I am 72 yes old with non diabetic neuropathy. Caused by sciatic nerve and degen spinal in L4L5. My PC does BP of my neuropathic ankles, but I don't see a calculation or measure. I will now see how I stand and I shared this with their office.

The one thing about blood pressure that always confuses me is when we take BP, we are told to breath, be calm, etc. Then the numbers generally calm down. But, so many people are on high alert all day, that even if you can get good readings by calming down, there is probably more time during the day that it is high that you don't know about.

Take your blood pressure when you are upset. Then, take it repeatedly to see how long it takes for it to become normal. The resilience is a sign that your arteries are healthy. But tell your doctor, of course.

Thanks Dr. Been, my husband, 66, an avid runner, fit, slim, healthy, no contraindications, just had cardiac bypass surgery. Of course before the surgery, they looked at his carotid artery, leg/arm veins - using the latest technology. It was all beautiful, clean. Blood pressure, excellent. Heart was fine, no evidence of prior heart attack. Shocked, he and I were that he required this surgery. (They could not stent because there was blockage at point where two vessels merged) They say heart issues in healthy seniors, men, is not uncommon. Must be genetics they say. I’m confident these tests are helpful but in my husband’s case, not predictive at all. There must be an “unknown”.

Thank you for great info. I am on the road at this time and will do the test when I get home. I am 78 yo and just returning from 50 days in Iceland. Climbed to the top of 2 glaciers, kayaked on a glacier bay for 3 hours times two. Most days hiked more that 4 miles and some days more than 7 miles. At home I play pickle ball 5 or 6 days a week. I credit my health to over 20 years in the US Army. PT test twice a year keep me in Shape for all those years and continued to be active after retirement. Again thank you for your video. Maj. Retired.

Actually measured sites should be a heart height. Each 10 cm height difference lower / higher of the measured site does add / subtract some 7.8 mmHg hydrostatic pressure - which is rather a lot.

The ankle and BP should be done with the same level ideally the level of the heart. When done in vascular labs, abi and anti are measured with the patient supine with arms and legs both at approx level of the heart.

Food choices are eminently researchable online (including RU-vid).A low carb, high saturated fat diet seems like a crazy choice but you may be surprised. Go look!

I disagree with this. The science points to low-fat plant-based eating. I recommend the Plant Chompers RU-vid channel for those who are science minded. He goes into deep dives on the scientific evidence for each kind of diet.

As I read and research for you, I find that the diet choice is a contentious area. All diet type champions offer evidence for why they like one diet over another. I find that reducing carbs do wonders for me. And, a few weeks ago I was in a conference for the plant based diet. For a few days we only ate this diet and my body responded to it wonderfully.

DrBeen, could you look into and discuss the odd thing happening in LC or injection injured patients where various, seemingly random, veins are "collapsing" or becoming "compressed" or closed or blocked or spasmed (constricted or dilated) or scared/thickened/stiff or ???. My daughter is 20 now, and recently showed very odd thing happening in the veins of her calves (found during ultrasound). She was also diagnosed a while ago with lilac vein compression. Other young people with significant complex LC are developing the same tyoe of thing including apparent eagle syndrom, thorasic outlet syndrom, nutcracker syndrome, and other strange vascular issues. All were previously very active, very healthy youth.

🙏 Thank you, Dr Syed, I'm watching now, I personally asked and had my carotid arteries checked a few months ago being now 71 but here in the UK they seem to consider anything below 50% of stenosis as good, which was my case . . . I'm not sure how to interpret that but it's all I have

Generally we should continue to make an effort to keep arterial narrowing slow or stalled, even, reverse. Healthcare system is tuned to accept the vascular narrowing and stiffness with age as more atherosclerosis takes hold. I feel that we should continue to try to reduce and reverse this situation.

Vascular health in general. As I discuss in the video sensitivity of this test for CAD is low. That means if this test says all is normal, then that is not a reliable result. On the other hand specificity is high, that means if this test says there is an issue then there is a fairly high confidence that there is an issue that should be discussed with your doctor.

Hi bonjour, do you measure BP in the morning before eating, what is the best times ? ALso, Do you take this while seated or upward or lying down... When I was in hospital BP for other concerns was normal, but they took it like just when I was waking up no walking before. At home BP, almost higher than hospital, and yes am nervous with white jackets of MD and nurses. So, they gave HBP meds as they seem not taking account of the nervousness of the moment when we try to get a good result, tryning to be calm, resulting in BP increasing at that moment !! Thanks for these equations, a piicture illustrating how to take BP from ankle would be appreciated !

Ihave a question from a laymen: what if you have high bloodpressure...so your arms have high bloodpressure and in relation to that also your ankle. But the ABI is within the range?

If ABI is within the range then at least this test can't find a problem. It's a low sensitivity test for CAD. I explain in the video.

Modern devices don't need orientation. I keep it outwards though. In a professional setting they will find the pulse and then orient it.

Who are "selected individuals" that should be tested? Would that be people who 1) have certain physical signs/symptoms or 2) "bad lab results". It seems like doctors might not want to "waste" time on this for people who don't have an obvious risk factor since it is not sensitive. What do you think? Disagree/Agree? Why? This possibly looks like a useful tool. Thanks!

Agree. We first need studies on a general population before primary practice doctors will be willing to do it. It is time-intensive, and right now, most won't even do the arm BP with the patient lying down.

It sounds like you already know this, but for those who don't know, calcified plaque is "old" plaque. It rarely is the culprit in causing heart attacks. It is the "new" soft plaque that ruptures that causes most heart attacks. We know this because people who have had CAD for a long time often have bad CAC scores, yet the subset of those who go on a whole food plant-based diet no oil have minimal incidence of new coronary events. As for exercise specifically causing calcification, I am not aware of studies that show this. Exercise is helpful for those with CAD but not as helpful as the diet modification described above.

@gtm, calcified plaques are when inflammation was long standing and caused enough damage that calcium depositions occurred. Soft plaques are currently ongoing inflammation. These can both be located near each other.

For a while my blood pressure was high and heart rate normal. Then after taking 1 clonodine it remained low . Both bp and hr. Then it was time for my yearly. My dr makes me nervous badly and takes little time very cold. So my blood pressure went up very high for a month waiting for the twp appts. First blood work then to see him. My hr went up as well. Got the visits done and a week a d a half later my bp is med high but my hr is low and i need anxiety meds and pain meds for chronic arthritis but have had to skip several dises a day between the two. Im wondering if this is heart failure.

From the ups and downs of anxiety and medication changes I doubt you have "heart failure." If your primary care doc didn't say anything they you probably shouldn't worry about it. You've been "seen" and he probably ran his stethoscope over your heart and listened to it. If you are still concerned make an appointment and ask your primary about it. I think your anxiety has a lot more to do with triggering elevated bp and hr than heart failure in your case. What ever you can do with calming yourself AND weaning yourself off anxiety meds (with doctor supervision) is probably going to give you the most benefit.

I knew a lady who's baby had a 1 degree temp and they fave him the polio vax anyway. He wound up paralyzed. Sad thing is she adopted him healthy and had no idea of the temp being a danger.

But you did not present studies on otherwise healthy people. We don't know the sensitivity or specificity in this case. You may end up worrying a lot of healthy people unnecessarily. I disagree with your recommendations, especially the ultrasound, as it is not free and insurance will not cover it for healthy people. What is really needed are studies in otherwise healthy people that look at combining the ABI data with blood pressure and cholesterol data. This may help to reduce the prescription of statins so that only people who actually benefit take these medications. (At present statins don't really show benefit when all you use is cholesterol data to prescribe them. They do help people who have had heart attacks, though.) Of course, lifestyle changes - especially in diet - is far superior to statins. For anyone diagnosed with CAD, I highly recommend "Prevent and Reverse Heart Disease" by cardiologist Dr. Caldwell Esselstyn, who shows the astounding benefits of an oil-free whole food plant-based diet for advanced CAD patients.

I like the plant-based diet. Having said that, this is an important measure for the ease with which it can be done. For the carotid artery thickness, yes, there is funds involved. Anyone worried about their vascular health should talk with their doctor to see if it is appropriate. If one can afford it then one or twice yearly is a wise approach.
