Excellent and balanced video, Shay and Brian. It's so important that people know that sound, even at low levels, absolutely can worsen hyperacusis. Many audiologists don't think this is possible. Despite the fact that the cochlea is the size of a pea, cannot be studied in a live human, etc., they often think of the ear in very simple terms and in terms of what's normal for healthy ears, when in reality, there are complicated pathologies, nerve inflammation, etc. With this said, given the state of treatments, I think most people with loudness hyperacusis should consider at least trying it, starting very very gently. To just add to the discussion, certain forms of loudness hyperacusis can also worsen from sound therapy, but like both of you said, this occurs when there remains an underlying injury from whatever pathology. Regardless of pain, loudness, or both, people should always be careful. Just like it is bad to push through pain, it is bad to push through worsening loudness hyperacusis if enflamed nerves are being aggravated. Thank you for raising awareness, providing information, and adding nuance. I completely agree with Shay about pink versus white noise. Low level pink noise is probably the best place to start at barely audible volumes and short duration.

The issue with sound therapy reminds me of GET, the previous recommendations for doctors to get people with ME to push through their fatigue and gradually increase their exercise tolerance. So someone else may be feeling tired and out of shape and it’ll help them, but for ME sufferers, it tends to exacerbate the condition. So when it comes to pain and fatigue, they are very complex issues. I personally have very bad pain hyperacusis but I tend to treat it much as I treat my underlying chronic health condition (hEDS). It means I push through the inevitable tough days when I have to go out and mix with other people (which for me is mostly because of doctors appointments), and then I rest in between. The rest is disproportionate to the activity so it means there’s more sleep and rest than active time. I just can’t keep going like clock work because my system is not set up to live life this way. From this perspective, I can’t imagine putting up with constant noise in my ears other than the tinnitus that’s already there. That would be like not giving myself all the rest that I really need in order to function when I really have to get things done. It’s not just about being a bit sensitive, it’s about being seriously debilitated, and it’s not a joking matter. Either way, I don’t agree that my life is lead in complete silence. I think there’s tons of sounds all around all the time, even on the most quiet day out here in the sticks where I live. The tinnitus itself is sound, too. I think for people with neurological conditions, the idea of pacing is perhaps worth applying to all forms of expressions of illness. Doctors don’t typically understand chronic illness and so they won’t understand chronic pain hyperacusis either. Everyone has to find their own rhythm, their own strategies. It’s possible to live with these problems but of course it all comes at a price. You may have to give up on quite a few nice things in life just to survive the illness. But it’s always possible to see something positive in all the strife. Endurance tests can give rise to mental toughness.

Very interesting as always my friends 😀 Brian, we have the same right ear... I do understand why in so many years I never saw people with hard pain hyperacusis having success with the TRT... Never. I do believe, as Brian, that it may be useful for some loudness hyperacusis (some people improve quite quickly), but for those with pain hyperacusis, it can worsen A LOT... So the main critical question each one with "hyperacusis" has to answer is : "What kind of hyperacusis I have ? Pain or Loudness ? " Thank you very much everybody☺

Its frustrating that politicians spend billions to destroy humans with weapons. The amount of aid and weapons send to ukraine is ridiculous to maintain their proxy war. Meanwhile, it took 20 years of research, meager funding for dr susan shore to develop her device to treat T and H.

My TRT maskers play tones .. dont play white noise or pink noise that makes my ears scream .. so using widex zen helped alot with my hypercacusis

I got much worse from my tinnitus, and develop very mild hyperacusis, because i change hearing aids. Does this happen to someone else around here?

Hi, a questiion please. How do I know the difference between loudness and pain hyperacusis, when my daughter gets headaches from traffic, assemblys, dishes etc? she is diagnosed hyperacusis with audiologists and has been given sound stimulators to use. Thanks in advance Tanja

mijn ervaring met mijn specialist was het zelfde ze zei oordoppen uitdoen en je gehoor terug laten wennen aan dagelijkse geluiden plus met een goedkope uitleg erbij dat het gehoor verlies erger wordt als je oordoppen teveel in houd.. zonder ziekteverzekering was ik failliet

@@ka3011 mijn ervaring met hyperacusis is jezelf goed verzorgen plus je oren en oordoppen aanschaffen natuurlijk en harde geluiden ontwijken.. maar doen je oordoppen af en toe uit succes 🙂

@@briannewman3552 How are you feeling now? My daughter got an acoustic shock. Diagnosed with ttts, tinnitus and hyperacusis. I believe the nerve is affected on her. So sad for everyone with this.
