Addison’s disease, also known as primary adrenal insufficiency, is a long-term endocrine disorder in which the adrenal glands do not produce enough steroid hormones.[1] Symptoms generally come on slowly and may include abdominal pain, weakness, and weight loss. Darkening of the skin in certain areas may also occur. Under certain circumstances, an adrenal crisis may occur with low blood pressure, vomiting, lower back pain, and loss of consciousness. An adrenal crisis can be triggered by stress, such as from an injury, surgery, or infection.[1]
Addison's disease arises from problems with the adrenal gland such that not enough of the steroid hormone cortisol and possibly aldosterone are produced,[1] most often due to damage by the body's own immune system in the developed world and tuberculosis in the developing world.[2] Other causes include certain medications, sepsis, and bleeding into both adrenal glands.[1][2] Secondary adrenal insufficiency is caused by not enough adrenocorticotropic hormone (ACTH) (produced by the pituitary gland) or CRH (produced by the hypothalamus). Despite this distinction, adrenal crises can happen in all forms of adrenal insufficiency. Addison's disease is generally diagnosed by blood tests, urine tests, and medical imaging.[1]
Treatment involves replacing the absent hormones.[1] This involves taking a corticosteroid such as hydrocortisone and fludrocortisone.[1][3] These medications are usually taken by mouth.[1] Lifelong, continuous steroid replacement therapy is required, with regular follow-up treatment and monitoring for other health problems.[4] A high-salt diet may also be useful in some people. If symptoms worsen, an injection of corticosteroid is recommended and people should carry a dose with them. Often, large amounts of intravenous fluids with the sugar dextrose are also required. Without treatment, an adrenal crisis can result in death
The signs and symptoms include fatigue; lightheadedness upon standing or difficulty standing, muscle weakness, fever, weight loss, anxiety, nausea, vomiting, diarrhea, headache, sweating, changes in mood or personality, and joint and muscle pains. Some patients have cravings for salt or salty foods due to the loss of sodium through their urine.[8] Hyperpigmentation of the skin may be seen, particularly when the patient lives in a sunny area, as well as darkening of the palmar crease, sites of friction, recent scars, the vermilion border of the lips, and genital skin.[9] These skin changes are not encountered in secondary and tertiary hypoadrenalism.[10]
On physical examination, these clinical signs may be noticed:[8]
Low blood pressure with or without orthostatic hypotension (blood pressure that decreases with standing)
Darkening (hyperpigmentation) of the skin, including areas not exposed to the sun. Characteristic sites of darkening are skin creases (e.g. of the hands), nipple, and the inside of the cheek (buccal mucosa); also, old scars may darken. This occurs because melanocyte-stimulating hormone (MSH) and ACTH share the same precursor molecule, pro-opiomelanocortin (POMC). After production in the anterior pituitary gland, POMC gets cleaved into gamma-MSH, ACTH, and beta-lipotropin. The subunit ACTH undergoes further cleavage to produce alpha-MSH, the most important MSH for skin pigmentation. In secondary and tertiary forms of adrenal insufficiency, skin darkening does not occur, as ACTH is not overproduced.

I'm here because an *endocrinologist* just tried to take me off prednisone. I told her I'm pretty sick from being on only 6.5mg, and think I need more. She said I'm fine and don't even need a follow-up! Thing is.. she's my 6th endo and one of the best I've found. Why is it like this? And yeah, in the ER they usually try to withhold steroids, disregard my Medicalert bracelet, it's a mess. I've had to sneak my own hydrocortisone out of my bag.

@@howtomeetwomen- I understand completely! It's not only frustrating, but dangerous. I carry an article with me that I printed to try to open their eyes. Even my PCP said she knew practically nothing about Addison's. Very disheartening.

* Firstly, am sorry for my bad English because English isn't my primary language. Great video. I've been suffering with all the addison's symptoms (Not fully diagnosed yet) for quite a long time. I'm craving salt, losing a lot of my weight, hypotension problem and more... Well, I also have Type 1 Diabetes (currently using an Insulin Pump) and Thyroid Disease. So, this video has opened my eyes about other autoimmune disease that probably manifest in my body. Thanks for the video because it's gonna save my life.

Most times 1.5 tab of Hydrocortisone is not enough. Summer time is increased to 4 TAB a day or during high stress. My question is how many patients have a DEFIB or what is the standard?

Good description sir but I d like to point out one minor mistake if u don't mind...U said androgen deficiency produces loss of libido in both sexes but actually it affects only females because males have other sources of androgens like testes so they are not affected... Thanks

I had a blood test that shows I have anti adrenal anti bodies with a titer of 1.10. I was tested since I have a lot of the symptoms minus the skin stuff. My cortisol was actually high at 23.1... could I still be feeling symptomatic of Addisons? It’s very confusing

Great video. Thanks for the upload. I've had Addison's for over 20 years. I believe the dosages for medication is different for every patient. My dosage is not as shown in this video. And I do very well. It is also important. To take more fludrocortisone before any medical procedure even dental work

your video is really helpful! one thing i noticed, though - for the ACTH stimulation test, you should give 250 micrograms (0.25mg) of ACTH, not 250mg, that's so much! also, i believe cortisol is usually measured in micrograms/deciliter.

I was diagnosed with Addison's about 7 years ago and wasnt suffering one of those symptoms. I was told by the Dr is was a stress management condition and I was having a few panic attacks at the time so just believed it. I have been taking hydrocortisone and fludrocortisone for the past 7 years every day. I think im going to get a second opinion.

@@d.indira7548 when I am ill and by body of fighting an infection then I am required to double my daily dose of hydrocortisone. This drives my blood sugar levels up so it is very difficult to manage my type 1 diabetes when I am doubling my steroid dosage.

Indeed, I actually have this disease and I need to take hydrocortisol as a medine 3 times a day. Since I have started taking the hormones, starting 3 weeks ago, I have felt healthier than ever. Of course, I cannot say yet how it will develop.

My mom was been treated pituitary adenoma by surgery and then radiotherapy. Several years later her pituitary gland seems to shrink in function , not able to secrete ACTH , and cortisol level is too low . She got the symptoms of heaviness in chest, sometimes stomach and is not feeling fresh and energetic . She was initially taking prednisone with dose of 5 mg in the morning and 2.5 mg in the evening . Since the symptoms does not subside, or increase more , doctor prescribed 20 mg in morning and 10 in morning for a week . And now she is having moon face and. Still not fresh and energetic. Please help those who knows

Atindrila Dey There is no known cure unfortunately and a patient can lead a relatively normal life but this is achieved with various medications it stops adrenal crisis from happening which can be fatal I hope this helps you with your question

very clear presentation. thank you.. am on hidrocortisone replacement. since i got this illness so late in life, am hoping it will be temporary.. this disease is awful and is affecting many of my systems and has caused weight gain. still not sure if cause was over use of steroid injection or severe head trauma, perhaps affecting pituitary.

Can you get Adrenal fatigue from head trauma? And have weight gain? Because this has been my experience but no diagnosis. In fact, the endocrinologist told me there’s no way I could have adrenal fatigue or failure because I was overweight. I think I was overweight because I couldn’t move for years and ate to feel better. (Which didn’t work ) Now I take adaptagenic herbs,Cordyseps mushrooms, vitamins and freeze dried cow Adrenals (found at the health food store) and I can at least function on some level. But without these I can’t function at all. The Video was very helpful though. Thanks

@@shaek6820 No such thing as Adrenal "Fatigue". Ask your Endocrinologist to perform a morning ACTH Stimulation test. Have an Addison's antibody test done. Get imaging done of your Pituitary & Adrenal Glands. Yes, u can develop Adrenal Insufficiency if your Pituitary or Adrenals were damaged by trauma/hemorrhage/infection. These tests will confirm whether u have Adrenal Insufficiency (AI) or not and need lifelong steroids. It will also differentiate whether u have Primary or Secondary AI.

in my country Norway we are 5 million people i read that there is only a little bit more than 1000 addison's damn yeah the sleep is true i sleep 4 to 6 hours and i struggle to fall asleep as quick as my parents : P they fall asleep within 1 hour i can spend up to 3 hours before i fall asleep

Adrenal gland is not the primary source of sex hormones.Testes and ovaries are the main source.So even if adrenal gland is insufficient the ovaries and testes compensate the losses of sex hormones.

Don't get too attached to labels, and don't be manipulated by so-called "authority figures". Do your own research, and heal yourself. You don't want to be a regular patient in the medical mill. Check out Pam Popper. I realize she's a little older, but oh well, think of her as a loving grandmother with a lot of wisdom. Good luck.

why did you say anorexia as a symptom. anorexia is a MENTAL disorder in which the patient doesn't want to eat to control food, to feel better about their bodies and keep control. I don't see how anorexia is related at all. you mean low appetite?

Medically anorexia just means loss of appetite. 'Anorexia nervosa' is the mental health condition where people dont eat due to perceived body image issues, but we have come to refer to that as just anorexia in daily life.
