This type of question really confuses me especially because there’s no indication of anything that’s medical, maybe if she complained of a headache or any other medical problem. Clients come in for MH assessments all of the time with the above symptoms.
Whether a specific medical symptom is mentioned or not, almost all of the symptoms stated can in some way be a result of a medical issue. We just never know. That's why the rule of thumb is to ALWAYS assess for or consider medical issues first, especially if it's in one of the answer choices. It's not implying that it's always a medical issue, the SW should just rule that out first.
I work in a hospital, so it makes since to me that you need to rule out any potential underlying medical condition. If the patient has an underlying medical condition that causes the issue, then treating the medical condition would come first. In the given example, one possible problem could be related to her thyroid.
I am very confused by this because I would be automatically assessing cognitive function while speaking with her. That seems like the obvious choice during an initial assessment to me.
Medical would be first because the client stated she does not know what is wrong with her. If an event would have occurred, then cognitive may have been the next best thing to do.
