The approach to the deteriorating and arrested patient is a critical part of every doctors education!
In this video, our ALS coaching team steps through how we would approach the deteriorating patient who arrests.
This demonstration is with only 2 helpers as consistent with our final test for the ALS trainees. While this is a bit artificial from a normal resuscitation it helps us to demonstration the technical skills and flow of an arrest.

9:58 why is that a non shockable rythm ? Isnt that still a vf ? Or is it VT, which is why she ordered to take a pulse ?

some long awkward pauses on the CPR there - @ 7:20 i count around 22 seconds without CPR - as soon as that shock is delivered the leader needs to direct someone back onto the chest immediately Again at 8:18 delivering the shock - around 10 seconds before instructing the compression provider much better @ the 9:25 mark with recommencing Good effort from this lady though - these simulations are the places to learn It is easy sat watching from the comfort of my chair to criticize xD

Please correct me if I’m wrong, it’s for my clarification too, but at 9:30 are you supposed to give another 1mg of adrenaline since it’s after fourth shock? Thanks

She should have requested adrenaline after 3rd shock, so the next one would be after 5th shock. But you are right, it is given after every other shock.

I think adrenaline after 2nd shock then 2nd cycle and amiodarone after 3rd shock

Pad application is not normally placed when patient is conscious.

if you are worried about a patient who is unstable putting the pads on can be appropriate especially if they look unwell.

At 4:55 there’s a 15 second period with no compressions taking place. That could be improved upon. Immediately after the shock is delivered, glance at the monitor (should be able to interpret a 4 lead instantly on sight) and jump right back into compressions if it’s still an arrest rhythm.

Rather than assessing the rhythm, you would be going straight back onto the chest after CPR, unless the patient is showing signd of ROSC. Even if it sinus rhythm, it is the patients breathing/conscious state that dictated CPR, not the ECG waveform.

Yes correct, ARC guidelines… keep Oxygen connected for closed circuits (LMA, ETT). Remove for open- mask ventilation. :)

Its a simulation with minimal help. In a real hospital emergency you'll have people flocking to help. The policy here is adrenaline after 2nd shock and amiodarone after 3rd shock - exactly how it was done in this simulation.

Might want to look up the ARC guidelines before attempting to gasbag a well thought out yt video somebody has taken the effort in uploading Amiodarone 3rd shock ✔️ Adrenaline after 2nd shock ✔️ There are literally 2ppl there for CPR. This is a hospital sim, not some roadside assistance so of course there will be more hands on deck for CPR

1 mg of Adrenaline and 300mg of Amiodarone should have been given after the 3rd shock. 😬

That might depend on Guidelines. The ANZCOR guidelines and Australia/NZ recommend Adrenaline 1mg after 2nd shock and Amiodarone 300mg after 3 shocks.
