Medications, particularly stimulants, remain the best documented and most potent treatments for ADHD. But we know that many of our medications, including forms of amphetamine (Adderall, Vyvanse), methylphenidate (Ritalin, Concerta), atomoxetine (Strattera), and bupropion (Wellbutrin) increase blood pressure and heart rate slightly in the first weeks and months of treatment.
A 2022 meta-analysis of almost four million patients failed to find negative cardiac health impacts from ADHD medications. But a more recent, longer term study (2-7 years), comparing those with ADHD and cardiovascular disease to those with ADHD but no cardiovascular disease, found that ADHD medications were statistically associated with hypertension and arterial disease. Longer use, and higher doses posed the biggest risks. But the medications were not linked to arrhythmias, heart attacks, strokes, heart failure or other serious events with blood clots.
Numerous headlines claimed that the new evidence showed how dangerous ADHD medications are. Although the research has important public health implications, most patients should be reassured by the low risk to any particular individual. Only 4% of the participants had a newly diagnosed cardiovascular condition arise during the years of the study.

Over 4% over 4 yrs its also not a indicator for cardiovascular long term risk. Maybe it will develop in 10 or 20 yrs. I take VIACORAM blood pressure meds and they can't bring my pd down.

Yes, we are very much in the dark about whether the long term risks are large or negligible for most people.

Hello Dr. Kruse. I really am happy I found your channel and all the wealth of knowledge you share. Thank you! I have a question, if I am allowed to ask. I take Vyvanse and Pristiq. I have taken this for the last 3 years. I am in good shape, good weight, and blood pressure is normal! I do have sinus bradycardia which is monitored - with the slight increase in heart rate from Vyvanse or ADHD stimulants would I be in a more optimal position given my resting HR is typically slower. Curious on your thoughts! No problem if you can’t answer! Thanks!

I can't give medical advice via this type of interaction, but I can say that I have seen other patients with bradycardia who had increased heart rates to a normal range when they were on either Vyvanse or other drugs that boost sympathetic nervous system tone, like either of your medications can do.

At least one cardiologist says that right bundle branch block "should not preclude" the use of stimulants, but you should definitely consult with your cardiologist on this. www.ncbi.nlm.nih.gov/pmc/articles/PMC9626759/
