UWORLD had a similar question for breast engorgement and it said for cabbage leaves the mechanism of action is unknown but may be related to the cold temp or a substance from the leaves!

People don't really know why cabbage leaves work. It does have anti inflammatory properties but that of you ingest it so IDK what topical effects make it work but apparently it works really well. Some say to not use it as often as it can reduce your milk supply though. That's pretty cool though, I like it when we can find some more cheap and natural options when possible..
