the practice was busy enough, and the 3 kids took up the rest of the time. With this forced break, I finally had time to work on these projects, while the rest of the time helping them with school, and rekindling my cooking skills which I hadn't been doing much since medical school. The RU-vid channel and comments section is like my online clinic. I get to answer questions and if people are having trouble making their masks, I can try to help them troubleshoot.

Genius! I purchased the paper shop towels from my local automotive supply store and made the mask with a shop towel. The piece of copy paper gives the mask more structure than the multi-fold mask that I'd been wearing with the metal nose piece. Plus, I can make this mask with fewer steps. I'll share you video with my Facebook friends and family.

@@GeorgeYangMDNYC I just tried both shop towel and paper towel masks. For me, the stiffness in the paper towel gives me a tighter fit.
@GeorgeYangMDNYC
@GeorgeYangMDNYC 4 года назад
Ariel TaylorMO Thanks for the feedback. If the paper towel give you a tighter fit because the shop towel is softer and more fabric-like which is prone to collapsing. I was hoping the printer paper stiffener would give some structure. Based on what your feedback, maybe the best combo, is using the shop towel as the filtration insert in place of the tissue paper and the stiffer towel to structure the mask for a better fit. 👍🏼

@@GeorgeYangMDNYC You can straighten out a LARGE paper clip and secure that with staples the rolled inside top edge. It gives you a good, adjustable nose hugging feature.
@GeorgeYangMDNYC
@GeorgeYangMDNYC 4 года назад
Leutchik Pipe cleaner or a piece of folded aluminum fold is softer metal and easier to bend. instagram.com/p/B-piJ2XjaR3/? The reason I left it out of this video, is that the paper has more stiffness than cloth. If you bend a strip of the printer paper, you will get an arch. Try that with a strip of cloth, it doesn't work. To keep the build more simple and straightforward, it isn't necessary, but if I were to make the same mask out of cloth I would definitely include some kind of metal nose insert. Feel free to try it both ways with and without the paperclip. You can also use a different filter insert if you have better materials at home.
