Overview -
Abortion: The medical or surgical termination of a pregnancy before the fetus can live independently outside the mother's womb. It can be spontaneous (miscarriage) or induced, depending on the circumstances and reasons.
Types of Abortion
Spontaneous Abortion (Miscarriage):
Occurs naturally without any medical or surgical intervention. It is often due to chromosomal abnormalities, maternal health issues, or uterine anomalies.
Induced Abortion:
Medical Abortion:
Uses medications like mifepristone and misoprostol to terminate an early pregnancy (usually up to 10 weeks). The drugs cause the uterus to contract and expel the pregnancy tissue.
Surgical Abortion:
Involves procedures such as vacuum aspiration or dilation and curettage (D&C) to remove the contents of the uterus. Surgical abortion is typically performed in the first or second trimester.
Indications -
Therapeutic Abortion:
Performed for medical reasons, such as severe fetal abnormalities, maternal health risks, or life-threatening conditions like preeclampsia or cancer.
Elective Abortion:
Performed at the woman's request, for reasons that may include personal, social, or financial considerations.
Legal and Ethical Considerations -
The legality of abortion varies widely across different countries and regions, often influenced by cultural, religious, and political factors. Ethical considerations also play a significant role, with debates surrounding the rights of the woman versus the fetus.
Procedure and Management -
Medical Abortion:
Involves a combination of medications taken in two stages. The first medication, mifepristone, blocks the hormone progesterone, which is necessary for pregnancy continuation. The second medication, misoprostol, induces uterine contractions to expel the pregnancy tissue.
Follow-up: After a medical abortion, a follow-up visit is crucial to ensure the procedure was complete and to address any complications.
Surgical Abortion:
Vacuum Aspiration: A procedure where gentle suction is used to remove the pregnancy tissue from the uterus. It is typically performed up to 12 weeks of pregnancy.
Dilation and Curettage (D&C): Involves dilating the cervix and using a curette to scrape the uterine lining, typically performed in the second trimester.
Post-Procedure Care: Includes monitoring for signs of infection, bleeding, or other complications, and providing emotional support and counseling if needed.
Complications -
Immediate Complications:
Excessive bleeding, infection, or injury to the uterus.
Long-term Complications:
In rare cases, repeated abortions may lead to cervical insufficiency or Asherman’s syndrome (intrauterine adhesions).
Psychological Impact:
Some women may experience emotional distress, guilt, or depression following an abortion, requiring psychological support or counseling.
