Joe Biden undoubtedly has either: Dementia with Lewy bodies, already diagnosed and under treatment. Parkinson's disease dementia, already diagnosed and under treatment. We can't know which diagnosis is correct because the treatment's start has been covered up. To differentiate between them clinically: if his parkinsonism began more than a year before the dementia, it is Parkinson's disease dementia; otherwise, it is Dementia with Lewy bodies. The problem is that he has been treated for parkinsonism for years, which explains why symptoms have been disappearing and reappearing. Parkinsonism symptoms are permanent and don't disappear without treatment. Treating Parkinson's disease with levodopa can sometimes aggravate hallucinations and confusion. Either Biden is aware of this, or someone else is administering medication in his food or drinks. It is hard to ensure that treatment has been introduced during episodes of cognitive decline, but it is easy to ensure treatment has been introduced during episodes of parkinsonism because parkinsonism is always a permanent disease, and its symptoms won't disappear without treatment.
