Episode 43: Chronic Kidney Disease Symptoms for Stage 5 Kidney Failure - what you need to know about symptoms, treatment, and ESRD renal diet. For those just diagnosed with CKD stage 5 (ESRD), this video gives an overview of how your kidney function is determined and then dives deeper into the common chronic kidney disease stage 5 (ESRD) symptoms you may experience. The CKD treatment outcomes highlight what the goals for treatment typically are for ESRD Stage 5. Your kidney diet (renal diet) is extremely important to slow down, stop, or reverse your kidney disease and I cover some of the common diet changes recommended with Chronic Kidney Disease Stage 5 (ESRD).

A person with stage 5 chronic kidney disease (CKD) has end stage renal disease (ESRD) with a glomerular filtration rate (GFR) of 15 ml/min or less. At this advanced stage of kidney disease, the kidneys have lost nearly all their ability to do their job effectively, and eventually, dialysis or a kidney transplant is needed to live.
CKD Symptoms Stage 5 (ESRD)
Symptoms that can occur in stage 5 chronic kidney disease include:
• Loss of appetite
• Nausea or vomiting
• Headaches
• Being tired
• Being unable to concentrate
• Itching
• Swelling, especially around the eyes and ankles
• Muscle cramps
• Tingling in hands or feet
• Changes in skin color
• Increased skin pigmentation
Because the kidneys are no longer able to remove waste and fluids from the body, toxins build up in the blood, causing an overall ill-feeling. Kidneys also have other functions they are no longer able to perform such as regulating blood pressure, producing the hormone that helps make red blood cells and activating vitamin D for healthy bones.
If you are diagnosed with stage 5 CKD, you will need to see a nephrologist immediately. This is a doctor who is trained in kidney disease, kidney dialysis, and transplant. The doctor will help you decide which treatment is best for you-hemodialysis, peritoneal dialysis (PD) or a kidney transplant-and will recommend an access for dialysis. Your nephrologist will develop your overall care plan and manage your healthcare team.
Dialysis treatments for people with stage 5 Kidney Disease (ESRD)
Most people report feeling much better once they begin dialysis. As the toxins are removed from the blood medicines replace the functions the kidneys can no longer perform, they find they can enjoy a good quality of life. There are two types of dialysis treatments for people with stage 5 kidney disease.
1. Hemodialysis: This treatment can be done in a center or in a patient’s home with assistance from a care partner. A dialysis machine removes a small amount of a patient’s blood through a man-made membrane called a dialyzer, or artificial kidney, to clean out toxins that the kidneys can no longer remove. The filtered blood is then returned to the body.
2. Peritoneal dialysis (PD): Unlike hemodialysis, PD is a needle-free treatment and a care partner is not required to help assist during treatment.
Kidney transplant is another kidney failure treatment option
If you wish to have a kidney transplant, tell your nephrologist. Your nephrologist will explain the process of how to get on a waiting list for a cadaverous kidney or how to find a living donor.
When at stage 5 CKD (ESRD), either dialysis or a kidney transplant is necessary to continue living if you are unable to improve your kidney function through diet and lifestyle changes.

Hello everyone I truly hope that all of your GFR's increase. I have stage 5 kidney failure and I was working out 5 days a week and didn't know I had kidney failure. When I found out my GFR was at 4. Yes a GFR of 4 so I was having all the symptoms of someone with kidney failure. Your diet is key. I do not drink, dont smoke, and dont eat red meat, and I only drink water. Your sodium, potassium, and phosphorus are something you need to pay attention to for the rest of your life. Three months later my GFR is at 17. So stick with the strict diet otherwise you'll probably need to get on dialysis. I wish everyone reading this the best. It is so serious.

When I watch my older videos I always cringe - I would love to take them down and replace them with newer versions...but they serve a greater purpose. They show people me actually improving and getting better right before their eyes. Unlike many other Kidney videos, I AM a kidney patient fighting back from Stage 5 - and I leave those videos there so that others can see exactly what you saw...a person facing an uncertain future get better...and better...and better. I'm glad you found my channel. I'm still at Stage 3 but super excited that I have no symptoms. I've been given a second chance at life and will use it to help inspire others. Thanks for sharing! Are you currently working with a dietitian? They can provide a lot of support and guidance.

This is a totally confusing illness for me. My mother now 89 was diagnosed with end stage renal failure 5 years ago. She basically had to choose paratinial dialysis due to very very poor veins from chemotherapy back in 1981 which made it impossible to do the 3 times a week treatment plan. When we meet with the kidney specialist she was at 8% kidney function. The doctor felt she could assist her in living approx. 3 years. With her dialysis she dropped to 3% kidney function. Upon meeting with mother's doctor from her first visit I reported to her that my research in this disease showed in Australia there medical field was seeing higher induced kidney function by adding sodium bicarbonate to the treatment. The doctor was completely against this. When mother's function dropped to the 3% I absolutely insisted on giving her a prescription for this. Her blood work in 2 months showed an increase in function upto 16%. Now keep in mind my mother still worked M-F, 8am-4 pm in her 80s. We went into the dialysis blind, they made it sound so easy. Plug yourself in at night, unplug in the morning. She hated this, I hated this. Hundreds of pounds of supplies and you being held hostage to this nightly treatment, creating horrible pain way up in the front of her shoulder. Mom got where she would refuse the treatment 1. 2 and even 3 nights a week. The nurses getting onto her that one morning she would just not wake up. She did dialysis for approx. 2 years total and finally 3 + years ago she said she was done. She was instructed hard and heavy that by making this choice to discontinue dialysis her life would come to an end very rapidly, less then 2 weeks but more like 10 days or less. My mother is still alive. When she got Covid nearly a year ago she was 8% kidney function and her test 9 months prior to that she was at 8%. What is going on. I have stayed with mom through this entire journey checking on her 3-5 times a night. I have been held hostage by this constant fear of losing my mom. Can anyone explain this. I do not know what signs to look at when time is getting closer. She still walks, does cooking, cleans and maintains her daily health and beauty routine. She still has her urinary functions. I personally just need to know how this looks when the end is near or will it just be so noticeable? I know I am blessed to still have my mom but the constant fear of when, what to do to make the transition as painless as possible has me tied in knots. Can anyone help with advice. All I can say is my mother is a tough old bird. She was never to live past 1 year with her cancer in 1981 and here it is 2022. MD Anderson was amazing at saving her life as well as we KNEW WITHOUT A DOUBT she was not going to go then. I told her doctor we looked at it as a wart, cut it out we had to much living to do and I would not accept anything less. Mind over matter is nothing short of a miracle and so POWERFUL. Lynn, Dodge City, KS,

I can’t thank you enough James for all your videos and your advice. I was born with kidney disease. I didn’t have to worry about it too much until I turned 11 years old when my autoimmune disease just damaged my kidneys even more. They told me that I would need a transplant by my 15th birthday and I just turned 29 years old and haven’t had a transplant yet. But sadly, last year in 2020, they told me just 3 days before Christmas that my GFR was at 13 and my kidneys got to stage 5. I didn’t know the symptoms of stage 5 kidney disease when I first got it. But when they showed me the list of symptoms of stage 5 kidney disease, I had every symptom on that list. If I knew what the symptoms were earlier, I wouldn’t be so surprised when they told me. But the last time they checked, my GFR went back up to 17. So all of your advice is working. So I can’t thank you enough for all of your wonderful advice and for giving us all hope. P.S. I love how you start a lot of your videos with “ kidney warriors “ because my entire family has called me the kidney disease warrior my whole life. 😊

Ive been told im not a candidate for dialysis because of the type of disease and to over weight to do surgery. I keep my blood pressure in check with meds and i have changed my diet and gone off pop. Ive already lost weight. I dont have any other complications so is it possible to slow or stop the progression? I am so scared right now. My 42yo son was diagnosed with end stage kidney failure after he had stomach problems and was immediately put on dialysis. He was on a keto diet a few weeks prior to this cos he had gained a lot of weight. The reasoning behind this, his dr told him, is he would eventually need dialysis so might as well do a "bypass" instead of having to take medication. After 2 months of being on dialysis 2x a week, he has lost a lot of weight and is 1.5 kl short of 78kl which is supposed to be his ideal weight. My question is: Is it possible to quit dialysis if his lab test results show a lot of improvement in the coming months? I think he was railroaded into doing dialysis without trying try doing it without. I would appreciate your thoughts on these. Happy new year from a bothered mom.

Every dialysis patients are very much different from their condition. I know it because I have end stage renal failure & I have a restricted diet. Dialysis patients can exercise too but try not to exercise too much.

Using diet and lifestyle changes (known as Conservative Management) I now suffer NO symptoms and feel amazingly great. I recently went back and watched some of my first videos and can not believe how much I have improved. While it seems like so long ago, my first video was only made back in February - so I have improved rather quickly. While most of my videos focus on the foods I ate and foods I avoided, it is just as important to manage your blood sugar and blood pressure. Also make sure to address the underlying cause to stop further kidney damage. These are areas where a dietitian could be EXTREMELY helpful. Good Luck!

@@soohyunali7786 The good news is I only used medications to lower my extremely high blood pressure - everything else I have done to improve my health is all through diet. No special foods either - more eating less processed and more basic foods. What is important is to get blood work done so that you know what your body is missing (what it needs more of) and what it has too much of (what you need to reduce or avoid). Having that information gives you limits to watch for as you select healthy foods (less processed). I'm always happy to help answer questions - though I am not a doctor and can not prescribe any treatment, your doctor is ALWAYS the best source for your treatment.

Is it still possible to follow this diet even if we are in dialysis thank you
@DADVICETV
@DADVICETV 4 года назад
The diet is similar, but your daily targets will be very different. For example, those on dialysis usually need more protein and have a lower daily fluid target. This isn't designed to get someone off dialysis, it is designed to help people eat a low inflammation kidney-friendly diet. I try to limit myself to speaking from my own personal experience and what information my health team has shared with me. I was very lucky to be able to completely avoid dialysis, so I'm unable to speak about life with dialysis. I do encourage those on dialysis to check out Lee Hull's book "Stopping Kidney Disease". It is a challenging read, full of lots of research backing his method of getting do dialysis. It is very similar to the treatment strategy I am using, though he relies on a few different supplements unique to those on dialysis. You can check it out on Amazon at amzn.to/2PG0Ovc

@DADVICETV
@DADVICETV 3 года назад
GFR 6 is a good place to start dialysis - getting below 5 can be extremely dangerous. As far as any chance of improving, it varies person to person. My lowest was 8 and I got up to 13 while in the ICU. Life was very difficult for my at 8 and I wasn't able to function or think straight. If I wasn't able to maintain at least a GFR of 10, I was going to start dialysis. You can try diet and lifestyle changes if there are things you haven't done yet - but there is no guarantee you'll improve quickly. I wouldn't feel comfortable at GFR 6 - that is extremely low - so I would start dialysis.

@@DADVICETV Thank you sir for doing service with these videos. My sister's createnine is currently at 585 because she has type2 diabetes. Recent blood work shows that her bp is normal at 120/70 and her fbs is at 96. She's been on low carb. The only thing not going down is createnine. So we're giving her more plant-based while being careful with potassium and phosphorus.

@DADVICETV
@DADVICETV 4 года назад
Something is causing this rapid decline and it is important to find out what it is. Treating the underlying cause is the first thing doctors try to do. This allows them to stop further damage. I'd also check with her on any changes to her diet. If there where no changes, perhaps something she consumes often has been modified and the ingredients now contain something putting stress on her kidneys. I'm not a doctor and these are just shots in the dark, but I would work with her to help find the cause of the rapid decline. A GFR in the low 20's usually starts showing symptoms, and a GFR in the teens was very miserable for me - I felt like life as I knew it was over and gone. I'm so thankful today to be Stage 3 and have my energy back. I've been given a second chance at life and this time I'm going to be as healthy as possible and pay it forward by sharing what has worked for me. Does she currently follow a renal diet?

Hi my mum is stage 4 and she's very worried. I though you can't reverse stages?
@DADVICETV
@DADVICETV 4 года назад
this is where things often get confusing. While it is true that you can not "repair" or "fix" physical damage to your kidneys, it is possible for many people to live with kidney damage without needing dialysis or a transplant. Now not everyone can do this, but many can and the only way to know is to try. I was diagnosed stage 5 kidney failure with my lowest GFR being 8. That isn't a typo - yes, a single digit. While in the ICU they got me up and stable at GFR 13, which is still kidney failure. I was told that was the best I was going to get and that there was "Zero Chance" of improving. I was told I had to go on dialysis or I'd die. I make immediate lifestyle and diet changes and got labs daily to track my progress. Today I am GFR 33 (probably even higher, but can't get labs due to world events) and have NO symptoms. Kidney Disease does not hold me back at all. My kidneys are still just as damaged, and I will quickly get back into kidney failure if I go back to my old lifestyle. So moving up stages is possible. Fixing kidneys is not. But what matters is the quality of life and symptoms. Get symptoms under control and life is amazing! Working with a dietitian, avoiding things that cause damage (ru-vid.com/video/%D0%B2%D0%B8%D0%B4%D0%B5%D0%BE-fAob_OJgQ2M.html), managing blood sugar and blood pressure are all helpful in improving what matters. Good luck and keep me updated on how you mum is doing!

I'm continuing to improve. My labs now have all of my blood within the standard range - not a single one too high or too low outside BUN and Creatinine (which will forever be high). Losing lots of weight - just ordered all new shirts size Large (down from 6X last year). Tweaking my modified keto diet to get BUN and creatinine lower to keep my GFR moving up.

@@jedviola2016 From speaking with doctors, they told me the absolute best I can hope to achieve is Stage 2. It is currently not possible to improve your kidneys 100% - there will also be some damage, but at stage 2 I have plenty of buffer to enjoy more foods as occasional treats than I do today. My goal is to get to stage 2 by Jan. 2020. Can it be done? Who knows, but the fact is NO ONE KNOWS. So I'm planning on doing it. :-)

Information is going to be your friend. Working with a renal dietitian is a huge help if you can - diet is one thing we control that can help reduce the burden placed on our kidneys and reduce or eliminate some symptoms. The secret is to LIVE HEALTHY - do what is healthy for your body and heart. There are no magic pills or secret diets - it's all about living healthy.

Be careful - those are not helpful for people with kidney disease. Work with your doctor to determine what is right for you and watch out for all the fake and/or dangerous supplements being pushed. These crooks try to take advantage of us with their junk supplements, draining our wallets and sometimes hurting your kidneys!!! It is a shame they do these, but greed is their driver instead of kindness.
