I had an MRI in November that shows at L5/S1 disc bulge and tear. I’ve had pain around my sacroiliac joint and severe back pain. I’ve had no leg pain but do still have issues with pins and needles/tingling in my foot.

I responded very well to epidural shot, it absorbs the buldge, lasts about 6 months. I get 95% daily mobility back.

7 Signs Surgery May be Needed: 1.Constant and severe pain made worse by any movement whatsoever, especially if happening over period of 4 or more weeks. 2. Constant pain at night. This severely interrupts sleep (body heals itself best during sleep). If you can’t sleep, you won’t be getting optimal recovery, which means disc bulge won’t be healing. 3. Cauda Equina Syndrome (pain down both legs, numbness around saddle region, issues with bladder and bowel). Time is of the essence. 4. Profound weakness in one or more of the muscle groups in the leg. If the disc is compressing disc badly enough, the fibers of the nerve that supply the muscle can’t do their job. Some weakness is not classified as severe weakness; this can be attempted to be rehabilitated through physiotherapy. 5. Suffering over a year with no improvement (after following all the correct advice and tried multiple conservative methods). 6. Failed course of treatment with a sciatica specialist. Complex cases need someone who specializes in that area. If there is no improvement between 8-12 weeks, a second opinion is needed. 7. Unable to return to work after several months of treatment. Not relevant for each person. This is done on an individual basis.

I had done L4 L5 laminectomy on 18/11/2022 and still suffering from little pain in butt and hip some times please make a video about exercises that safe to do after laminectomy surgery .thank you so much for makeing informative videos

I began getting pain in both legs around 15 months ago the symptoms progressed slowly at first but early in 2024 they become much more noticeable and disabling, I had an MRI scan in early March showing that I had 3 bulging discs one at L5-S1 which indents the theca and narrows the lateral recess but with no neural compromise same with L4-L5 plus mild reduction in space of central canal, the third at L3-L4 is a broad protrusion which indents the theca and compresses the central canal which is reduced in space and is stenotic. My discs are desssicatedm and L5-S1 is reduced in height. A recent diagnoses was significant canal stenosis in the lumbar spine with neurogenic claudication and incomplete caudal equina syndrome. I do experience weakness in my legs but only when the neurological symptoms/pain is present such as when standing or walking for only a few minutes and I feel I might fall if I did not sit immediately. Would you advise surgery if so I am advised the waiting list could be up to 70 weeks.

Please give your opinion on benefits of spine decompression clinics for bulging disc problems especially for people who are still suffering for over a year & many interventions tried.

Dr. Good morning. I have been following all yours videos. I got a protrusion on L5-S1 with this comments: para central disc protrusion with mild central canal narrowing but no exit nerve root impingement but there is contact with S1 never root bilaterally in the central canal. No para espinal lesions. What could you consider about it? Thanks.​@@HT-Physio

The best set of videos I've seen. Very helpful. In my case I had severe sciatica, then I started falling (7 times) until I was unable to walk without a walker. Then the muscle in my right leg refused to work. My doctor paid me a home visit and did several tests. One of which was he lifted my left leg and told me to hold it. I did that without pain or any problem. When he repeated the test on my right leg it fell to the ground uncontrollably. I have had both hips replaced one of them twice. Your thoughts?

Curious about size of herniated disc. For example, 10-11 mm disc bulge, stage 1, etc.if the size of the bulge shown in mri determines your opinion or if you go by just a person’s reaction to 7 signs that you mentioned. I wish I had seen this video earlier I might have waited a little longer before committing to surgery.

Super knowledgeable videos Dr. My MRI showed l4-l5 disc protrusion to the left and severe pain running down my left leg which was intolerable in any position so had my laminectomy -discectomy past a month now. But still the numbess is there in both toes and I feel a little sciatica pain behind the left knee sometimes actually very often. Are these the recovery symptoms or a possibility of reherniation? Please advise

Thanks, Dr. I have a L5S1 disk bulge and after onset of pain almost 3weeks ago, the only pain is near right hip. I can walk, kind of jog with no pain. And there is no sciatica. Do i still need to extension exercise or combine with flexion exercise? My ct scan report showed right foraminal stenosis. Thanks a lot. You are doing a great job for us.

I was offered surgery for my back but the gas man said he wouldn’t be comfortable doing any sort of procedure because of my being a long term lung patient, and I have since had bilateral denervation on 2 level’s but not the others, so it didn’t seem to work out well for me. So I’m now in limbo and even though I have got some pain meds, I haven’t had them upped in about 10-12 years and so they don’t work as well anymore. So what is someone like me supposed to do? GP can’t do anything for me and now I’m in pain almost 24-7. Any advice I would be very grateful for!!

My dude your content is superb, gotta say though I dodged your videos for like a week solely because of that over 50's note. Might be putting yourself in a niche accidentally Meanwhile though.. I'm a little lost on centralisation. Can it suddenly come on like a balloon popping? I was doing my usual routine and a few things felt a little bit dodgier on my back than they have the past couple weeks. During a quarter squat there were some little jolts at the originating site;I checked my posture was good and resumed. A couple squats later and it was like a sudden jolt or pop of pain that's been entirely absent for days. S1/L5 protrusion - nerve pinch, L5/L4 broad herniation into nerve sac but not pinched. So confused if good or bad lol. Lots of mentions about what centralisation around but not much of a what to expect 🤣 I'll probably find out soon enough but might be a good video idea

Dr i have a bulge disc on my thoracic spine. I don’t have any pain now but when i sleep on a little harder mattress next day i have a tight muscle at my back which makes me difficult to move. For how long should i sleep on a soft mattress?

Here just numbness in my foot and leg and sometimes pin and needless. No real pain. 6 months. I will now start with PT. I hope it will get better. It's bothersome.

Sitting and standing to much hurt the same. Even laying down can aggravate it :(

I have had a bugling disc since last year September 2022. I still have a little pain down my leg and mornings still a bit sore but I feel so much better now I meant my orthopaedic doctor this week with a positive attitude because I was hoping he would say a few more months of continuing physio I could go back to slowing getting back to training (CrossFit) But the orthopaedic doctor told me to forget about training with weights and CrossFit and choose another sport to do - which put a downer. I have seen many successful story of people going back to weight training or Olympic weightlifting after this injury - could you do a video on what you think about going back to Olympic weighting after a bugle disc 🙏

Right that’s how I feel I keep reagrivating my L5-S1 bulge and I have the pain in both legs and numbness in between my legs and I I’m not sure if I have bladder issues if I wake up every hour at night lol

Hello sir I'm 22 years old I've been dealing with herniated disc (l5-s1) for almost 3 months in the first it was a little pain in back and leg and was responding to medication then it got so severe any move was killing me then it became less but the problem that I still can't sit or walk with out pain in my thigh it gets painful as more as I sit or walk I have tingling in my two small toes but it's light i can stand on my toes when both feet but in one feet i can't stand on the toes of that foot not any movement I'm afraid that after all this patience I would have to do surgery Can you give me your opinion it's really this situation has missed up my mind Thank you so much

Do not get surgery unless its urgently necessary, its not a quick fix and you can end up in a worst position then you are currently in.
