A lot of people with high blood pressure, diabetes and heart failure take ACE inhibitors. Recently there have been some reports that ACE inhibitors may expose patients to a more severe form of COVID-19 infection. In this video, I share my understanding of this subject.

Thank you for taking the time to do this , i am on ACE inhibitors for my hypertension and whilst it is stable as is my type2 Diabetes. i had read an article about the virus possibly causing complication with people who take ACE inhibitors. you have good way of descring this process in clear terms and you have given me some peace of mind. i am very gratful to you.

Thank you... Recently just started back on an ACE...was supposed to have weekly basic metabolic lab done to see how my kidneys are handling the drug..had a problem before...but now no lab work is allowed unless a stat order from your doc because of the stress on our medical system. So, I am making the executive decision to go back on the calcium channel blocker.. Thank you for your time and very thoughtful, articulate videos.

Thank You Dr. I’ve been taking Lisinopril 10 mg for one year after having a hypertensive episode. I am doing well on this medication. Thank you for your explanation related to ace inhibitors and the covid19 virus 🦠 it was very helpful. Your calm voice helped me listen more intently and understand better.

Thank you so much for an excellent video, so well explained. I contracted a virus which gave me severe heart failure 13 years ago, so have been worried about keeping taking my ACE inhibitors, but knew I had to. I think your great.

@Elliott Fire & Ice Dr RU-vid is always there to help you. Low vitamin D is one known cause of general pain. Low GABA also increases sensitivity to pain. Have you tried finding someone to help with nutrition in general?

@Elliott Fire & Ice Have you had testing for your Vit D3 level, Vit B12, iron? Pain is greater if Vit D3 is low. What about taking magnesium malate or glycinate? You could rub on magnesium version on your feet. Blood testing for magnesium is not reliable as it is serum level in your blood. Read about maganesium with Dr Carolyn Dean. What about taking a Spectra cell test to find our what you are low in in micronutrients? Look into alpha lipoic acid for neuropathy. If you're a MTHFR patient, taking Vit B12 should be the hydroxocobalamin version or methyl cobalamin version. Not synthetic cobalmin. Cut out all sugars & processed foods. Garbage in..garbage out as pain. Look into red light emitting diodes LED red light therapy (photobiomodulation) for neuropathy. If you're low in your b Vitamins, make sure you take the active versions as supplements (B2, B5, B6, B12).

Excellent, thank you for a cardiology perspective on this. My dad went off his ARB a few months ago because he didn't need it anymore but I'm happy to learn what I can in case he ever needs to go back on it.

When I first learned how the corona virus attached into the lung cells I became very concerned about the involvement of ACE in that mechanism. And since I take an ACE inhibitor I have been worried about how that would combine with Coronavirus. You put me at ease. I hope to not get it, but I will know what the right thing to do with regard to my medication. Gratitude for your kindness, thank you.

I'm not sure if this is what the Dr. said, but what I heard is this: * Patient P has diabetes or another long term condition * as a result too little blood reaches the kidneys * the kidneys compensate by secreting ACE hormones resulting in more blood reaching the kidneys, satisfying the kidneys' craving for blood * because the condition is long term, over time this stresses the heart * so the medical community prescribes ACE inhibitors to inhibit the kidneys' efforts to obtain more blood * this thwarts the kidneys' corrective action and relieves the heart of the long term stress of providing the kidneys with more blood * the result of defeating the kidneys' efforts to obtain more blood results in more ACE hormone in the body * in theory, since the corona virus' "crown" latches onto the ACE pathway in order to invade the cells of the lungs, it would suggest that stopping the use of ACE inhibitors would be the smart play * however, there is no HARD evidence that this is the case * and, since there is hard evidence that having a heart weakened by prolonged production of my blood at the behest of the kidneys' injection of ACE hormones makes one more prone to heart failure during a bout of corvid-19, the current recommendation of the cardiac professionals is to stick with pharmaceutical compensation to undermine the natural compensation of the kidneys Am I understanding that correctly? If so, am I also correct that the hypothesis that by not taking ACE inhibitors during a corvid-19 pandemic one is better positioned against the corona virus has NOT been shown to be false but is simply unproven and thus not recommended? If so, wouldn't it simply be allowing the kidneys to naturally compensate for disease and get the blood it needs without the risk of long term stress on the heart, since you are only quitting the pharmaceutical compensation during the pandemic? IE: The long term stress to the heart is not a factor? Personally, I think there is possibly (and I'm speaking simply as an uneducated, non-medically trained internet opiner) a disturbing parallel in the short term thinking of the cardiac pro community's approach to pharmaceutical "compensating" versus "decompensating" to that of dragging away fallen tree limbs to prevent small fires resulting in uncontrollable huge fires (in forestry). My wife is on ACE inhibitors. Our family doctor agreed with this doctor. She is continuing the medication. Thanks for posting. Scary world we now live in. I thought humans had become the dominant species on Earth.....maybe not. Beat of luck to you all.

Dr. Gupta, this is exactly what I've been wanting to learn about. Covid 19 attacks the respiratory system and experts include diabetes and hypertension as part of the high risk group. Wanting to understand how these are related I've had to piece together bits of information from many various sources without much success. Here you give us very thorough information.

Serious respiratory system problems in COVID-19 usually means LACK OF OXYGEN to vital organs. Kidneys usually the first to go in ICU treatment.

Dr Gupta - I'm an RN and, for a long time, have been trying to get a better understanding of what exactly drives the pathologic activation of RAAS that is targeted by ACEIs/ARBs. I heard about renal hypoperfusion, but it never quite made sense -- most of these patients have just hypertension, not heart failure? But your 2 minute explanation made so much more sense. The kidneys are very sensitive to small drops in perfusion. That makes so much more sense, and it clarifies why RAAS activation could be chronic and pathologic. THANK YOU! One question -- what causes the slight hypoperfusion in hypertension? Is it just an increase in afterload that is not entirely compensated for?

Thank you very much for posting this! I was directed to discontinue an ACE inhibitor by my doctor based on hearsay as i can see now. I am very grateful for bringing clarity into this issue. I am going to stick with Telmisartan until I know better!

Ronald W. Jastal I take Telmisartan with Lasix water will. Only pill combination that manages my blood pressure. Typical readings are 115/74 range.

I'm currently pregnant and was placed on a Beta blocker as a precaution due to increasing blood pressure. It's labetalol. What an excellent explanation, especially of what an ACE inhibitor is and how it works. I was put on Metoprolol after my by-pass surgery last April and wondered what it actually did.
