Dr. John Oró, of the Colorado Chiari Institute, gives an amazing review of the brain's anatomy, particularly why the part of the brain affected by Chiari malformation. He gives an excellent description of the symptoms that can show up in patients with Chiari, particularly the symptoms just recently being understood such as gastrointestinal distress, cognitive deficits and others.
Dr. Oró also describes some other disorders that may cause headaches and other similar symptoms to Chiari. The presence of these conditions might help explain why an original Chiari decompression would not have helped the patient the first time around. Two of the disorders that he goes into more detail about are called idiopathic intracranial hypertension (or IIH, also called "pseudotumor cerebri") and spontaneous intracranial hypotension (SIH).

I wish you were still doing surgeries. My daughter just got diagnosed with chiari malformation. She's 22. I don't know who to send her to here in Indiana. I saw you in 2016. We drove all the way from Indiana.

I was diagnosed in 04. 8mm. Symptoms minimal but started to dictate lifes choices. Maybe shifting image time from every 2 years to an on call study may drastically change findings. Also, change the term of head ache to a pain in back center of head. I'm not sure about others but I get kinda bugged when someone says headache. I have tried to ignore my chiari but had a new event today. I believe the herniation or what ever you want to call it can instantly go from, example 8mm to 20 or more then back in seconds. Increasing pain unimaginably for a little bit. So maybe run tests were we can image at any given moment in order to see if there is a difference. Also, Sometimes when it feels worse momentarily, I get the same taste in my mouth as if I fell on concrete hitting my head. Since about 08, I've had to work from the time I wake, until 12am ,1am or until I am exhausted in order to hopefully sleep

Fantastic lecture! I sure wish you were my neurologist. I think I may have Chiari Malformation which has been dismissed by two neurologists. Headed to a third in a few months.

Hi I was diagnosed with a benign meningioma in 2019 still awaiting surgery but on my 2nd MRI they also discovered a Chiari Malformation 1 I have lived with severe vertigo and balance issues and occasional pain during coughing bouts at the back of my head I have a continuous tinnitus and sore eye from blocked ducts and vision problems in my eyes. The thing is the tumor is pushing in my cerebellum pointin angle putting pressure on my brain stem will the tumor removal ease the symptoms as the symptoms are similar for both tumor and Chiari

Do the symptoms always have to be severe. I have been dealing with this for years, but have not had the "severe" symptoms as described here. Can this cause other issues such as PVC's?

The reason the first decompression doesn't work can't be quantified when there is no concensus on how to perform it, especially in years past.
