I'm a dental technician, what he is not telling you, and no dentist will for that matter, is a fixed denture or all in four dentures have a serious draw back. You can't take them out to clean, food and bacteria will get and stay under that fixed denture because your tissue is still malluable eventually causing the implants you spent 20,000 to fail. ( thats called job security) Yes they recommend to clean with a water pick daily but let's be honest not cleaning everyday is the reason a lot of people are in dentures, the mini implants are great they hold the denture secure but allow the prosthetic to be removed and soaked in antibacterial cleaner. Mini implants are also 15,000 cheaper per arch. But in my opinion the best bang for your buck if you had to replace all of your teeth, 2 standard implants ( not mini's) on the upper and a 4 standard implants with a bar on the lower. The bar helps distribute the forces from chewing which are incredible, and takes pressure off the implants. JUST remember folks implants aren't for life they fail all the time so if you do choose to go the fixed denture route. Water pick religiously and never let them sell you more than 4 implants per arch. Honestly 2 per arch is sufficient for most people if they are standard implants and not Mini's. Mini's need 4 implants because the abutment is not as deep and therefore can't carry the load a standard implants can handle. I despise dentist they rip the public off like you wouldn't believe, I know this cause I've been in the field for 15 years. Makes me scared to go to a medical doctor or surgeon after some of the shit I see at work

Just got removable mini implants, and hate them. This was 2 days ago. I got temporary ones in. I went back to the dentist for adjustments yesterday. Today it's pitching the back of my tongue. I wish I would have never did this. I haven't eaten yet, and been crying. I spent the last of my money on these horrible bulky implants that don't fit right. I'm so angry. I feel like getting them removed. Mini Implants are just glorified dentures . Regret

Love your comment. I want full replacement as I need to much work . What you recommend ? Individual implants I can’t afford . I’ve even thought of flying to Mexico or another country cause here in USA prices are rediculous.

Man, what a soothing voice! This man could ask me to rob a bank and I'd probably do it! This guy could be my dentist any day! Don't get me wrong, my dentist is a great guy and he is super calming but this guy is almost on another plane of existence!

Thank you Leona. Definitely check them out, they are good when you have limitied bone available. However, if your budget allows it, I would look at the normal sized implants as well because there is more research behind them, and because they have more surface area, they might be better in the long term. If you speak to an experienced dentist then they will be able to discuss all options with you.

I've needed implants since fighting lung cancer in February, 2012 where I lost 40 pounds of muscle mass in only 2 weeks and also teeth. 6 to 9 months to live I was given. Well, after the 2nd biopsy and MRI, I was negative for cancer. How? I'm clueless. But my teeth continued to deteriorate as the body will use teeth, bone, muscle, etc in order to survive. And I no longer can eat after last week as I broke 3 teeth which exposed the roots on french dip sandwiches of all things. 2 days of the worst pain ever. I live on nutritional drinks and supplements. I'm 65. I thought about going with the mini dental implants due to commercials by a local dentist. But although he didn't say it the way you did, he said he was doing it for patients who had been told they couldn't receive the regular implants but they found a way!!! Well, now I know the way.....they didn't have enough bone density to support the real implants as mini implants don't go as deep so can be used in in replacing a tooth here and there. Question.....a dentist seems to either do only regular implants or mini implants. Why not use a combination depending upon the individual? Second & final question; why do dentists insist on doing a root canal rather than a regular dental implant since in all reality, they are the same price. Examples: I've had numerous root canals (4 to 6) over the years. One dental implant is basically the same cost as a root canal. Because first you spend alot of time grinding the tooth down and then remove all the nerves from the tooth to eliminate any future pain. It's then shaped & formed so a temporary crown can be made, installed, while the permanent crown dimensions & characteristics are sent off to be made. A week or two later, the temporary crown is removed, the final crown is attached, and any adjustments needed are then performed. For the EXACT same PRICE, I could have had that ONE molar just pulled or extracted and a regular ("load bearing") implant installed into healthy gums and jawbone. Then send the tooth off to make the permanent crown that goes on the implant? No temporary needed as I can keep the implant area clean and even take an antibiotic. Then attach it to the PERMANENT implant once it arrives? It's $2,000 either way, so in essence I could have had 6 load bearing implants right now! And being 30 years younger, the bone would have grown around the implant and the gums around the crown. It's a much more permanent solution, especially in your 20's and 30's, then when your in your 60's!!!!! You still need to brush, floss, and have 6 month cleanings at your dentist. But I honestly feel I would have all my permanent dental implants right now. Because when your performing a root canal, the tooth's roots are already somewhat comprised! Please tell me why as that would be 6 implants or $12,000 I wouldn't have to worry about today. And when one is looking at $32,000 to $40,000 for a full mouth restoration, I could he looking at $20,000 to $28,000. And that's a HUGE amount to me.

For starters, I am not white and for even further clarification, if I am not referring to you, then why are you even commenting. I think people of Middle Eastern descent are gorgeous and I have been feeling this way about them for over 30years. This wasn't a dig at other races, I just like the olive tone of their skin with the dark prominent features of thick lashes, facial hair, and eyebrows. This man is spot on! How do I KNOW this? Right now I have immediate dentures in my mouth. My dentist is one of the best in denver. He steered me in this exact direction. In a few months when my bones fully heal I will have mini implants in. The dentures are tough to get used to for the first month. But like any prosthetic, you gotta learn how to use it. Now my dentures are good enough for me but my bottoms can easily come out. So I went from this “miracle” all on 4 super expensive treatment thinkin that’s what I need. Then my dentist showed me the correct slow rebuilding process. Now I’m thinkin I’ll just keep a top denture (it fits really good!) and get mini implants on the bottom. Total after all said and done it’ll be about a year long process but cost only ~$10,000.

Lord I wish I could find someone like you in the Houston area. I have tooth decay & I’m sure Gum Disease. I’m 48 & I only have 3 teeth in front lower left in my mouth. All others have broken off & my gums grown over them. I’m sure this is a very expensive procedure also. Another reason I haven’t seen a Dentist in over 30yrs. Living off Disability is hard enough. It’s a shame people like myself have to live in shame, depression & seclusion because our dental care is not covered under general healthcare

Great video. I did get all on 6 (upper and lower) here in Dallas. I had a healthy mouth but had a problem with roots splitting under crowns where I had had root canals. Over 25 years I was down to 19 teeth, facing the removal of one that held a bridge. I was chewing with my front teeth and everything was shifting forward and downward. A mess of bad past dental work and old dental work. No decay, no gum disease, non-smoker, non-drinker. I wanted all on 4 and interviewed quite a few places, settling on Radiance Dentistry in Irving Texas (Dr Mounir Iskandar). He has everything in house and uses a material called XircOn for the finals. Some sort of nano-ceramic flexible material. We took our time, allowing about 9 months before fitting and creating the final bridges. I had my finals placed permanently about a year after surgery. They are not like my real teeth, meaning they have no ‘feeling’ but I am super happy with the results. I was meticulous with dental care previously and now as well. I hope they last the remainder of my life (I’m 55) with exceptional care. I look 100% better and feel 110% better about my appearance. My prosthodontist did offer me options, but I was determined. Thankfully he, like you, was determined to take the cautious and careful path, allowing healing and proper fittings, etc. I think it’s a good option for some people. Maybe I was the perfect candidate? I also had great sinuses and lots of bone :)

@@jpoupon66 He did both so he’s a great person to ask regarding how hard materials behave against one another. Does he get any clicking or does it bother him at all? Sometimes dissimilar materials make more sense but all risk vs benefit. Also ask about warranties. If the guy/gal performing the surgery and restoration won’t warranty the work, something is amiss. It’s too predictable not to. If you don’t find the answers you need, feel free to message me if I can help. There are a lot of great providers in and around DFW.

The challenge really is finding a good and HONEST/ objective general dentist whom would give me the best course of action,to make sure full arch is what I really need...just to be sure before removing natural teeth out and spending $$$$,and in the Miami,FL. area it is mission impossible ! I'll take my time until then,even if I have to wear partial denture in the mean time...cause with implants one can literally get "screwed"😁🙃🙂

The only downside is the ledge im guessing. This type of denture has a ledge which sits on the gum,like removable dentures. The ceramic/zirconium dentures don’t have that. Receding gum tissue is a problem when you don’t leave time for it to heal. I have ceramics and have no issues with gum tissue loss,however it took a year from extractions to final ceramic bridge to be complete. I wore temporary dentures during that year.

Yes - I know what you mean. Dentures have a flange (we use special words to make us feel special!) or ledge. With any fixed teeth supported on implants, the ledge is a bad thing, because it means that you cannot clean the implants. If the teeth are removable, then the ledge can be useful to support the lips.

Thank you, you just told me what I thought. I am going to have complete upper with implants. I am 58 and I have a mouth full of crowns. All 4 front and 4 rear. They are old and in need of redoing. So the expense of redoing them is going to be better to have them all pulled and a full upper with 4 implants. I didn't like the ideal of pulling all the teeth and then installing the implants on the same visit. My ideal is to pull all the teeth and let the gum and jaw bone heal. Have a upper Denture made to have temporary teeth while the gum and jaw bone is healing. After the healing has taken place then have the implants installed. Thank you for your time.

Mentioning dentist, lab tech and anesthesiologist scheduling is not a good reason to not have a procedure - it is your logistical limitation. The patient already has a lot to worry about, so don't add to it. Facilities that specialize in this treatment have the staff and equipment in place to do everything in one day. Complications due to disease or other factors are valid points and must be considered, but a specialist surgeon would ensure these are not a problem. I hope you are not persuading people to not pursue all on 4 due to your scheduling problems and personal limitations. You are a great speaker and I encourage you to reflect on patient outcomes with specialists in the all on 4 treatment rather than your own experience exclusively. Watch your video as a patient would rather than as a practitioner. Thank you for your perspective.

From your comment, it seems like you are a dentist. In these videos I can only cover one or two ideas at a time. I have 6 immediate loading full arch cases booked in this month, and I have a two hour consulltation with each patient before starting any treatment so my patients are fully informed of every treatment option. This treatment is not the only way forward, but it seems that many dentists will push it on their patients because it can be very lucrative and an 'easy sell'. In the right situation, it is the right treatment - but not all situations. If you feel differently, start a youtube channel, and tell the world what you think, and I am sure many people will agree with your way of thought.

No, I am a patient. I have looked at the various options and ironically have surgery scheduled for tomorrow morning. The truth is I am tired of the back and forth trips to the dentist and the associated pain with each visit and need a resolution. The cost has caused me quite a few sleepless nights, but I do not want to deal with this any more. I wasn't trying to be critical of you, on the contrary you put a lot of effort into what you are doing and that is what got me here. Just remember to think of what we patients are experiencing. I did gain valuable information from your videos, so thank you for that.

I'm a firm believer in letting the gums heal before implants. Then the implants don't have to be as long leaving more room for acrylic or porcelain or a metal framework if necessary. Less breakage. I'm also a dental tech for 46 years doing mostly over dentures. Easier to clean and if they do break a lot less hassle to repair

Hi Larry, I'm not sure what you mean. Placing immediately does not mean you have to use longer implants, as long as you have bi cortical fixation, is fine. Also, the alveolar reduction (and increasing ovd) is the part of the procedure that creates the prostatic space, nothing to do with length of implants, or immediate/delayed placement. Most of my patients have tried dentures and although they are easy to clean, they find them terrible to live with.

@@drgurssehmi What I'm saying in a two part implant the screw on abutment has to be longer to come through the ridge with immediate placement. When the ridge shrinks the abutment doesn't have to be as long but seldom replaced when redone. We have done a lot of dentures over implants with great success.There are pros and cons to both systems fixed and removable. Have a good day.

Hi Liz, its confusing because most dentists will call it something different! Essentially it is the all-on-4 treatment - but very few people will do it with the mini implant phase. I have only spoken to a few dentists who do it like this. If you don't have much pre-existing infection, then it might be fine to do the all on 4 treatment without the mini implants.

Dr. Sehmi, Let me start by saying that I am absolutely terrified of what is happening with me and this process. I absolutely love your energy and happiness you emulate when you speak. My name is David and I just started my process of getting a whole new upper arch. Thanks to genetics and an autoimmune disease picked up while in the military, infections are rampant. My dentist is presenting an all on 4 arch for me, but I am not certain about it. I’m not a dentist, but your issue of having a sterile environment for the implants is exactly what I was thinking. Plus, I have not seen many people use the all on 4 method, I’ve seen mostly a minimum of 6, with 7 implants being the key amount for an upper. My dentist claims that the titanium arch where the strength is. Can you please advise on what I should do? At this point, what is a little more money, I’m already spending over $20,000.00 US.

Hi David. Most people who start this journey are terrified and anxious! Its a big deal, and you always want to be sure you are doing the right thing. Some studies have shown that there is hardly any difference with 4 or 6 implants. The extra two hardly add any benefit. However, if the dentist is placing the implants and finds that your bone is quite soft, and the plan is to give you fixed teeth on the same day (like the all-on-4 type of treatment), then its usually better to place more implants, so the biting forces can be spread more widely. If you are concerned about infection, then I always recommend removing the hopeless teeth first, let the areas heal, and then place the implants - this really reduces the risk of infection. But it does mean that you need to deal with a temporary denture for a period of time, possibly up to 9 months. These are often a real pain to get on with because of looseness.

Dr Semi....Hello..I was just browsing watching music videos and saw your video somehow came up and Inwanted to kindly share something.... Once a patient's tooth/teeth have been extracted and then you're basically left with a gaping black hole, you can clean around inside the extracted hole AND THEN FILL IT WITH processed crushed bone and then place the crushed bone inside the burned-out area of the extraction and really pack the bone in there....then finalize that area and then [maybe you may have to harvest some gum tissue FROM the roof of the patient's mouth as a 'donor']...and then stitch up the gums to and fro, and then after few months...I'd say anywhere between 3-6 months, check-up on the healing ...AND THEN AFTER all that, then proceed further with the actual implants.... this treatment incl a lot of avoidable risks so i tend to avoid this option. losing bone has massive cosmetic issues. it was once thought immediate implants prevent bone loss, but we now know it has zero effect on bone. 5:20 its logical to think putting implants into a sterile area is better than into tooth socket infected with gum disease that lead to extractions. 6:55 i like to reduce risk maximally. 7:20 step one isnt extracting teeth, its mini-implants designed to hold a denture in position, then make removable dentures. then when teeth are removed, we can place fixed dentures so they can eat fine, protect the extraction sites, remove the infections, take time to assess how far back to start implants.

Thanks Dr., for such a well-made video that makes sense. However, isn't there a similar risk with putting in the minis at the same time of the teeth being removed because there could still be the harmful bacteria present at this point which could inhibit the bonding of the bone to the implant? If one chooses to go this route before starting the All-on-4 procedure, do the mini implants stay in your mouth while normal implants are put in? Would the normal implants have to go in at less than ideal angles in order to avoid the mini implants? Thanks for your informative videos!

Yes, there is a risk of this, however, if using the mini implants, and they are not in the ideal position, they are easier to remove later on, if needed. (Well, they are never easy to remove, but easier than normal implants). Sometimes you can just take the "head" of the implant, and leave it under the gum- this is less traumatic. To be honest, these days, I try not to use the mini implants option if possible, as it can add these complexities.

For all of you who live in the USA: Plan a 3-4 weeks holiday and come to Europe. The prices in the USA are absolutely absurd. You can have the best dentists, most of them with a PhD, in countries like Hungary, Turkey or Bulgaria. Their prices are a fraction of what you spend in the US. And equally good, if not better. I am 71 years old, and had my upper and lower jaws (27 units!!) completely 'redone' in Brazil. And that was 15 years ago! All amalgam fillings replaced by caps, a bridge (result of an accident) replaced by implants, and crowns. I visit my dentist in Germany (a University Professor in dentistry) twice a year, and each time she tells me "except for cleaning your teeth, I won't be able to make any money on you. And these will be yours for the next 25 years!". That should tell you something!

Bless you, I am getting 2 tiny implants to support a lower denture plus extractions and all of it will be around $4800. I didn't even KNOW you could do that, but my dentist is like you and so today I got them. About 10 years ago I got a single implant on top and it alone was around $6000 so there is help for you people that want white healthy "teeth" that you can actually chew with.

That’s a big problem I’m having...lots and lots of bone loss. Dentist said they could do implants. but I’m afraid it’ll go wrong or I’ll have issues later ...losing more bone. Other dentists said I need bone grafts to be able to hold in a denture but I don’t want a denture. I don’t want my face caving in later. I’m 59 and don’t wanna look like I’m 90

I just got immediate dentures after having everything pulled.. it's been about 3 months, and I HATE THEM. Not only that but my dentist is completely of the thought process that he's got my money so I'm just a nuisance now who won't stop complaining about stuff he doesn't want to fix. He's already done a hard reline on the bottoms and actually told me the other day, and I quote "most of my patients bottom plates don't fit well anyway so they just put them in a drawer and don't wear them. You'll learn to eat without that bottom plate in" !! I'm a 45 year old woman who was told I would have beautiful teeth after the surgery and dentures... never that the money I spent was for bottom dentures for my drawer to hold. And you can see my bottom gums when I talk so you can see that I have no TEETH !!! I hate this so much and can't eat, have lost weight, and am completely depressed, beyond hope. Does it get any better guys or is this it ?? I need to know.

I am really sorry to hear this, and I would totally disagree with your dentist - why have something to never use it again? The truth is that lower dentures are difficult to get on with. They can work for a lot of people, but if the problem is that they keep moving, then you might need to consider something with dental implants. I would find someone with a little more empathy to do this though!

It does no good to file a formal complaint because the Dental Board is nothing more than a country club for Dentists who pay dues to have them dismiss any complaints against them! I feel so bad for you I wish there was a way to change the honesty of the system of healthcare in this country but it seems the Medical/Dental industry has a fail-safe system for those who are morally bankrupt and mediocre in their profession. To some such as your Dentist, perhaps he never committed to memory the Hippocratic Oath. Heather W. 3 months it will take you some time i had 16 extraction. Removed by Las Vegas dental school. 50$ to remove each 🦷 And i had my dentures done for about 1000$ i have a friend of mine that owns dental lap. So until you’re gums keep shrinking your going to have to keep adjusting your dentures. After 2 years I’m finally going to get my implants done. I’m turkey 🇹🇷 7500$ great place . I’m not paying 40k to 50k in the USA . And you will be ok just be patient it’s going to take some time.

very logical ...thank you so much for your insight, you have helped me so much I am going to have all my upper teeth removed May 3rd This is the scariest thing ever. was so concerned with the immediate denture no implants good to know I have 6- 12 months to get a Hybrid Denture mini implants etc... my teeth are not horrible I kind of love them but they are tiny and falling out so my options are limited. Very informative video! I agree that I don't think these mini implants are the best route nor is this "in and out on ONE day". I need 100% implants. But because of illness, I'm just hoping they can remove what few teeth i have left and then probably various roots that are still intact but tops have broken off. I see it going down like this: xrays and 3D imaging to actually see what all needs to be extracted plus looking at the bone density. For the molars, you would want "load bearing implants" and not minis as they are doing the heavy chewing. You might use minis on the front but may take 1 mini per tooth as they are doing the "tearing" (like an apple). Once the game plan has been established, the first thing would be to do a thorough cleaning the same day as the consultation to assist in any infected areas. Then on antibiotics for say 7 days to eliminate any infections and begin the healing process. Then go in have the implants put in. I'm thinking for truly permanent teeth that do not have to be removed, that 8 up and 8 down would work. That's a total of 16 implants. And if using no minis, one implant could hold like 2 teeth. So 2 x 16 = 32. Having 32 beautiful teeth beats what I have now. By doing it this way, shouldn't the gums grow, heal, and somewhat attach themselves to the newly implanted teeth? 9 years ago I needed my first 2 front implants and thought that was it then, all my top root canals started either fracturing or getting infected at different intervals so now I have 5 upper implants and a bunch of slightly mis colored, off shaped very expensive crowns , one coming in next week - what was I thinking? I also still have 3 natural un-touched healthy looking teeth. You can't see the back molars, so it doesn't really look all that bad. And with the implants so far everything is holding up well. Now, I noticed that all my lower root canal molars are acting up and might need extracting but my lower front teeth are holding up. Every time I get dental work done my crowns act up and they did with this last implant and the pain was so bad with the 3 remaining root canals, I had to go and see him. Of course he wants to place one more upper implants and a lower implant. Here we go all over again. All the pain has subsided except for one lower old root canal, for sure I am going to get that tooth pulled. I don't want to end up with 7 upper implants it but looks like I might have to. I also don't want any kind of lower partial that will put stress on the already compromised lower front teeth. From the comments I have read I have too many upper implants and it is better to have 4 regular implants on the bottom with a bar. Now I am looking at pulling my lower teeth, inserting 4 implants and this time I want to do a snap-in denture. What a financial nightmare. I had braces in my 20's and numerous root canals in my 30's and 40's with the intent of having teeth in my old age and at 67 I am getting dentures. Dentists only tell you what is profitable for them. God I wished I understood this when I was in my 20's. So knowing the truth now, I have to make the best informed decision and watching these videos and reading everyone's comments has helped so much, so thank you.

Hi, thanks for your comment, your story is not that uncommon! Actually, I think you have probably made the right decision, although it seems like the most expensive one. If you only have 1-2 missing teeth, then its generally accepted that replacing only those is the best plan of action (why remove teeth that are otherwise pretty good?). Having crowns and implants can sometimes interfere with your bite, and this can cause pain in otherwise silent teeth - this can easily be overlooked by some dentists, and some patients are affected with it worse than others. Your existing dental implant can be used to replace a "full arch" or "full set" of teeth (as long as the system of implants is suitable). We tend to use 4 implants as its the minimum to support a full set of teeth. In the past I have used 6, and I have seen cases where 8 have been used. The more implants, the more the cost to the patient. Remember that things can go wrong with implants too - so I often recommend to delay implant treatment for as long as possible. It is very expensive and complex to fix failing dental implants!

Having implants on the right side put pressure and trauma on my left root canal crowns and when the dentists tried to balance them out they nicked both old crowns to prevent the crowns from bagging. Now the root canals are hurting unless I keep them quite with turmeric and garlic. Still the upper one really started hurting and I can tell that an abscess is cooking down there. So rather than wait I am having them extracted. Deciding what to do has been one of the worst decisions in my life and yes the expense is painful tool. So after many hours of looking at the pros and cons I decided to have two more implants. I have 3 choices - a partial that will end up affecting the non-filled teeth and end up causing more problems for them, leave them be after the extraction and wait for more bone loss to occur, and potential biting problems, or place implants, stabilize the bone, stabilize the shift so trauma doesn't cause the implants on the right to fail, and life goes one like new teeth. My only concern was dealing with the toxic effects of titanium leaching into the organs and the potential for infection because for some reason, the surgeon has to leave the infected dental ligaments in place. My only course was to maintain the best immunity I could by eating an absolute healthy diet, which I have already learned to do. Also by refusing the pain medications and antibiotics because they play a big part in destroying my immune system -- the very entity I need to heal - Apparently implants only last 20 years at best but I am a 67, so yes that will most likely work out okay. And yes, they are crazy expensive. I know you mentioned to wait as long as possible but I did that with my first implant, not understanding the complexity of implants and by the time I agreed to do it I more bone loss had occurred. Now I have an issue going on where I have lost tissue only on that one implant and decided not to have that pin-hole procedure because the dentist has to stretch the remaining tissue down that actually thins and damages the tissue even more. So yes good oral care plays a big factor in maintaining your implants too. Proper diet and oral care play a really important part in maintaining implants and if people are willing to try and prevent the problems that caused the tooth loss in the first place - then go for it because implants are so much better than other alternatives. BTW I knew my implants were causing havoc on my root canals I just didn't realize how much havoc, a problem the dentists tried to prevent but could not. Now I am praying that the 7 implants won't cause too much havoc to the remaining two crowns and natural teeth I have left.

@@drgurssehmi So my general dentist was finally able to place the crown on my last implant, balance the bite and for several days now the root canals felt better and things started like they were getting back to normal. I am coming down with a cold and now my ears and throat are hurting, and my 3 root canals are slightly hurting once again. I don't see any usual signs of infection. So now, do I keep the root canals hoping the pain will subside, or do I pull them and have more 2 more implants inserted? I see my implant dentist in two days, and he usually leaves the decision to me. And yes, I am trying to hold off on more crazy expensive implants because they come with their own set of problems.

I am thinking of getting ceramic implants. Are they a lot more? I do not want metal in my mouth. I can't even wear costume jewelry because it turns my skin black. I am probably going to be paying for this for the rest of my life, but your mouth effects your health so much. You gain weight without teeth, because you can't eat healthy stuff like root vegetables or anything hard, and you can't chew your food properly, you also have heart problems (bad teeth can actually cause a stroke, and so much more. Your mouth is what your food passes through and takes everything down to your digestive system then it gets released into your bloodstream, people don't even realize how crucial your oral health is. I was obsessed with a healthy mouth and nice teeth. I used to have gorgeous teeth until I went through chemo, it destroyed my gorgeous teeth. I know how many of you feel, it is making me feel very self conscious, not even want to leave the house, especially that I always made other smile with my smile, when I came into the room, I had white straight teeth, I loved smiling and making others happy, now they just stare and judge. It is a horrible feeling. Right now I am looking for a dentist in North FL who does ceramic implants and has great reviews, but is not greedy like most American dentists.

You're speaking about my MAIN concern. Existing periodentitis and it's COMPLETE elimination before the final implant and teeth are installed ready to go for HOPEFULLY years. I'm in the USA and I won't name any company, but I'm sure you're well aware of them that offer a quick and ez removal, scrape, dremel, clean, install temps and then come back in 4-6mos for the finals...BUT,,like you are soo correct on...HOW can the disease within the bone and tooth socket be removed and "sterilized" in a few hours?...Anti-biotics?....I like your approach,,,tell me more soo I can book an airplane ticket,,,I guess a few tickets!...Thank you Dr.

Thank you. So I am always super cautious when the reason that someones teeth are in a bad state is because of gum disease. During this treatment a lot of the infected tissue is removed, but if someone is having this treatment in the top jaw and the lower jaw still have lots of infection- its impossible to eradicate it completely.

Well done Sir. I have been a bit of a skeptic of the immediate placement of implants and reduction of the alveolar ridge for placement of an “all on four” fixed / removable prosthesis. How long will these last? I have been around long enough to see the results of the 40 plus year old subperiosteal implant failing. Trying to find a solution for patients in these situations is very difficult sometimes after 40 plus years of bone loss and resorption.

Hi, when patients ask me this, I am always deliberately vague. I say decades. A lot depends on cleaning around the implants and how easy the design of the teeth is to clean. I have seen full arch treatments fail in 2 years (this is the worst I have seen), and this was a combination of the patient not coming to see the hyginist or even having a review, and what we suspect was a bite interference. Other than this, we tend to see stable bone levels around the implants over the long term. I would say that almost anything can fail - just like natural teeth. What is more important for a lot of people is how many years of good use can you get from them. If the treatment is done well and regular recalls and hygiene is followed, then they should last indefinitely or just require minor work to fix small chips and cracks in the teeth (but implants should remains fine)

It’s alright for you people saying do not do this bla bla.. Have you ever been toothless ? totally toothless, unable to eat anything but ice cream, baby food and slops?? And not from not brushing either! Maybe we were hit, had an accident or had bad parents! I haven’t had teeth for 15 years, just a boggy top denture. I crave an apple, or salad. I have dry skin, terrible hair and a mouth like the bottom of a bird cage. You think we want this ?? That said its something for US. Teeth, a new life !!! so please think before you bang on about how implants are wrong. I had. Three on 6 and its taken so-long because my surgeon is so meticulous about it all going well. I cannot wait for my apple. 🍏

Anti biotics to kill the infection and you have removed the source of the infection, so in theory shouldn't return, how ever I imagine after it's been freshly placed the open wound would get infected quite easily and require alot of upkeep until healed

I have 6 on top and 6 on the bottom. And I did heal for a year.My problem is I now have a permanent fixed bridge .And they have no flange.And I can't stand it.It has changed my whole structure My smile is empty and all I feel is extra large teeth.Food is constantly caught.Please help me can I change this.I had them done in mexico and I have had a specialist tell me they are the best implants he's seen but doesn't know how to go back to a fixed denture. I'm so desperate .I just want these out.I was happy with the over denture. Please help.

I had bad teeth and my dog accidentally knocked out my filling an I just worse I had all my top teeth pulled to get a denture an then they told me I needed aveoplasty I can afford it they said it effect the fit but I just healed up an getting opened up again makes me cringe I fear they will take too much bone an then I won't have even have it as future option.

Hi Doctor, thank you for sharing this information with us. It is extremely helpful. I do have a question. Today i have my first consult to discuss options to replace my current dentures. Ive been wearing full top and bottom dentures for apprx 2 years and i am very insecure and dont like the way they changed my smile and appearance. The dentures look nothing like my original teeth, and my #1 concern is to like like "me" again or close to it. Is there any option (other than dentures) that could help achieve this, or are the sizes and shapes of the teeth standard? thanks so much again!

Hi, sorry its taken a while to reply. With any option, including dentures, there should be try-in stages, where you can see what the final result should look like. You are able to customise the colour, shape and position of denture teeth. If you have a fixed bridge on dental implants, then you could have porcelain teeth, which can sometimes be more customised - however I rarely do these due to the standard ones being so good!

Love you way of explaining. QUESTION. Instead of implants can porclyn crowns be replaced? All my teeth are porclyn crowns. And I'm missing all my back teeth, or several. I Only have 20 teeth left. I want new crowns. But also need implants in the back where i have missing teeth. Is this possible? Replaceing crowns?

Hi, yes, this definitely sounds possible, in fact, it would be the right thing to do. If you can keep your own teeth (and just replace crowns) then you always have the option of dental implants in the future. Right now, you can just replace the back ones if you feel like you need to.

Can you do a bridge of say 4 teeth on 2 implants. I have crowns on one side with no teeth to grind to on lower jaw. It’s opposite on the other side.

Yes, this is usually possible. I would always have the bridge resting on implants in both sides, definitely not implant one side, and tooth on the other.

I really like this plan..When the 2 initial mini implants are placed are those the kind that allow the temporary denture to snap in and out or is that a fixed denture? And when it comes time to do the all on 4 are the original mini implants used or are they take out and replaced with regular size implants? Can the two minis stay in as part of the 4 added at a later date? I was also wondering if I get one implant placed at a time over many years as each tooth needs to come out can the already placed single tooth implants be used to attach an all on four hybrid denture. It’s my understanding that all on 4 treatment needs to be done all at once since the bridge of teeth works together while implants placed for single tooth one at a time can be placed at different heights making it difficult to use for an all on 4 denture? I really appreciate the info I’ve kinda hoped to do this slowly over time vs all at once in a day and your plan really appeals to me

Hi Trishy - the "all-on-4" is a way to get you to the end result in a short period of time. If you have implants placed as and when you lose teeth, then you can still have exactly the same end result (as long as its planned properly from the start). Essentially you need to connect all the implants you have - with a fixed bridge of teeth. With mini implants, depending on the brand of implants, the connections may or may not support a fixed bridge. So it is usually better to have standard implants from the beginning, as this gives you the most flexibility in the long term. I hope that helps!

Dr Gurs Sehmi it does help thank you! I feel like getting implants as I need them over time will ultimately help me cover the cost over a longer period of time and allow me to get the two in the back I need now but have the option when those front lower teeth start causing problems. It seems like that might be easier recovery as well

I enjoyed your information regards all4. I'm in Australia and thè cost is high. I know I need denture soon,however can't afford it. But I dislike loose ones and like them to let me eat and speak normal. What is the name of the ones you can button up. Like semi permanent? Or is loose ones and permanent implants are the only option?

Hi, ask your dentist about using locators on dental implants to hold your denture... They should know what this is - I think its what you mean
