My month of night float as an intern was by far the most difficult in my entire medical training journey. I left that month in a pretty deep depression. Trust me, you are doing great man. Keep putting 1 foot in front of the other, taking care of yourself first, and before you know it you’ll be putting intern year behind you and will gain massive experience and confidence.

I just turned in an assignment that I toiled over for 5 days, and then Andy drops. This must be my reward.

I just started med school at the MCG Athens campus. Before that I worked in an ER so I can really relate to this video. Your videos really inspired me! One of my facilitators told me "never question your decision to become a doctor during residency right after you wake up. It's a trap." I guess you're going through that now. I'm sure you'll get through it like everything else. You're a literal doctor now so take some solace in that fact.

Yes, do not worry about not being able to upload videos all the time. Priorities, man. Rest, whatever you have to do! While I'm not taking a career in medicine, I've been researching doctors, cops, marines, and others of The Not ALL Fun Jobs to learn firsthand what they have to deal with. Hey Andy, I hope you're doing well. Make sure to listen to your body and get the well deserved rest you need! I am also in the same boat as I currently just started my first night job as a nurse in the ER, and I must agree that trying to sleep at 0830 and wanting to sleep 8 hours is such a challenge. It can be hard and lonely at times, but just remember your 'why'. God bless and take care!

As a nurse for over 30 yrs, there are so many good and rewarding aspects of what you do. Persevere through the rough times because it will get better. My daughter is in her 3rd yr MS and hoping to go into anesthesia after having worked as a BSN for 4 yrs. I found your videos after she started her 1st yr and got her following you. Happy belated birthday to you! We are always with you along your journey. One of the hardest thing is to make the best decisions for the patients when we are totally sleep-deprived and tired. Proud of you and take good care of your health too. 🍀

Strong work. For nights, I found going to nurses taking care of your sickest patients is a good way of reducing pages and developing good teamwork. That reduces pages. Even rounding on your list and seeing nurses reduces pages. For upset patients, always reaffirm their fraustration, listen, and give an honest plan. You can't always win, but it'll reduce nurse fraustrations. If nurses are fraustrated, always reaffirm their fraustration before giving your perspective. It helps reduce hammer paging and feeling ignored by residents. The more you move around at night, the faster it goes. Make sure to do workouts before work on nights; sometimes the sauna is the quickest way to refresh. Nights are a good time to do studying and reading too. A good night is whenever no one dies, gets hurt, or any drama happens. It's a great feeling when you get a a sick patient through the night for the day team. I've always appreciated good night team care because days become easier. If you do a good job, patient's will always remember you as the night doctor they trust. If you get the angry family situation, always use the patient emergency to get out. Sometimes, families will never let you leave unless you do so first. It's a sad reality, but it does happen. The more you make nights like days, the more you'll find patients appreciate your care. There's a lot of BS in the system. The truth is that residency isn't about being fair. Again, it's not right, but it's a reality I've learned as a PGY2. What you can do is take care of business and make sure in your heart you know that no one was hurt because you got complacent or lazy. My favorite moments is doing a good job overnight and standing out as a resident that didn't do BS work or wasted time. Your reputation will help other people do some extra favors to help you out. What goes around, comes around. Make sure it's always good.

Oy! After all these years of vids from med school, this one was brutal. Andy, I'm so sorry that you experience this degree of fatigue. You do note being able to deal as the doctor you are with cases that might, previously, have felt much more challenging. You're growing in your role. Get some sleep. Blessings and prayers.

I feel ya, night shift is pure torture!! At least you can hopefully sleep a bit, as a nurse you are awake and on all night long, and you have to actually to physical labor and be able to think clearly for 12 solid hours. I will NEVER work nights again as a nurse!!! You will get through it (mainly because you have to) , eat healthy (that will help) and try and find a routine for when you are on night for getting sleep during the day! Happy Birthday as well!

Would know a total knee anywhere, LOL! Hard to keep up your wellbeing in residency but glad to see you being vulnerable and trying your best. That’s really all you can do.

Yes, it is brutal but you signed up for this. U.S. resident training is hard and changes should be made. If you’re going to complain work to make meaningful change.

How does call week work if you’re sick? I assume you gotta show up anyways haha but is there PTO, or backup residents for call shifts?

Here where I live, you cant get fired for consuming medical marijuana on the job. Which can be prescribed for insomnia. The only way I can handle 16 hour sessions of psychoanalysis without actually getting up one time. Inpatient psych is something else. We love tangentiality

As a first year, when you’re on call, are you expected to do things completely alone? Like do you get to text your seniors and ask questions? I’m just really intimidated by the idea of taking such a responsibility so early on my training period 🫣 (applying to med schools now so we will see if that even becomes my reality..)
