My son has had these episodes since he was 1 and now I know he has RAS thanks to you. He has been misdiagnosed with possible epilepsy but all the EEGs we have had done showed no signs of epileptic seizures. Thank you so much. I am going to follow up with his neurologist.

Might you see a long QT on a spot ECG - this would be diagnostic of congenital long QT syndrome

Hi Dr. Kazmi. I have a kindergarten student that I had suspected was on the spectrum and possibly ADHD because he is hyperactive, loud and struggles to keep focused. Multiple times a day he has “episodes” where he stops, goes into a blank stare, his body freezes and becomes rigid and then when he attempts to move or walk he is doing so in a robotic way with his legs locked and no bending in the knees. After about 10-15 seconds he appears to be alert but confused and unaware of what had just happened. The parents say they haven’t witnessed any episodes like this but I see it multiple times a day in the classroom. Do you think this could be RAS even though I haven’t noticed a color change (although it may be so because the child is tan skin) or any passing out/crying?

I forgot to mention that he’ll blink multiple times during an episode while staring ahead blankly and will often accompany it all with with rapid head shaking (side to side) almost like when you get a whole body chill but more aggressive.

Well from the description, there is a possibility of reflex anoxic seizures but the baffling aspect is that it is only happening in school and doesnt happen at home. I am wondering more about behavioral stereotypies specially complex motor tics which are relatively more common in children on the spectrum or ADHD.

My 2.5 year old has had this since she was born. She had her first one not even 24 hours into life. She has them at least 3 times a day .. can be a lot more depending if she a tired and startles more. I think she has the RAS because it doesn’t hapen after crying. She only cries after. And she gets pale. She tries to walk to me when it happens. But she drops to the floor and has those seizure like episodes every single time. Should I have her seen to rule anything other out ? I just read it could also be a potential to QT syndrome ?

The fact that it started in 1st 24 hours of life makes it a bit odd. I would definitely suggest that she gets reviewed by Paediatrician and if possible an EEG and an MRI of brain to rule out epilepsy and structural brain abnormalities.

Well though reflex anoxic seizures are more common in preschoolars, they can occur at any age according to Scottish NHS guidelines which can accessed here at clinicalguidelines.scot.nhs.uk/rhc-for-health-professionals/guidelines/primary-care-referral-guidelines/medical-paediatric-pre-referral-guidance/reflex-anoxic-seizures-in-children-advice-for-referrers/?ssp=1&setlang=en-GB&safesearch=moderate. However it is important that video EEG be done on your siblings to rule out any underlying seizure disorder which might happen by chance, concurrently
