In lesson three, we start to take a look at a few of the basic arrhythmias in our ECG/EKG interpretation. This will essentially be the first part in our basic arrhythmia course with our goal being to have arrhythmias made easy!
We start off talking about the overview of the major types of arrhythmias that you will come across. From there we move in to talking about the most basic of these rhythms. First is sinus arrhythmia. We cover the basics including its defining characteristics.
From there we move on to our various premature beats that we will see. This includes PACs, PJCs, and PVCs. We show examples of each and again give causes and defining characteristics for each.
Final we move on to talk about our basic brady and tachy rhythms. This includes sinus bradycardia, sinus tachycardia, and supraventricular tachycardia (SVT). Once again we talk about their causes and help explaining their identifying defining characteristics.
We hope that by the end of this lesson that you would be able to recognize and identify these arrhythmias by their defining characteristics so that you would be able to interpret them on your patients rhythm strip if you came across them!

V-Tach and A-Fib. Survived cardiac arrest in 2020. Had V-Tach episode 12 hours later in hospital. They had to shock me twice. 5 days later, had a Biotronik AICD implant. No shocks yet 🤞 FYI, had a WPW episode 30 years ago with heart rate of 320 bpm. Tried RF Ablation, but could not find extra circuit after 8 hours on the table. Great job with these videos 💯. After release from the hospital after the SCA, found the ECG strip that recorded the V-Tach episode. That's why I'm here 3 and a half years later, trying to read the ECG souvenir 😊

Just wanted to say Thank you for thr ekg/ecg interpretation. I work for oral surgeons and am taking the ACLS for the first time. I am not a nurse, but your tutoring is fantastic. I received a package from the appointed instructor and I was not able to get what I have gotten from your lessons. I feel confident in passing my test now

I just want to say thank you SO much. I have gone through hundreds of videos trying to study for my NREMT paramedic psychomotor exam. Your videos are by far some of the best videos I have seen. Clear, concise, easy to understand, not too long/too short and keep my interest. I’d love to see you to some more rhythms, maybe some less common ones, and how they look on a 12 lead. I don't know if anyone has thanked you for this but Thank You for spelling out your words. I'm "old school" and like things spelled right and I'm not the greatest speller (I take notes while listening to your videos)...so again, thank-you. Ps, Im probably the oldest nurse studying to be an ICU nurse...I've been a nurse 36 years. I'll be 61 years old in a couple months. Never too old to learn - that is what keeps you young. I left Ortho Medsurg and Now in New To Specialty ICU training. I never had to read EKG strips in Ortho. Pts were cleared by a Cardiologist prior to surgery and they usually went home same day or next day after surgery, Thats quite the opposite in the ICU. The trainers are nice but they seem to make things harder than they hv to be and so dry that I find myself fighting sleep. Your vides are clear and to the point and I understand the differences between the strips. I feel more confident for my test next week. Thank you!!!

Glad you like them! :) In the PAC strip the rhythm is otherwise regular, so you will see equal time/distance between each beat and then this occasional early beat. Sinus arrhythmia is just an irregular beat in which there is not that consistent time/distance between each beat. It's speeding up, or slowing down or both. Hope that helps!

Thank you for the video, but actually "T wave opposite R" is not like a constant feature of PVC. There are different ECG leads and T will have different form in different leads. You can just say that QRS will have different form from usual SA complexes 🙌

What if you have a PVC but have a P wave showing up? These happen to me often throughout the day for the past 6-8 days. Usually 1 or 2 every hour. I think I have anxiety/stress right now. Also thought it might be caffeine. Wish I could share my Apple Watch ECGs on here.

at 10:32, I think the QRS is just the positive part of the wide wave, the negative defection should be the T wave. But you drew the line across both QRS and T wave. Am I understanding it right?

Good catch. The example there wasn't the best drawn, but somewhere probably south of our normal isoelectric line is where the inverted T wave probably starts.

I screencast my computer to my iPad and then use that to do the writing. I just then do a screen recording on my computer and use the mic for the audio as I go through the notes that I prepare 🙂

First thanks for the video. However, the morphology of the p wave in the sinus arrhythmia where somewhat different which makes it difficult to differentiate between sinus and PAC arrhythmias

If it helps, don't focus on the P-wave morphology. Its a matter of the timing. The PAC will usually come in "premature" or early compared to when you'd expect, in an otherwise normal rate rhythm. In sinus arrhythmia, the rhythm is speeding up and slowing down and so you'll see fluctuations in the HR. Hope that makes sense.

"Innocent" arrhythmias ? Do doctors often call supraventricular premature beats "premature ventricular contractions" when the person really had premature atrial or junctional beats? (Healthy person with "palpitations" perhaps due to coffee? ) "narrow complex" vs. wide complex (in someone whose normal EKG shows nice narrow complex normal ventricular complexes as to their basic rhythm)

Only issue here is that in SVT it may be from an underlying medical condition. I was misdiagnosed with SVT when I actually have Hashimotos Thyroiditis. I ironically work on a cardiac floor and have rolled into work with a HR of 140+ on numerous occasions.

I have pvc since last year, come and go and approx 10-12 pvc per day in a week very often no particular time and days sometimes 20 days no PVC sometimes 2-3 month no PVC please suggest me what to do and is it okay ? Safe ?

How can we distinguish between a PAC and a sinus arrhythmia.. because they look the same to me

Typically PAC is going to be a single beat and the rate resumes at the same rate (approx) that it was before. Sinus arrhythmia is more a steady changing of the rate and fluctuating up and down.

I went to the doctor and got an EKG, they said my heart was normal, a week later my heart skips a beat and it all started with shortness of breath about 3 weeks ago, I have no clue what to do. I’ve been very concerned about this.

ICU Advantage I talked to my doctor, he said my heart was healthy but an ekg was the only test we did.

@Oh Simply Like ICU ADVANTAGE said, consult your doctor. Now idk If know each other personally, but it’s never a good idea to get medical advice online unless it is directly from your doc. Theres no way ICU ADVANTAGE could tell what is going on with you, there’s just too many unknown factors . Also an ecg is just a snapshot, it doesn’t tell you everything. (And trust me I get it, if I was concerned I’d want answers ASAP lol) Also I am not trying to be rude or sarcastic in anyway just advice from a friendly paramedic 🚑👍😊 ....if you’re problems persist, or you are concerned in any way and can’t get into your doc...don’t hesitate to go to the ER! Hope you are well!

Cool video. I had been struggling so hard in interpreting ECGs in MedSchool and this video has just made things better for me. I still have a doubt though. In Premature Ventricular Complex how can you exactly point out the Q,R and S parts of the wide QRS complex???

Glad this video helped! You can still make out where it starts and where it ends, which is primarily the end points we often are looking for to determine the length of the complex. Q waves often aren't visible so you'd go from the start of the R wave.

Please i hope you get to my comment but i get twitches like on my heart or chest feels like in my throat sometimes there not painful but its scary they only last a second its like if my heart stops and continues. Ive had chest xrays and ekgs and blood work ive gone 2 times to the er and all fine and i still get them i had them for years and they blame my anxiety but now there daily like 3 a day its scary

I'm so sorry to hear you are going through this. Unfortunately, theres not anything that I would be able to do or to help with. You might talk to your primary care provider and see if they can get you a referral to a cardiologist just to make sure. They might be able to put you on a mobile monitor for a few days or longer to see if anything shows. I sure hope you can get it figured out!

I've been told it can be due to stress or anxiety. if not those then most likely an electrolyte imbalance.
