Many cases of AKI could have been prevented by timely interventions in the community before things got so bad that hospitalisation was needed eg recognising and treating infections early ,avoiding or stopping anti inflammatory and other medications and being alert for dehydration and acute on chronic retention of urine due to prostate enlargement .

Totally agree with you..I see lots of ED patients who eventually develop AKI requiring RRT later.
