Thank you for sharing this doctor! I think my physician may have missed out this; I'm having constant neck pain and neurological signs after a hyperflexion fall yet the mri shows no abnormality. Time for me to get a second opinion, probably.

It's not uncommon to have a normal MRI despite symptoms. I offer second opinions of you decide to go that route

I have multiple changes in my MRI as I see in this video many disc protrusions a nd subluxation not even stated. Go visit a chiropractor, massage your neck gently from all sides. Be careful front has nerves so don't go hard but do soft massage and daily for 8 days and see the difference.

Dr. Agten, as always, great content, thank you very much. Would it be possible for you to post a video on dynamic cervical spine MRI? There is not much online as to how to assess them properly. Again, thank you for the amazing and concise content.

Hi Bernardo I have no experience with dynamic spine MRI but would think you assess each position and then compare stuff that gets compressed in which position.

I have OA in the C5-C7 cervical spine and possibly in the lumbar spine and have loads of pain. It would be good to see if there’s bone Odeama in the affected sections and in the lumbar spine.

Nice cases. I have a query. The case with facet arthropathy, how do differentiate it from the infective condition.

. I would say the clinical presentation is different in many cases. Sometimes it's unclear clinically. On MRI I use the following to distinguish between OA with inflammation and infection. First, extensive soft issue reaction and bony destruction are helpful signs, as with OA we don't expect that really. Also, the degree of degeneration is less than expected for the amount of inflammation. But that is tricky as the patient might have had an infiltration with steroids and now a iatrogenic infection. So the history remains a key factor ( immunocompromised etc?). Occasionally we do biopsy or try aspiration. It's not 100%😁

Any fluidsensitive fatsat will do, TIRM, STIR, T2FS, ETC

Dr Agten. First time subber on here. I'm a 70 yr old female with cervical facets disease C2-C6 and have have PT for awhile with massage and it made the pain worse. I also was prescribed Pregabalin 50 mg X3daily. I'm not feeling any pain relief, in fact I'm feeling needles on my arms and ear pressure with pain. Any idea? My Dr.says I need a Neorologist. Steriod injections? I can't hold my head up, it wants to drop down. Your thoughts. PLEASE! Thanks so much.

Hello Dr Agten. do you believe its possible for cervical facet joint to cause constant headaches in the temple area and constant pain on sides of neck .. my boyfriend has had these pains for 2 years now. he was an athlete who used to exercise with exercise bands around his neck ..

Dr. A - do you have an example of C-Spine Rotation Measurements? I find that Rotation is rarely reported. Facet Over-hang is also rarely if ever reported. - And I've noticed in some imaging one will report a Rotation of some 9.8 degrees, where I see 5.8 Still I'll never understand why a C-Spine MRI never includes The CCJ - C0/C1/C2... Thank you for your Channel!

It's also a topic of unclear evidence and lot of alternative stuff with manual treatment of some dysfunction of the joint But there is so much mobility up there that angles will overlap. Normally I don't measure But I still haven't found convincing evidence or good articles about it If you have have please share

@@DrChristophAgten unfortunately the articles are all sourced from Regenerative Medicine Professionals. Rotation CT I understand But lateral mass C1 overhang on lateral flexion is only a big Regen tool. I've been studying more about these others like - Grabb Oakes, Harris BDI, BAI, OAI, Powers etc...
