We have the Toyota part the screws into a pre-existing threaded hole. We had to have the shaft shortened and threaded because the wheel is inverted to contain the items depicted in the video., and the shaft was too long. The plastic handle snugs into the large hole of the wheel when it is tightened. You can see the "hold down part" and the threaded hole in the thumbnail image.
I had the opposite problem! The retainer for my spare tire was too short for my full sized spare. The hard part for me was determining the diameter & thread pitch of the bolt I needed. I was lucky to find a retainer on Amazon that fit. I also got a pinch weld protector, something I never knew about before.
