I went low carb and in a year i had some fat still but my abdominal area is soft and i find that exciting. I really feel good. :-) now 60lbs later i have intrest in exercise and do some and picking up power. Getting better is cool.

Dr Natasha Campbell's McBride's, an extremely experienced clinician and creator of the GAPS DIET wrote her book "Put Your Heart In Your Mouth! What Really is Heart Disease and what we can do to prevent it and even Reverse it" in 2007. She felt the need to write the book as she was advocating a diet of animal fats, including meat, fish, eggs and fermented dairy (homemade kefir) to her numerous patients. This was in the early 2000s, Her book Gut and Psychology Syndrome was first published in 2004. As she was going against the mainstream dietary advice at the time, rather than having to explain to the hundreds of thousands of people following the GAPS diet why she was recommending her diet, she wrote a book about it. I am delighted to say that I have been on her diet for about 4 years, I recently did a CAC test my score was a big fat 0, and the cardiologist when reviewing the ultrasound called my heart the perfect machine. My cholesterol was around 330, DR. McBride would say "Rejoice!" and so I am.

The long distance runner who switched to sprinting blew my mind. It makes so much sense. I've always noticed at my gym, the overweight unhealthy looking people are on the treadmills, and the lean healthy looking people are at the weights. I see people jogging down the road, they almost never look healthy. 50% of them are overweight, and the other half look stiff and sickly.

Those MRIs are riveting! Great job explaining to lay people what we are looking at. I can't help but think how many strokes, MIs, early deaths, etc. could be avoided if patients could only see and process this on an individual basis! I believe the stumbling block is the expense of an MRI and the non-coverage by insurance for "healthy" patients.
@LaserChair
@LaserChair 3 года назад
That's a major stumbling block indeed. Where I live, you need to have serious symptoms before anyone even takes you seriously. Even then they try to resist having scans/tests done by skipping straight to trial and error with prescription meds. It's stupid that the only real hope of getting that help is taking a somewhat sociopathic approach by manipulating and deceiving.

55:00 Position on what percentage of carbohydrates in the diet benefits "most" people. This is highly subjective dependent on the person's metabolic health and activity level. I tried a LCHF diet instead of keto in 2020 with 112g - 13% Total Carb, 74g net intake and gained 10 pounds over the year, with cravings, binging, large waist gain, and mood swings. I strongly agree with Dr. O'Mara's take on this subject for "most" people. Just like Dr. Atkins suggested, adding back carbs (including honey) is possible over a 20g total or even net carb intake. This discussion revolved around athletic performance and visceral fat, but really did not delve into other health/lipid markers. Dr. Volek (among others) reported the difference in blood markers between even a ketogenic diet and a LCHF diet. This may be a good topic for a Round 2 discussion.

My score is 123. I tested about a year ago. And need a retest. It cost $150. I asked my family doctor for the test. She pleaded it doesn’t show the soft plaque. That was extremely strange because some very smart people recommended that Test. Much smarter than my doctor. So it was a bit troublesome trying to understand. She offered a stress test instead. But I refused. I have since learned that stress test will only pick up inclusion, blockages, of 50% or greater. And no wonder it’s promoted because at about $2000 for a hamster wheel test to $150 for a CAC score test. It’s all BS and about greed and profits. And my doctor ( U of F) can’t even figure that much out. The funny thing is, my doctor wants me to retest. Retest the test she didn’t want me to take in the first place? The CAC test is the best health test I have ever taken even if I didn’t get a score of zero. I now implement secondary prevention without waiting for a cardiac event to happen first like current recommendations prescribe. Because more profits are generated that way.

@@Bhatmann have you seen Dave Feldman videos regarding the carotid artery scans...can't remember what the test name is at the moment... but he was able to show progression of plaque with diet alone and subsequently showed reversal of the plaque with a carnivore/keto diet... might be a way you can get data on whether what you are doing is reversing the bad score on your CAC ... don't know how long getting to Zero CAC takes ...but it can be done 🤓

I bought your book and I agree with 99% of what you say I'm a 76-year-old male and the one thing that I do not agree with is I believe that a small amount of fiber is definitely beneficial, maybe when you're younger not so much, I think it is beneficial to have a little fiber in your diet. if I don't I am always constipated and having to use something like milk of magnesia about every three or four days a small amount of fiber something like Sealyham husk solves the problem and I am quite normal. I suspect that as we age your intestines are less active and therefore a little fiber is beneficial, probably not necessary in people 55 or younger. I have noticed that most of the people that say fiber is not necessary are under the age of 55, time will tell. My motto is I'm going to live to be 100 or I'm going to die trying.

Dr. Paul - had a question for perhaps another podcast day - which by the way, I listen to all the time. Any chance you can explore Sesame? I've heard that it has properties which block a lot of the bad Omega-6 aspects. I have been making great strides towards a healthier lifestyle (just had some grass finished beef liver the other day :) ), but every so often, I run into questions about some seeds. I know you typically avoid seeds and their oils due to the linoleic acid, but again, really curious about exploring Sesame (also, i think it tastes pretty good). Separately, if you could spend some time on Flax and Chia too, that would be awesome. I'd say I'm primarily animal based, but will have the occasional flax, chia, and sesame seed. Just curious to know what the truth is - thank you again! Love the show.

Great podcast! The only piece of info I’m trying to work through is the last bit about chronic cardio. I just learned about Phil Maffetone’s work in what he calls the Maximum Aerobic Function (MAF) method. This is right up your alley, Paul, because it has to do with beta oxidation of fatty acids and keeping lactate production minimized. Just curious to hear your thoughts on that, because it strictly based on heart rate control for aerobic exercise. Seems like it could benefit mitochondria.

First meal today was at 1P.M. 3 rashers of bacon, 2 eggs cooked in bacon fat and butter and a cup of coffee. @DrSeanOMara

These are NON contrast MRI scans of the cerebral arteries or “Circle of Willis”. Coronary arteries are not good to image by MRI bc of the dynamic motion of the beating heart causes artifact. CT scans can though but miss soft placque. If your brain arteries are healthy your heart arteries will be as well and vice versa.

@@DrSeanOMara Thank you for taking the time to reply. I should have realised... the "dynamic motion" issue is quite logical & was indeed mentioned during your discussion. Are you perhaps licensed for an international consult? I've had 2 CTA scans (2010 & 2014), now the won't do one due to the volume of calcification which obscures the arteries. This guy isn't a radiologist but is trying to pretend that he is. I'm board certified in radiology and have issues with most of what he says. Selected points... The fist images showing the volume of visceral fat are misleading. the last one is taken lower down by the umbilicus where there is always less visceral fat. i could take the same person on the same day and take different axial slices and make it look just like that. Im curious what software they are using to estimate total volume volume. you would have to draw in regions of interest by hand to include the peritoneum but exclude bowel and organs. we do similar things for valve areas for example but its pretty tedious and inaccurate He says "not an angiogram I minus doesnt use radiation" but it is in fact an angiogram, thats what the "a" stands for in mra, and it does use radiation, its just nonionizing radiation, and no radiologist would say I minus, you dont even use iodinated contrast in mri. you would use something that is T1 shortening like gadolinium. Wtf When he is showing the 9 months later no plaque image that really doesnt show you anything very interesting. You arent looking intra-luminally just at the contour. i see vasospasm for example that looks like that resolve in 30 mins on follow up scans. attributing it to dietary change is misleading at best. You would have little idea what changed and no idea why. When he is talking about mri being ideal to capture atherosclerotic disease disease in the brain where there is no motion. No shit but then he talks about someone having a myocardial infarction. Didnt help much then did it? I mean maybe there is a correlation between coronary disease and and cerebrovascular disease but probably should have done a cardiac gated coronary cta huh? I highly doubt that people are "getting smarter" because of increased blood flow from atherosclerotic regression due to dietary change. I guess its worth testing but he didnt prove it by saying its so. There is such a thing as multi-infarct dementia but while you can stop progression, it isnt really reversible. If there is a change in cognition i would guess its probably from decreased systemic inflammation but who knows. He says endorsement doesn't happen with other diets but that's not true, i know lots of vegetarians and vegans who endorse their diet. Most people endorse what works for them as if its best for everyone. He says that most of his examples are "n of one" but then says "show you what the science says". n of one isnt science. Making falsifiable statements and testing them is. He is making broad statements to try to make sense of trends that he sees and supports his theories using tools he doesn't understand. In the picture where he is pointing out the pericardial fat, that's mostly anterior mediastinum. Most of the pericardial fat is inferior by the diaphragms. And then Paul summarizes that all of this incontrovertible evidence that an animal based diet is the best. Huh? The guy wasnt even arguing for a carnivore diet but i dont think you can conclude that based on anything talked about. Just my 2 cents

Sprinting reference: he said 5/100 without any other explanation. I took it to mean 5 times for 100 feet each. I run a little and i find that i run a cycle similar to this. Hope that helps. Forward Go.

I thought he said 3 × 100 metres 5 times a week, just went back and found it was 5 ×100 m a couple of times a week, now to find out what blood flow restriction bands are! His sprint regime sounds super reasonable, In comparison to the ten miles 5 times a week!

Question for the doctors: Traditionally fermented wheat (aka sourdough bread) ... is this possibly an OK carb in small amounts the way small amounts of honey seem to be OK?
@qthirteen13

possibly ... the more healthy you are...the more you can tolerate... but objectively you want your blood glucose to rise and come down within 2 hours at the most after such an indulgence 😉

I am not a doctor, I recommend, butter, ghee, Tallow, suet, coconut oil, duck fat, goose fat. Any animal fat, no seed oils.
@korbitror
@korbitror 3 года назад
He has expressed in the past (and I'm paraphrasing here) that even the healthier options like coconut oil or avocado oil aren't that great, and they're possibly adulterated with seed oils as well. If you can tolerate dairy butter and ghee are great. I presume that lard and poultry fat are good if the animals aren't fed with corn, but they likely are however. The very best option is beef tallow and suet, or just not use oil/fat at all.

I'm curious if there would be a difference or improvement in someone that does a mix of faster (sprinting/speed work) with distance running while eating a low carb (animal based diet). I am a distance runner/athlete but include a mix of strength training and some faster speed work throughout my longer distance training. I have seen a huge benefit in my overall health in reducing my carbohydrate intake and eliminating traditional foods that are common for distance athletes (pasta and bread for example.)
@bidnow2946
@bidnow2946 3 года назад
It sounds like you have answered your own question. Like everything else, this depends on context and your specific goals. Combining distance running with lifting and HIIT is probably optimum (IMO) for overall health and well balanced athletic performance, however not likely to result in world record performance in any single event or bodybuilding show. As you know, low carb/keto is a completely different metabolic state which I equate to a diesel truck vs. a sugar fueled sports car. Lifting performance is fine, although one may have a slightly lower PR and apparently flatter look. Ultra endurance running (over 2.5 hours) highly favors keto. Sprint type activities are indeed hurt, especially at first, however they do recover. Lebron James does keto off season to lose weight, but not during the season. I expect that you have seen these: www.mensjournal.com/health-fitness/zach-bitter-100-mile-american-record-holder-he-also-eats-almost-no-carbs/ ru-vid.comvideos
