Huge problem with all of the treatment options: For almost every patient, Covid-19 has two phases. The viral replication phase, and then possibly the autoimmune phase. If you look at the viral load over time chart, you can see why antivirals have very limited benefit in phase 2. You can also see why steroids are contraindicated in Phase 1. In fact, antivirals would offer the most benefit before any symptoms appear! So the FDA's humanitarian use standard for remdesivir only allows it to be given when it does no good. But the scarcity and cost of remdesivir also contributes to the problem: In order to see benefits you would have to give it to everybody post-exposure, not knowing if they would get sick or not. Testing every day with spit strips would offer early detection, before any symptoms appear. Getting a positive test would be the ideal time to use an antiviral treatment.
@jasonvoss1984

Giving post exposure is nice in theory but with limited supply a better option may be very targeted choice of patients who are at highest risk of developing a severe case. We have excellent data on who is highest risk.

What about anti-coagulants and anti-platelet drugs? Much of the damage caused by medium and severe cases of COVIDS is due to micro-blood clots that form in various tissues and organ systems of the body. If a person gets sick with cold symptoms, at what point do you say I should go get tested? I was thinking on day 3 of symptoms. In the meantime STAY HOME! My granddaughters were just sick with something. Ages 3 and 5. The 3 year old had a fever for a day and cough. That cough continued for 2 days. The 5 year old was sick the following morning. She had no fever, cough, and sneezing. Both girls were sick over the weekend. Monday the 5 year old had chest congestion and went to the dr. Doctor took precautions her coming in. Said it all appears to be upper respiratory and did not test her. I feel she should have been tested. Otherwise, parents may still take kids places and spread the disease. Our city has an uptick from 0-1 a day to now 14-19 confirmed a day.

Isn't it Squibb that already developed a 10-minute home test that costs about $1-2/saliva test already? Hopefully that meets the needs and gets it out asap. While RT-PCR qualitative may be the "gold standard", it tells us nothing about quantitative or viral load, nor infectiousness. A hyper-sensitive test may result in misreading effectiveness of medications too... Perhaps the reason so many are getting good results clinically with HCQ+zinc and some with a Z-Pak and others not. It seems the test is a part of the problem. Perhaps we need to be using a less sensitive, cheaper test daily at home with a saliva swab, then re-evaluate HCQ+zinc, as it makes sense from a scientific point of view, seems to have produced good results for many, and if the criteria of whether or not it works at all is an oversensitive PCR test, then no drug is going to pass. A quantitative PCR test needs to be done to determine effectiveness, instead of an all or nothing test as to whether it is there or not. Why aren't people asking the right questions?

So, let's toss out a hypothetical. Suppose you are given a choice of two things: 1) You are part of a study of 30,000 vaccine takers, and with a 50% chance all you got was a placebo. You was picked for this study because you are a nurse in Houston, where people are dropping like flies. Of the 15,000 with placebo, 200 are likely to eventually catch the virus and die. 2) You are assured that you are getting a vaccine that has proven to create antibodies in 100% of those given it in phase 1 trails, but will be exposed to a mild dose of virus in six weeks. AND, you will be among 199 others exposed in this way, but will have the knowledge that your risk will save a million lives. I see a no brainer in that mix.
