IMPORTANT 2023 UPDATE re: IUD DURATION OF USE- US FDA now approves 52mg levonorgestrel IUD (Mirena, Liletta) for 8 YEARS use for contraception. When treatment of heavy menstrual bleeding is the primary indication, best results are usually seen in the first 5 years. Kyleena (19.5mg) is still 5 years. See video #35 for info on copper IUD extended use.

I have had my IUD for two years and just been told mine perforated. This has been a very all encompassing video and made me feel extremely relaxed about this stressful situation. I hope I can get it taken out soon! Thank you so much for this video!

This video was so helpful and informative. I had my first IUD (mirena) perforation 3 years ago where my IUD became embedded in my uterine wall. (This had no pain, only realized once I tried to have it removed and the OBGYN was unable to, and scheduled me for surgery). I just had my SECOND perforation yesterday after several days in the ER. I couldn’t stand up without needing to faint, had intense pelvic pain, hemorrhaging, and a swollen cervix. This time my IUD was embedded in my cervix, and the OBGYN also said she had to scrape my cervical tissue after removal and clean up all the dead tissue that was there. This was an extremely painful experience even now in recovery. I know after second IUD was placed 3 years ago, the doctor did a follow up a month later and my IUD was in the correct position. Is it common for them to move on their own? I asked the OBGYN on yesterday’s surgery if this would effect fertility, and she said “I don’t know honey that is up to god.) 🤦🏻‍♀️ the last answer I wanted to hear from a professional… so your reassurance was extremely comforting as I’m only 24 with no children. 😭

So sorry to hear about your experience (but glad to know that you found the video helpful). Most perforations occur at the time of insertion, but there is a (very small) chance of an IUD moving or shifting after the insertion (or even after the 1 month check). Most procedures to deal with this situation would not effect future fertility (although I was not the surgeon in this case and so can’t comment more specifically). -Dr. Dervaitis

I NEVER HAD A MISCARRIAGE BEFORE THE IUD BUT AFTER IT I DID AND MY IUD HAD BEEN OUT OF ME FOR ABOUT 2YRS. I HOPE UR BODY HEALS AND ARE ABLE TO HAVE CHILDREN IN THE FUTURE. I DO NOT RECOMMEND IUDS TO ANYONE!

@@sweetcookiecrush thank you! I’m so sorry that you experienced that and I truly hope there is no direct correlation to the IUD. It’s been 7 months without the IUD (no other BC, no protection) and I have been actively trying to get pregnant with no luck (yet.) but I’ve had tons of other random pains in that area. I try and tell everyone I know to stay away from the IUD it’s a nightmare.

Hello Dr, your videos and suggestions were so helpful for most of us so I would like to thank you. My question is that it's been like 8 months but I sometimes get small brownish discharges, is it something related to the iud or what? Other things like my health... Are very okay.

I had a Paragard IUD for 10 years. When I went to have it removed I discovered it was embedded. The first doctor couldn't remove it at all and called another one over who gave me an injection for pain and dug around for several minutes before managing to remove most of it. However, one of the arms of the IUD with the copper coiling is still inside of me. Is it dangerous to leave it there? It seems like surgery would be the only way to remove it and would be quite invasive.

.....yours is the ONLY vid I've found that even MENTIONS partial perforation and not until two minutes at the end as a dismissed "subtopic". I am having serious pain and am in line for appointment with a gyno ...but there's very little info out there on partial perforation YEARS after insertion. I'm so frustrated

I love my liletta iud! Ladies do not make the same mistakes as me. I had one kyleena and 2 mirena iuds taken out because of horror stories. The iuds never caused me any serious side effects. The iuds caused me no problems. Don't read the horror stories. I am not doubting the women who say iuds caused them problems but there are a lot of women who have no problems with iuds. I made a appointment to get a liletta iud inserted at planned parenthood and my liletta iud was inserted on July 21,2020. Not everybody will get acne from iuds. If you do get acne spirolactone helps but you need to be patient.😊😀

Hey there! Thankyou for your video, it's really helpful. I've had my mirena for 2.5 years now and about 4 months ago I started having light spotting every month to eventually a full period. I went to Dr's and they've said its missing and I have a scan next week to try to find it. If it was inserted correctly, how else would it have perforated? It seems odd that such a blunt object could go through the organ. Thankyou again! Xx

Hi Rachael- Best not to speculate at this point, your scan will help to distinguish a perforation versus expulsion situation. I can’t give personalized medical advice over the internet. Generally speaking I will say that delayed perforation (ie, months or years after insertion, versus at the time of insertion) is very rare but has been reported. All the best, -Dr. Dervaitis

I had Mirena placed back in 2008 after giving birth to my son. I went through all worst side effects from abdominal pains, pelvic inflammation, foul odor (fishy) discharge, water discharge, nervousness, chills, fever etc... due to not having health insurance for 5yrs I wasn’t able to go back and get it taken out right away. After getting it when I moved back to my home state my OBGYN doctor told me the IUD was inserted wrong and I now have Pelvic inflammatory disease with heavy periods for months. Now I’m anemic, anxiety and I have heavy water discharge and randomly fishy smell is released from the vag. I don’t know what to do anymore!! Do you think a hysterectomy will help relieve my symptoms?

Ive had mirena's for about 9 years. My current one i've had for 4.5 years. For the past couple of months, ive been having spotting between periods. I started cramping around my last period ( I was thinking it was bc of my period.. ) But the cramping has been coming and going for a month! Only on my right side! Does this sound more like an ectopic pregnancy, perforation, or more like a cyst? I know I need to get it checked out, but don't have health insurance so I am kind of just waiting it out for now until maybe it goes away

Dr. I have a question, I got an copper iud 3 months after giving birth and it fell out right after but my doctor at the time replaced it but a year ago i went to a next doctor and they told me they couldn't find it and I'm guessing its still there should I have been worried to why they can't find it?

Hello, I was wondering if you have a video in which you explain how the IUD stays in place? I really want to get the copper IUD, but I am scared that the IUD will move or even fall out during a workout or other movements like dancing.

The design is such that it is securely held within the uterine cavity- the “arms” of the T only collapse (in a way that the device can fit through the cervix and come out) when one directly pulls on the string (such as at removal). Vigourous activity does not affect IUD position. -Dr. Dervaitis

Could it be possible for a Mirena to be partially perforated? As in part of the IUD is still visualized in the uterus, but the other part coming out of the uterus?

Partial perforation is very uncommon but theoretically possible (most commonly a perforation is complete and occurs at time of insertion). -Dr. Dervaitis

Hi Doc, I know you can’t give medical advice online. I got an IUD about a month ago, the pain was terribly intense and came in waves for about a week. It got to the point where my side, hip, back, and thigh were radiating. I went to the ER and they did an ultrasound. They said nothing was wrong and they can be uncomfortable and I should tough it out. I’m now experiencing this again, but on the opposite side, is this a normal side effect?

How would i know if the mirena shifted? Would it feel sort of like a tampon not inserted all the way or would i just feel painful cramps and bleeding?

My missing iud was found 17 years after insertion attached to my large intestine and omentom. I had 2 pregnancies with it there unbeknown to me as the doctor had told me that I had expelled the iud. He only gave me an ultrasound on my uterus and didnt look further into the abdomen. I still have the X-ray of the iud in my abdomen as it was found after I complained of pain. I underwent major surgery to have it removed.

If my strings are so long that they stick out of my vaginal opening, is this an indication it may have moved? I had very heavy bleeding, clotting, and cramping post-insertion and when I first checked my strings after a few days I felt it stabbing me and noticed how long they were.

Generally, the strings should not protrude out of the vaginal opening. If you are concerned about the position of your IUD, see your doctor (and use back up contraception in the interim). -Dr. Dervaitis

Hi Dr, I got my iud in November the doctor who placed it told me the iud was lower than she would of liked it to be but she left it how it was. Should I get it reinserted and is there any risk with leaving it as is?

Speaking generally, if a hormone containing IUD is slightly low in the uterus, it is still functioning. Precise positioning is more critical with copper IUD’s. -Dr. Dervaitis

If iron deficiency is secondary to heavy menstrual bleeding (and bit some other cause...) the levonorgestrel IUD may help because it tends to decrease menstrual blood loss in 90% of patients. ( Please see video #19 & 20). -Dr. Dervaitis

I had to have my IUD removed after 7 days of insertion, my Dr tried to find my IUD by trying to find it without ultra sound and looking internally, the Dr came to the conclusion that my IUD was retained as he could not see the strings and i was booked for surgery the following week. They spent 15 min in the operating room to search for it and while waiting in the recovery room i was told by my dr that my IUD was removed and to go home as i didn't need to worry about the IUD anymore. However i come to find out 8 years later that my IUD was still inside me and it had perforated inside me for 8 years and had perforated my uterus and bowel. They did not use any x ray imaging or ultrasound in the operating theater. Is this the correct procedure, as in this video, you mentioned once IUD is suspected of being retained, the next step is to do an xray to find the IUD before the surgery.

Hi doctor I just come back from the doctor and I have removed iud and put a new iud, while he was doing tha he performs my uteros I don’t know how tha happens but I was bleeding a lot and painful.. I’m worried that can leads to something worse.

I have a question. I have never taken any form or birth control. I don’t have a primary doctor to ask either but I was wondering: which kind of iud should I get? Should I get the hormonal one or the non hormonal one. I am worried that the hormonal one will cause too many problems but I’m not sure. What would you advise?

Light This obviously isn’t a professional recommendation but just to share my story, I’ve been the Mirena IUD and as far as “side effects,” mine have been very minimal. I already suffered with acne and on the IUD, it actually got “better” (or I just got older), no significant weight gain, I did have slightly stronger cramps but that’s it!!! Hope that eased your mind a little!!

Is there a bigger risk for mirena vs paragard? Is it possible to have a perforation months or years after insertion?

Most perforations happen immediately at the time of insertion, although there are rarer instances of delayed perforation.

I am currently experiencing what you called partial perforation. I am scheduled to get my IUD removed dec 1st. I am very worried because my PCP brushed me off for months when I kept going back complaining about pain and a lot of other signes of this and it took me going to the Ed in pain for her to see what was going on. She said she won’t even touch me. The way she described it got me very anxious because she said they might need to take it out surgically but my OBGYN is gonna try to take it out manually. I am already in so much constant pain I’m scared of what it’s gonna feel like when the time comes to take it out..

If a patient that has had one in for over a year, has lite spotting or noticeable bleeding during intercorse consistently, has their period, and then 2 weeks later, notices abdominal pain, and then has bleeding that night like the start of another period, what could be potential causes of this?

😢 Hi I'm in this situation right now...I have tried to get the IUD removed twice and the doctors have told me that I need to go under surgery, because there is no string, and it is moved up into the uterus. Do you know how risky this surgery is? Would this be a case in which a Laparoscopy is next, and how is scarring?

1 in a 1000 but happened to us :( The explanation about omentum as well as the flexible nature of the unit helped us feel better about this situation while we await over the weekend on how to proceed with surgical procedure that is yet to be scheduled. We had a terrible doctor that was clueless. Ultrasound was not visualized and we had to take an X-ray to confirm. Our next step is the CT scan to figure out its exact location and then surgical removal. Just frustrated that the doctor that did the IUD insertion was useless and I had to do the research myself. The explanation given in this video was SOOOO helpful in understanding what is going on and gave us a lot of comfort during this uncertain period where the medical professional we chose failed us. We truly appreciate the detail and in-depth descriptions. Thank you.

I want to ask a silly question. I read on the internet that the estrogen or combination birthcontrol pills make you age slower and that people with hormonal iuds age faster. Do you think that's true?

I got a mirena iud inserted on August 27 but I made the big mistake to get it removed because of some acne that I got. I regret getting it removed because the gynecologist said the acne gets better and she said spironolactone helps . I made appointment to get another mirena iud inserted. But this time I'm not getting it removed unless I get a serious side effect.

Hello Doctor. I just wanted to share my experience and potentially get some feedback from you. I’m 22, I’ve had the Paragard for about 2 years. They warned me that I’d experience more cramping and heavier bleeding. However they didn’t tell me that although never experiencing contractions that I’d feel as if I could relate the amount of pain during my periods to what contractions could possibly feel like. THEY ARE HORRIBLE. I thought a little more cramping wouldn’t be that much to handle but my menstrual cramps have become 10x worse. And ALOT more bleeding. This started ever since the IUD was placed. I was always able to feel my strings until recently, I went in to get the IUD out after a lot of unnecessary denials and delays from doctors. I have had a friend who also couldn’t find her strings when she wanted it out but the doctors performed an ultrasound and with a lot of “digging” they managed to remove the IUD from hell. I mean the IUD. With that being said I am praying that I don’t need a surgical removal. Even though the strings aren’t visible for whatever reason, if they managed to get her IUD out with a lot of digging (and not surgery) can you explain how this could be done ? Does the doctor dilate the cervix ? Is this painful ? I’m just praying I don’t need surgery but if I do I’m also hoping I can still have kids later in life. They should take this form of birth control off the market. It’s horrible. Even if a doctor were to tell me that with a perforated IUD and the need to have surgery that future fertility wouldn’t likely be affected I have a hard time believing that. No offense to you as a doctor. If your reading this and your considering getting an IUD DONT DO IT.

Yes I had the same exact thing, I’ve always had bad cramps. But these are not anything I ever experienced before, and I can say confidently that this type of pain was on the same level as birth contractions. I was fine for the first few days, but after that it was pure hell every day. I was also working a very active full time job at the same time of the pain, it got so bad that I had to leave early. When I was at home it was at its peak, and if you were my neighbor you would’ve thought I was getting stabbed to death by the sound of my screams. The first er I went to was local and of course they were full.. my dad rolled me in a wheelchair and some staff just rolled me in a empty room and left me there for like 25+ mins without any information as to when I can get seen or atleast even looked at. It felt like I was in the wheelchair for hours just screaming in pain, only after 30 mins gone by did somebody finally check on me. It was a male doctor telling me to be quiet and that I’m over exaggerating, when this is the most excruciating pain I’ve ever experienced in my whole existence. So I called my dad to come get me and we went back home because thankfully my dad gave me half a percasett which took so long to kick in and was not that strong, but atleast it got me thru the night. Same type of agonizing contraction pain the next day, but this time my dad took me to the ER a town over which is a way better one and bigger, and nicer people. They got me to a room immediately and got me on some pain medicine right away, and later did a vaginal ultrasound. The first five minutes of it really hurt but then the medicine was kicking in while the other nurse was holding my hand and talking to me, she also said my hands were really cold which is normal for me since I’m a bit thin and get really easy. It turns out the iud punctured my uterus lining and took it out asap, I had it for about a month and a half to two months. This all happened a year ago and nonstop heavy bleeding ever since, with the exceptional week where it lightens up or stops for a day or two. I can never get warm, and I am chilled to the bone all the time

I got my IUD a little less than a month ago. I had a lot of pain with insertion that never went away and constant bleeding up until 2 days ago when I started just having chunks of tissue come out. Contacted my GYN multiple times and the RN just kept telling me it was all normal. The feeling that my IUD is stabbing me has never gone away. It has been constant severe pain to the point of vomiting. The RN reluctantly finally ordered an ultrasound and I found out that it has penetrated 6mm into the lateral myometrium bilaterally and is 2.5 cm below the funds endometrial lining. Does this mean it is embedded? Does it mean it can’t be removed normally? I had it put in due to bleeding through ultra tampons in 30min

When it was put in I had told her that previously I had issues because of a small uterus but she had assured me the size difference between the available IUDs wasn’t enough to make a difference.

Hi im 19 with 2 girls and i got my IUD put in oct 30th 2023 at my 6 week postpartum appointment once i had it the first 2 day i just spotted then had severe cramping i was told its normal and that id be ok then i began to bleed heavily which i was told was also normal and that it was just my period so i didn't ask more questions about 2 days ago i had sharp pains in my lower ribs where my kidney is on my left side an brushed it off this morning Nov 12th 023 at 2:20am i woke up to havimg really bad pains in my back and side but mainly my left side i told my self it was probably another muscle spasm but realized it wasnt the same and started to think i had another kidney infection the last one i almost caught to late and it almost spread to my blood so i went to the ER and im so glad i had a great ER Doctor and nurse they took a urine test and blood test to rule out kidney infection or anything elso and then took me to do a CT after some time they told me everything looked good on my tests except for the CT i was then told my IUD had made its way out and up to where my kidney is on the left side it is now 7:12am and at 8am i have to call to setup an appointment for tomorrow and go from there on how to get it removed ill make an updated when i get it removed but i was also wondering if it could cause any complications where its at right now or if it continues to move Update it is Nov 13th 2023 I have it removed and did go to a different hospital for a second opinion and they admitted me to get it removed because of where it was at and it actually moved and was half way through the fat surrounding my liver but they were able to get it out I was told that if I had waited like the other hospital wanted me to I could have more complications so I'm glad I listened to myself to get a second opinion im going to be using the patches for a couple years then try the IUD again hopefully with better luck then

I had an iud perforation in the side but not through my uterus. I was postpartum when I got it placed. I found out when I went to get it removed. I had no extra pain to make me think it was an issue. It was more painful getting it removed but no issues besides that.

Hi Dr. Kristina, I would like to share my story regarding my IUD perforation/migration. I’m 19 years old, 3 months postpartum, and exclusively breastfeeding. I would like to update in the comment section as soon as possible. On April 14th, 2020 I had a Kyleena IUD placed and it seemed like everything was well. My bleeding stopped shortly after the insertion. I then suddenly started getting sharp pains on my lower left quadrant and I had heavy bleeding. I checked in with my OB this morning and they couldn’t find the strings or the IUD with an external ultrasound and internal ultrasound. After an X-ray and Cat scan, they found that my IUD was no longer in the uterus and is sort of upside down. The hospital sent me home and I have to go to my OBs office in the morning so the IUD can be removed via laparoscopy. I’m laying in bed worried with some pain here and there. I’m not a medical professional. I just wanted to share. If anyone has any concerns they should 100% consult with their doctor.

Thank you for responding! Well, the IUD is still wedged in me somewhere haha. I’m scheduled for surgery tomorrow morning at 7:30am. It might have to be pushed off just for insurance reasons.. and Because I had a cesarean section, they might go through there instead of my bellybutton.. the procedure needs to be done at the hospital. Weirdly a lot of my pain has subsided and my bleeding has been minimum.

I've had mine for 1 month & 3 weeks. Started feeling severe pain in stomach & back for 3 days. Knowing I had an appointment 4 days later I just used Tylenol, but did not help did a self check & can feel it protruding thru my cervix & called doctor she said not possible im just feeling strings i told her no! Hey Doc! I've had my IUD for about 2 years now and never had an issue until I had laparoscopic surgery (for a shunt placement) back in October 2020. Since November 2020 I have been having strong/stabbing pelvic pain. I have seen every doctor and even had a colonoscopy performed to see where this pain is coming from and I have gotten no answer. Can a laparoscopic surgery have an effect on my IUD? All my pain is in my pelvis/vaginal area.

This happened to me. I had it in back in Feb. Never had a follow up appointment but was still heavily bleeding. I finally managed to get an appointment to remove it and the doctors couldn't find the strings. She couldn't see it at all and now thinks it's fully embedded inside me

@@andreasmedley1262a month ago on that Monday, my gyno was booked back to back for pregnancy appointments (cus ya know how people ain't got nun better to do than to be doin the hanky panky in these times of quarantine) so she didn't have time to remove it, she had her nurse do an x ray on me to see if it had migrated further or embedded or anything. It just migrated upwards and i can't feel my strings. It's not life threatening or unbearably painful, it just makes me feel like i'm being stabbed at times. Tomorrow is when i have a whole hour booked with my gyno to have the removal procedure. I also got prescribed Valium to ease my nerves lol.

I almost died from bad infection from Mirena and was hospital stay for a good week also diagnosed with pid. They only did ultra sound and didnt see it and never looked any further. I am so positive it never fell out this was 12 years ago and had abdominal problems ever since like cervical cyst ect. I had my son 4yrs ago so its def possible to conceive after complications and was normal vag delivery. I cant help to wonder all these years though if its gonna cause a problem and show up one day!

Rhea Apuad This is only speaking from experience, I had some bleeding and cramping in the days following my insertion and my doctor said it was to be expected. Of course, if it is significant bleeding and pain, tell your doctor that you want to be seen. Better safe than sorry. Hopefully you’ve recovered now!!!

Rhea Apuad I can’t tell you if it’s “normal” or not, you should email your doctor. But if you read the pamphlet for your IUD (or whatever info your doctor gave you) it should tell you what to expect afterwards. And watch the video she recommended!! #7. The IUD can cause pretty irregular bleeding. I even have to ask my doctor sometimes because I worry and I’ve had mine for 3 years. I’m from the US and luckily I was able to see my doctor a few days ago and she checked my strings during my Pap smear!

Hey I have a question. I got my Kyleena IUD put in last September after watching all your videos. after 1 month I went for a checkup and the strings were missing. I was sent to have an ultrasound and they found that the IUD is in the right place so they said I can keep it in... However, since having the IUD I have had terrible sharp cramping pains randomly (some so bad I have to just lay down and stop what I am doing to deep breath). since then I have gotten another ultrasound and they said it was in a correct location again (this was 2 mo this ago) since last week I have had on and off stabby cramping pains and no bleeding... (I have pcos but it has never been a problem like this) The pain usually lasts all day then it's okay for like 2 days and so on... Is this normal? How do I know if it's in the right place if I can't feel the strings? What is the process of removing an IUD when the strings are missing ?

@@serenaperez5132 Hey, I ended up getting a 2 week period after it, I was in pain for like 2 weeks non stop now I have no pain at all. I didn't get a chance to go to the doctor because of Covid-19 and seeing as my strings went missing from month 1 (ultrasound shows it was in the right place at the time so I kept it in) I really don't know what is going on up in there... Ugh frustrating I hope nothing is wrong. Hope you manage to get help soon xxx

Luna Vicente Are you sure the strings are “missing” or you yourself just can’t locate them? It seems to me like the IUD is placed correctly according to the ultrasound. I wonder if it’s a possibility that maybe just the strings have wandered up and that’s what’s causing the “stabby” pain you described? Have the doctors been able to locate the strings or did they just tell you to ignore it?

me: has pain to where I can’t walk, move and have thrown up almost two weeks after placement and asks for advice to make pain stop my doctors: no help for you, no relief for you

I am so sorry to hear this. Remember, any doctor can remove an IUD. If you don’t feel that your concerns are being adequately addressed by your doctor, you can be seen at an urgent care clinic, “walk in” clinic, sexual health clinic, or emergency room. -Dr. Dervaitis

Dr. Kristina Dervaitis i feel a tad bit better, I was stuck in bed for abt a month. I only get pain from the arms that make it go down to my legs but hopefully it’ll stop sooner or later

@@Leahv103 The 10year copper one. I didn't know it was perforated until I found out I was pregnant. The pregnancy was not successful. I required two separate doses of methotrexate as it was an ectopic pregnancy.
