Cause-
Main Cause-
Gastric acid hypersecretion {Acidity - due to Helicobacter Pylori (H-Pylori)}
Obesity, alcohol, Smoking, stress, tobacco, Spicy food
Infection
Reduced Length of intra abdominal esophagus (inversely proportional to Acid exposure)
Reduced LES Pressure (normal pressure= 10-20 mm/Hg, During swallowing pre. drop 1-3 mm/Hg)
Sphincter pressure-
Increased By- Protein meal, Alkalization of stomach
Decreased By- Fatty meal , Chocolate, Gastric acidification
Clinical features-
Triad= heartburn + Regurgitation + Epigastric pain
Typical symptoms= Triad + Odynophagia, Dysphagia, Hematemesis
Multiple ulcer
Hoarseness of voice (Laryngeal symptoms)
Chronic cough, Shortness of breath
Post-pardinal fullness
Recurrent Pneumonia
Symptoms increase in supine position

Esophagoscopy
X-ray(Barium meal)
Urease test- H. pylori
Esophageal Manometry
Mucosal biopsy
Treatment-
Conservative- Analgesic
Antibiotics (Amoxicillin)
proton pump inhibitor- Omeprazole
antacid- mg+ trisilicate
H2 blocker- Ranitidine
Prokinetics drug- Domperidone, Metoclopramide
Eradication therapy- Avoid Spicy food , Smoking, Alcohol & frequent meal
Life style change
