I swear we have a lot of professors and professors and many who hold high academic degrees, but not everyone who holds a degree becomes a teacher in my opinion. Those who are able to simplify and communicate information are the ones who deserve to be appreciated and called professors, not those who hold a degree without being able to communicate ideas as well. Great content. One thing to remember, inpatient treatment of a confirmed clot (DVT/PE) requires heparin (or LMWH) combined with warfarin for a minimum of 5 days and INR >2 for 24 hours to prevent rebound clotting. Warfarin’s half life requires a much longer time combined with LMWH/Heparin to provide full anticoagulation. For DVT/PE prophylaxis, the overlap can be less.

Would you ever use tPA for a DVT/PE? I only hear about tPA for ischemic stroke events, but couldn't it help with DVT/PE as well?

@@qman1131 Zach talked about using tPA infused directly onto the clot. We employ this option at work. Typically, it’s a low dose infusion directly onto the clot as the location is identified with radiology. That’s for DVT. We don’t use it with PE. Obviously, it’s a lot harder to feed the catheter to the clot location in the lung.

Zac and the team... what the heck did I just watch? This was EXACTLY the lecture on antiplatelets, anticoags and thrombolytics I needed. I was always confused on why we give P2Y12 and ASA for MI and not Heparin, ASA and P2Y12 for stroke ect ect but this just made it so clear. Then you give reversal agents, drug monitoring and case examples just to remind us that you're not just a channel for pre-med but for clinical med students too. Love it so much and so happy with how far you guys have come.

Zach your videos have been so incredibly helpful. My 62 Y/O wife has had Lupus since 1985. In 2015 her mitral valve was replaced due to Lippmann Sacks, in 2020 she had a massive DVT, and in October a subdural hematoma followed by bilateral PE’s, followed by an even bigger subdural. Your videos really made a difference. Whether Hematology (Platelets) or Brain anatomy. I seem to always find one of your videos that make it more understandable.

**hello zach in this lecture at 36:37 you have mentioned that gamma glutamyl transferase make proteins functional that are factor 2, 7, 9, 10, protein c and s but in another video of you on warfarin you have mentioned that it is gamma glutamyl carboxylase that make these proteins functional, the book I am studying for pharmacology is katzung and in that the enzyme mentioned for making the proteins functional is gamma glutamyl carboxylase not the transferase. So, I think it is a mistake.

@@kxcreative6064 I have heard of and understand all of them…although, our hospital (like most Healthcare Facilities) only keeps a selected number/type in stock. Most hospitals have a formulary of medications that are stocked and used on a regular basis.

I love this channel. I found it this semester, my last semester of nursing school, and it’s so hopefully! I took anatomy several years ago & these videos are great at helping me remember things I thought I forgot. Among the antoplatelet drugs the commonly used drugs are Aspirin , Clopidogrel etc. Anticoagulants include rivaroxaban , abigatran and edoxaban . While prescibing these drugs proper education of patient is extremeny important.

Thanks Prof Zach for the beautiful and simplified lecture as always !! I would like to add 1. platelet transfusion is inefficient to reverse the effect of Ticagrelor, because circulating ticagrelor and its metabolite are likely to inhibit the fresh platelets. 2. Bentracimab is an antidote to Ticagrelor, it is still under trials though. It binds ticagrelor and its active metabolite with high affinity and specificity. 3. Apixaban clearance is less dependent on renal function(only 25%) compared to other DOACs and it is approved even in ESRD with reduced doses.

Absolutely fabulous content and gripping presentation! If only every teacher could grab people's attention this well! Oh, but the ads! I was getting bombarded by 2 long ads every 5 minutes. There are also 1-2 ads to watch every time you jump back or forth to watch a particular section again. The excessive ads really destroy concentration, which is the opposite of what you want to (and do) achieve. They made me so mad, I just wanted to swat them out of the way like flies. After this, l will never, ever, ever order Lieferando! Your videos are great! Surely, there must be a less intrusive way to monetize?

Unrelated question, I had a fusion on L5-S1 14 years ago. It seems scar tissue grew into S1, S2, and been on Pain Mge for the past 12 years. Now, over the past 6 months, I've been having pain around my right side, just above the right hip. My GP sent me for scans and all came back good, (no problems anywhere). Last Monday I was having coffee at a local shop and talking about the pain in my side, in the next booth a person said it sounds like a nerve in your lower back, WOW, can this be?
