Great video!!! Only update is that thrombolysis is a bit out of date: STEMI (r/o takusobu) send to the cath lab with aspirin and cangrelor or another ADP antagonist. Thrombolysis has shown worse outcomes. tPA is ok when you don’t have access to quick cath lab though so there is still indication. At least this is what we learned!

I thought vasopressors were contraindicated in shock d/t increased SVR.

Great video. Could i have your opinion please. Patient male, 81 years old, with dry cough, without dyspnea, low pressure, cold hands & oliguria, 4 years ago diagnosed heart failure with e.f. 30%. Do you think that is a case of cardiogenic sock? and what is your prognosis? Thank you!

it could be a case of dehydration as evident by dry cough, low bp, cool hands, oliguria. However, if his bp is below 90mmhg and he has other symptoms of low perfusion and o2 delivery as indicated by cool peripheries and also if we consider low urine output then we can talk about cardiogenic shock.

I think this is too slow though, maybe you should work on getting it down to four or optimally 3 minutes. You talk too dang slow so speed it up please and thank you.

you are talking way too fast like someone giving a report to an attending .most of the people viewing this are just med students maybe next time speak slowly and explain more .
