For some embassies it is a requirement to apply for health insuarance and provide the document showing that you have it - that is before going for the interview at the embassy? Did you experience this?

Alright. The amount you pay depends on the type of insurance you use (public or private). Private customers pay as low as 30€ whiles public can start from 130€ upwards.
