AFib in the young

Introduction
Most of the AF we see occurs in older patients - usually those above the age of 60 and therefore much of what we know about AF comes from studies involving older patients. What we are however beginning to realise is that AF can occur in younger patients too and perhaps this group of patients represents a group that we do need to study and research a lot more because what we understand about the causes of AF and the disease course may not be extrapolatable to younger patients.
The prevalence of AF in the young is significantly lower - in the order of 0.5%. Whilst we blame age and comorbidities such as diabetes, hypertension and obesity in the elderly, the same comorbidities are not seen to such a great extent in the younger patient. In these patients there are thought to be three main possible causes which are always worth thinking about.
The propensity for AF could have been inherited
The AF could be the first manifestation of silent cardiac disease (i.e if the heart muscle were in someway abnormal (very difficult to test for unless it looks abnormal) then it is more likely to be irritable
There may be an abnormality of the ionic channels within the heart which regulate the electrical currents within the heart. This is termed a channelopathy. Common channelopathies include Brugada syndrome, Long Qt syndrome etc.
I was very interested in understanding what we already know about AF in the young and i found an interesting paper on this subject published by Wutzler et al in 2016. A reference for this paper will be available on my website.
Wutzler et al studied 124 patients who were below the age of 35 years when they first presented with their AF and followed them up for an average of 4 years to see what happened to them. They noted some interesting findings:
57% of this group of patients did have some comorbidities or underlying structural heart disease. 20% of patients had an underlying cardiomyopathy. So the first important message is that it is important to see a cardiologist and for him to look hard for an underlying cardiomyopathy and address any comorbidities.
None of these patients had a stroke during the follow-up but it is also worth noting that 36% of the study group were already taking an anticoagulant. Hence the second point is that until we have more data, it is recommended that patients who have comorbidities or structural heart disease should still take an anticoagulant.
No-one dropped down dead suddenly so it appears at least from this study that AF in young patients does not confer an increased risk of sudden cardiac death.
All the patients who had a cardiomyopathy were noted to have it at the beginning of the study. None of the patients developed it during the course of the study. Additionally none of the patients were subsequently diagnosed with a channelopathy either.
Almost 57% of patients in this study were also found to have an incidental SVT when they underwent more detailed electrophysiological studies. Most of them had been referred for AF ablation but during the study found to have extra electrical pathways which are why SVTs happen. Very interestingly, when the SVT pathway was ablated (rather than AF ablation), no further heart rhythm disturbances were noted during follow-up suggesting that in some way the SVT was also responsible for the AF.
This is a very small study but hopefully the findings will help inform scientists to design bigger studies but in summary, the findings from this study are that young patients with AF are not uncommon, need thorough investigation by a cardiologist and ideally an electrophysiologist (including electrophysiological studies to look for extra pathways) and should be anticoagulated if they are found to have structural heart disease or significant comorbidities. In addition it is always a good idea for all patients to address any lifestyle issues to try and become healthier people.

I got my first AFib in my twenties now I am 56. My last episode was a year ago exactly whilst riding my bike, coincidentally my first episode of AFib happened when riding my bike. After watching you video on gastro cardiac syndrome it was a major turning point for me in regards to AFib. When visiting my cardiologist I keep telling him when I get AFib I feel I have digestion and pressure coming from my abdomen and causing constant Palpitations. After much research I watched a doctor talk about vitamin D deficiency and the symptoms, particularly digestive issues, which I constantly suffering from. So I started taking vitamin D supplements, even though I live in Australia and ride about 300km per week. I can not tell you difference it has made to my life! My palpitations have reduced significantly and my heart burn is nonexistent now I feel I can train and race a lot harder and feel confident that I will not go into AFib. Finally I also believe that going on a whole food plant base diet 7 years ago has make a difference but the vitamin D supplements was a games changer for me. Thank you.

My story is oddly similar. Bread would bring it on in my 20s. Sometimes exercise would bring it on. I'm now 57 and I rarely has be this happen. I still eat bread but my diet as a whole is clean.

Can an AFib be caused due to beta blockers ? Do you have acidity like burns while having an AFib ? Where do you usually have the discomfort ? Is it on the right side of the chest ever ?

do you have premature atrial complex pacs? i mean do you get pacs before you go into AFIB? because i have pacs every day around 200 a day but my heart is normal and i am 31 years old male

@@abc_cba Beta-blocker helps to bring me out of AFib. Usually takes about an hour to take effect though!

talal tallal never of heard of pac! But I find that when I have reflux issues that I am vulnerable to have a AFib episode. However as explained in my post taking vitamin D supplements has eliminated my reflux issues and consequently, touch wood, my AFib reoccurrence. My experience is only anecdotal but I feel confident there is something to it.

Thank you. I had paroxymal AF at 27, for five years. No heart abnormalities were found in the several ultra sounds or other tests. I'm now symptom free for 3 years after changing my lifestyle, supplémenting with vit c and mg and Intermittent fasting.

Just wondering, what were your episodes like? Mostly a fluttering feeling? I'm not sure if I have Afib or not. I may have had a handful of episodes, they are usually a 10-15 second string of flutters

@@tear728 it was several hours, up to 24h. These long episodes were happening once a year but short flutters like 5s to 10s quite often

I'm the exact same! Being diagnosed at 27 that is. In march this year I went into hospital with fast afib (very scary) I still get flutters but I'm worried about a possible underlying condition. Any advice on lifestyle changes to help combat afib?

My husband was in his 40’s when he was diagnosed with intermittent AF. After lots of research I started him on iodine with iodide. His episodes are very rare now. His heart is structurally sound.

Thank you!!! Nearly 2 1/2 years ago I had a severe instance of a fib that Converted on its own about 17 hours later. I have not had a diagnosed issue since then. I have vasovagal syncope issues that predate the afib by many years. I have found your videos quite helpful and informative.

I got AF at about 45 and I’m now 64. I had an ablation this January after 19 years on medications. So far, so good. I never (well so far) progressed beyond PAF thankfully.

Gloria Mary Haywood my EP instructed me to stop Flecainide after 2 weeks and Sotalol after another 2 weeks. So now I’m only on hpb medication and apixaban . As I said so far so good 15 weeks post ablation.

One more question, please: About how often did you use to experience AFib? Mine went from one episode every few months, to about once a week recently. Thank goodness it seems to have settled down again. Usually takes from around 30 minutes to an hour to get back into normal sinus rhythm. (BTW....It seems kind of strange to me that you are still taking a blood thinner?🤔)

Gloria Mary Haywood I was getting an episode on average every 4 months but had a run of 3 in a week last summer which is when we decided to have the ablation. I’m still on the blood thinner as I have HBP.

I was recently diagnosed with paroxysmal atrial fibrillation at 26 years old. Chest X-ray, echo, blood work, EKGs, and finally an EP study ruled anything abnormal and SVT, but just AF. I've learned that really cold food and drink trigger my atrial fibrillation, which can last from a few minutes to a couple hours. I underwent a cryo ablation for AF about a year ago, and have experienced three AF episodes since, although they were fairly short-lasting. Since my first episode, I'm on on metoprolol, baby aspirin, and fluoxetine. I've cut out alcohol, exercise 3x weekly, and try to stick with warm-to-room-temp foods/drinks. I still experience PACs and such every now and then, which seem to get worse as I think about them more. So far, so good. I'm not entirely sure how my AF will evolve as I age, if the triggers will change or it becomes persistent, etc.

Sheesh. I got put in metoprolol short term and stopped taking it after a week of very bad sleep, extreme lethargy and chest pains. Lifestyle factors are magnitudes more effective than that poison for me. And that was with just 25 mg once a day instead of the 50 twice a day they prescribed me

My heart first went into afib when i was 33 at a hospital as I was walking in. It’s an unmistakable and horrifying feeling. Mainly doctors are only interested in old people’s heart problems. Take aspirin and cross your fingers. I love this doctor here, he is very studied and knowledgable on current research. Thank you!

pramod bhookya it’s like you can feel your heart is vibrating instead of beating and takes your breath away. You will panic if you ever have afib. Mine was confirmed on ekg.

I presented to A&E with SVT 10 years ago when I was 20. I was ablated twice and now I have been on flecainide for 10 years and have very occasional bouts of AF that always right themselves within hours with chemical cardioversion. I exercise a lot (run a lot) don’t be afraid people embrace it and learn about it. :)

This extremely insightful, helpful and well explained. Hopefully, good news for young with Afib. Just a minor correction: you mention in 2:47 that 57% of the patients had comorbidities, the actual number referred in the paper is 40% (you may have gotten confused with the of patients having an SVT, which you correctly mention at the end of the video).

Great videos and I like how easy they are to follow. The medical terminology is explained really nicely! I'm surprised to see such a thorough series of videos and no mention of Taurine and L-Arginine supplementation for stabilizing heart rhythm and reducing/eliminating PVCs. I use this combination to control my PVCs as do a number of my friends and family members. We all swear by it as it seems to stop PVCs within 2 hours of taking them. I'd be interested to hear your thought on this and your interpretation of the studies that seem to prove this.

its scary im 36, always had high bp thats been controlled since i was 18 and rarely missed work, ever. Had afib since my 2nd moderna shot in april 2021, 3 days after ive had numerous unexplained afib for about 3 months now, almost 6 times i had to pull off the road n call off work n had severe sob and numbness/tingling all over my body, almost fainting. had to call off work for about a month. 2 er trips and 1 day hospitalization, see a psych md now thinking its panic attacks and a cardiologist who is now referring me to an electrophysiologist. the longest ive been w/out an afib/tachycardia episode is now like 2 weeks but just got it again this morning... did a 24h holter w no success, now debating if i should go thru with a 28d event monitor or not, and whether i should see my electrophysiologist or not, random afib/tachy is the worst, and im really scared if they suggest i get an ablation which my cardio suggested i may need... stress test and echocardio shows im 100 percent healthy but i still have these episodes i hate it so much

The first time I had afib I was 19. It lasted for about 3 days. They checked into the hospital for a few days testing me. It went away and I did not have any issues again until I was about 29. Again, it went away and I had no issues until I was 39 (the nines seem to be bad for me). At 39 it came and went off every day for months. I've been on flecainide/diltiazem since (17 years). I have done multiple stress tests, nuclear stress, CT with contrast, etc. and there does not seem to be damage or other issues.

I had afib at 28 that lasted a few hours it was scary and I woke up to it. Felt like my heart was knocking in my chest and I felt very weird and kinda lethargic. By the time I got to the hospital it vanished. After all the tests etc holt monitor and echocardiograms I found out my heart was very healthy and it was said it was caffeine and nicotine induced and episodic. So careful to smokers and caffeine lovers

If you haven't already, I'd advise looking into immune system induced AFib, specifically the action of histamine on H2 receptors in the heart. It can cause inotropic and chronotropic disturbances. When H1 antagonises and/or H2 inverse antagonists are administered, the incidences of AF and other irregular beats (ectopics) are reduced considerably. I can't link any studies here due to RU-vid blocking URLs.

@@maeradelgemao5954 Not necessarily. It depends if it's the main causal factor or not. Since I have many food allergies and sensitivities, anti-histamines work well to reduce the incidences of irregular heartbeats. It doesn't stop it completely, though.

I wonder if there's a correlation with leaky gut and gluten sensitivity in the gut causing inflammation and triggering signal to the vagus nerve and flammatory response in the body effect in the AV node and throwing the heart out of rhythm I noticed if I ate a half of a Subway sub within 3 to 5 minutes I'm in afib certain foods would cause inflammation throw me in AFib I have I was 40 years old when I had my first episode had a cardio ablation two years ago successful did change my diet and I'm gluten free your thoughts thanks Brian

I’m 31 and had a afib incident in the morning and since then I have shortness of breath at night when I’m trying to sleep and into the morning and I can “feel” the a-fib coming in the morning it’s like a feeling of dread

I'm a super fit 47 yr old. I've been diagnosed with SVT, but occasionally have events that feel slightly different (more of an irregular beating very fast)..maybe AF. My question is this...it usually occurs when I lie down. Also have gastro issues with it, like pressure. I eat before 6pm and go to bed at 11pm or 12am. It is really confusing. Why would posture make my arrhythmia act up? I've been tested and do not have sleep apnea. Also had a full GI work up...everything is fine. I miss sleeping...it doesn't matter which side I lie on. I have to literally sit up for hours until it decides to calm down.

I got AF at the age of 44. I have a Mytral Valve Prolapse and not sure if that was the cause. I’ve been active all my life and was very healthy otherwise. I’m currently 46. I actually had a small stint of SVT when I did a stress test about a year before my first AF episode.

I'm 30 and I first had a fib after minimally invasive mitral valve repair. I had to have an ablation and it fixed it. After the valve repair failed and I went into mitral valve stenosis, I had to have open heart surgery and got afib again. This time I had to have a double ablation. One for the fast heart rate and one for the skipped beats.

I was also diagnosed of AF when I was 35, turns out that I have RHD with Severe Mitral Valve. I was hospitalised for TIA. Now that I'm 50 I am dealing with low heart rate and skip heart beat sometime.

But I have anxiety and depression badly. is this my anxiety and depression cause me to have this. are they related? . please reply.

@@maeradelgemao5954 Hi Maeradel. I've had episodes since I was 14 years old. I also did echo and holter and they did not find anything . I also thought that it could be anxiety and I was living with it for many more years,. The symptoms were getting worst and now at 28 yrs old went back last week to get checked . THIS TIME, right away they noticed my arrhythmia and I got asked to use the Holter for a day, after doing so, the cardiologist told me I do have Atrial Fibrilation. I hope you find out what is wrong with you and wish you the best of luck

Got my first Afib when i was 13. Now im 35. Still occasionally happen. Maybe once a couple year. I do have occassions where my heart tends to go afib when after short rapid weird misfires it gets back in sinus. So im not actually going into afib. One time in 2012 my cardiologist saw an brugada signal on my ecg. This brugada signal was never found after that. Also did an ajmaline test and that was negative as well. So basically it is still unclear what my underlying issue is. Once a year i visit the cardiologist for a checkup. I do have sleep apnea btw that was untreated for years. Since i sleep better i get less rhythm issues. So get yourself checked for sleep apnea. A lack of sleep is a major trigger for me.

do you have premature atrial complex pacs? i mean do you get pacs before you go into AFIB? because i have pacs every day around 200 a day but my heart is normal and i am 31 years old male

talal tallal PACs are quite common in people with afib. I have pacs and pvcs. The latter is mostly cause of panic disorder. Occassionally multiple pacs in a row is common for afib. It can result in afib.

Sanjay, do a video on pain medications and heart pain. I kept complaining to my cardiologist about heart palpitations and pressure. He said nothing was wrong with my heart. I tolsd him I took Diclofenac and he told me to stop. I did and all the problems went away!

Question if your Magnesium levels are in the normal range should you start taking it??? I was told to much can cause palpitations.

Started taking Magnesium two weeks ago because I read where most people don't get enough mag in their diet.....The other day I woke up at 3am with severe heart palpitations that lasted for about 3 hours and then stopped. I'm almost certain it was the Mag. I haven't had a problem since stopping and throwing that crap in the trash. Just my 2 cents.

I'm scared of a fib. I have a heart condtions, one I cant find anyone with the same , all 4 valves are backflowing (mitral, aortic, tricuspid and pulmonary valve ) do u have any info on all 4 backflowing at the same time .

Had first episode 3 months after having baby.. 1 year later 3 weeks ago had another episode and I was finally diagnosed with SVT with AFIB..

Hey.. Dr. You tend to explain everything so well I'm 19 I have a question My 24 holter test showed that I'm suffering from SVT so i feel fast and abnormal rhythm and many other symptoms. I feel pressure when i lay down.. Also the most important thing i want to ask is I feel a hard drop kinda feeling in the centre of my chest.. Sometimes it just happens once sometimes more than once and it's horrible and happens anytime Can you help I feel so anxious and cry sometimes because it's so horrible All other tests are normal including echo

Argh I have this sensation sometimes too, for me it's often at night or when I'm relaxed, heart drop I call it, followed by a little mini surge feeling like a small adrenaline surge. It's not pleasant! I'm due to see a specialist in August for svt episodes. Can't wait to find out more and get sorted.

U just do breathing exercises like pranayama, square breath, fire breath etc. Breath is the key.

Hi I'm 31 year old and I diagnosed with loan AFib in my age of 22 only. may be I have AFib before that also. Recently I have done health check up and it's found that I have vitamin d dificiancy and vit b12 dificiancy . Just wanted to know I have the Loan AFib due to vitamin d dificiancy? Thanks for the videos I’ve just recently found your channel. I had afib in my late 20s followed by an attack 12 years later. So far I’ve had an x-ray, bloods tests, an echocardiogram, holter monitor tests and a treadmill stress test undertaken while under NHS care with nothing found. I’ve never been referred to an electrophysiologist to check for any abnormal electrical pathways. Can and should you request this through your GP? I’ve never felt my afib has been properly investigated to find the cause. Thanks again for sharing your knowledge and experience 👍🏻

Me too!! Lately I’ve been having shortness of breath and been worrying about my heart so much that i think i make it go fast cause i get really nervous so I constantly get heart palpitations. a few weeks ago i went to the hospital cause i had a very made panic attack and they checked my blood, lungs, and did a ekg and everything came back fine but I’ve been feeling really anxious from the moment i get up to the moment i go to sleep worrying if i have something and literally the whole day its me having shortness of breath and diagnosing myself with every heart problem. Like i wanna believe its just anxiety but then i overthink and say like what if its actually serious but I’m convincing myself its anxiety? Ugh it sucks cause i also feel fatigue too but I don’t know if that’s anxiety or me worrying so much that it’s exhausting me. Honestly I don’t know anymore

Hello doc. My husband had angioplasg done with three stents two years back. Now he is fine with regular medicatiions n fitness. He yawns a lot as soon as he lies down. Then it subsides say after ten times of yawning which makes him tired..it happens almost everyday when ever he lies down to sleep. This complsint started only after a month of his angioplasty . Otherwise everything is normal. Kindly tell me the reason for this.

Jewlz4ever hi jewlz, so far so good . I haven’t an palpitations and no signs of a fib. I actually feel better and with losing the weight and changing my diet I hope it stays that way but glad I did it. The procedure was great and had no issues I went in the morning and was discharged that evening. The only issue I had was with water retention because they pump you up with fluids and some growing pain. I also was stiff from laying down on my back but wasn’t that bad looking back. Just had to distract myself with tv after

hi dr gupta, I am watching from the philippines. I have this when I was 9 , and now I am 20 . But i have holter monitoring, and found out that it is normal. I have to de echo and its normal. But I have anxiety and depression badly. is this my anxiety and depression cause me to have this. are they related? . please reply.

@@maeradelgemao5954 I just found out that I have this last Wednesday. So this is pretty new to me. Everything I read says to keep up your electrolytes ie magnesium and potassium and keep a healthy weight. But I would ask your doctor. But if I find out anything I'll definitely let you know. Sorry you have to go through that at such a young age. I'm 34.

I believe that magnesium deficiency certainly could be one of the causes. But I think there are different triggers for different people. I truly feel that mine are related to the vagus nerve over-riding the sinus node signals. If I am in Afib I can usually have success in going back into normal heart rhythm by pressing down firmly on my abdomen, right below where my ribs meet. I've had a total of 3 Afib episodes so far, the first one being last August. And I noticed that all three episodes happened when my stomach was nervous and I felt jumpy inside.

This is merely anecdotal but I have been taking magnesium regularly, do not have a deficit and have had afib since my 40s. I have no comorbidities and have exercised regularly, ate healthful food my entire adult life. My thought is in our population cohort, there are more variables contributing to cause.

It has many causes. Iron deficiency, diabetes, electrolytes, hormonal issues such as low dhea, narrowing arteries, etc.

Look for his videos on ectopic beats. From what I understand, unless you are having around 25,000 ectopics a day and have had your heart confirmed to be strong, there isn't much to worry about. Most people have ectopics
