During the final mission in Batman Arkham Asylum, while fighting the Titan-enhanced version of Joker, Batman is injected with the Titan formula himself. In this scene, Joker demands that Batman change in order to defeat him. However, Batman refuses and instead decides to use the antidote on himself.
Arkham Asylum is the first game in the Arkham Trilogy created by Rocksteady, followed by Batman: Arkham City, then ending with Batman: Arkham Knight.

Batman isn't bullet proof and she had a clear shot. Batman is still human he isn't perfectly aware at all times. Most of the time he barely pulls through, more than anything his equipment saves his life much more than his skill and training.
