Neurosurgeon Dr. William Tobler and physical therapist Lisa Cleveland talk about how to best diagnose Chiari malformation. Importantly, Dr. Tobler discusses the symptoms, etc., but he also explains how the decision to operate is made... and it's not so black-and-white. While he describes what most surgeons look for when assessing when a patient does and does not need surgery, he also admits that there is "not strict agreement amongst surgeons about when you do surgery, and what kind of surgery you might do." On top of all that, he describes what patients and their families may expect after a Chiari surgery is performed-- you may see resolution of all symptoms, there may be no change, or symptoms may actually progress and get worse. Dr. Tobler does an excellent job explaining what to expect before, during, and after surgery. But there is an unfortunate amount of grey area in the surgical management of Chiari malformation.
Because of that, Ms. Cleveland jumps in to talk about the role of physical therapy in managing symptoms and pain in patients who may or may not have had Chiari surgery. There is a lot that can be done outside of the operating room that can help patients struggling with Chiari, syringomyelia and related disorders-- it's just important to know what to ask for and how to identify if it is, or isn't working!

OH and I would like to add, I had a surgeon at MT Sinai say that the cerebellum wouldn't change after surgery at all (mine is still low and very very crowded) and I was told the surgery would NOT change the crowding but looking at your before and after surgery pictures, its clear that if done right, it should change the crowding. I still have no CSF flow at the foreman magnum.

I had crianecomy and laminectomy last year and it’s been a uphill battle, I’m so tired of the pain. I just wish it would go away already.

I've been watching these videos for a couple of days now but they all seem to be talking about Chiari type 1. What about Chiari type 2 which is what I have. Why is there no videos on the site about this type of chiari? Either way its all very interesting and informative

statistically it usually takes about 2-4 hours. I had this surgery done and it took 8 hours though. each patient is different

Temporary worsening of symptoms up to 20%????? We have to do better than this. I am sadly one of the 20% AND its not temporary and keeps getting worse. I was told I needed surgery to prevent nerve damage and I have WORSE nerve damage now.

What about the Chiari Institute in Spain? there is a completely different method of treatment

Same. I had surgery due to worsening nerve related symptoms and all of my symptoms either stayed at baseline or worsened post surgery. What a nightmare.
