You'll need to have beaten the game on 0BC in order to get the Homunculus rune before you can get the Disengagement blueprint. The Homunculus allows you to detach your head and get items with it, which is how you can reach the blueprint way up there at the beginning of the Prisoner's Quarters. Your detached head can stick to walls and can also double jump, which is how you can scale the odd tile sets on the left side of the entrance.
If you place yourself a step or two to the left from the flame lantern and use the Homunculus you'll be in a good position to both reach the walls on the left side of the room as well as make it all the way up to Disengagement without the head popping back to your body. A good marker is to have your feet on either side of the thin piece of wood that sticks up from the floor (as seen here).
Overall, Disengagement is a better mutation than using Ygdar Orus Li Ox (YOLO) since it can be used once per level and doesn't take up a mutation slot after it activates. YOLO is great for early game for those first few runs but I recommend moving to Disengagement once it's unlocked. I hope this helps - good luck out there!
