D wilson Your mother is totally recovered from post covid interstitial lung disease after 12 months? Actually my mother is suffering from post covid ILD, after 8 months still having spo2 drop on walking or exertion. But spo2 rate at rest is 94 %.
@@subhadrakaysth5647 my mother is still not well, she requires oxygen nearly all the time and still had coughing fits. She tires easily and if she walks around with out oxygen her o2 will drop into 80s.

Dear Dr Raj, I had a severe Covid and still suffering from high heart beat and cough. The last CT I did after discharge showed some scarred tissues. I am wondering if I can email you to share the CT scans and more details and have your advise sir? Thanks
Superb !! I think still sometime and follow up n more data required to come to a conclusion ! But , this disease is really weird as you have shown some worst cases recovered and mild cases goes for scarring . We are seeing here similarly !
Hello sir, after 4 months of covid recovery my FIL is having breathlessness after exertion and chest pain. His follow up hrct shows "peripherally distributed Multifocal, multilobar ground glass opacities, patchy consolidations noted in bilateral lung parenchyma (extent involvement is 20% of lung parenchyma) s/o-viral pneumonitis-cov-19 Typical (co-rads 5)" Does this also mean fibrosis?
I wonder if I nay have fibrosis. I never had any other classic symptom except shortness of breath so not sure if I had the virus but now my lungs feel tight. It's been 6 months. Does fibrosis feel like tight lungs? My oxygen never gets lower than 95 though it's usually 97-100
Actually, It's all about the way of the treatment. The faster the Steroids to be given (Methylprednisolone for sever cases) the better the outcomes you will expect i.e. a lesser chances for fibrosis, specially if you add N-Acetylcysteine NAC both Oral and inhalation. Also not to stop anticoagulant after cure for 3 months at least.
Thank you for your kind comments. Unfortunately the has not been much literature or Publication around this as of now. Based on our experience a very small portion of patients do end up developing long term lung changes.
I have a question. After 4 months my ct scan report is - mild fibronodular changes in right lung. How do I determine that I have progressive fibrosis or I can stop this with treatment?

Dr.. a query in practice is anti fibrotic drugs being prescribed after discharge in suspected fibrotic group? Do you see better resolution In patients who are on drugs than who are not on any medication? Please share your experience..
There is no universally accepted treatment policy or guidelines for this. Yes there are units which are prescribing anti fibrotics at the time of discharge for people with severe disease. We are seeing some promising result in these patients.
Sir my question is directly related to What Dr R. Guleria stated , which is : Is it safe to get a CT scan done for moderate cases? Or is it avoidable ? Is it really true that one CT equals 300 X-ray? I didn’t get CT done because of that statement, but I am considering getting it done now given that I feel exhausted after 500 meters of running 3 months after recovery . I am a 29 year old active male weighing 70 kg and height 5’8
What Dr Guleria said is true and in many scan units the dose will be equal to 300 Xrays. Most of the units that have a high end scanner can reduce this dose significantly. Also the risk of radiation is hight only if you have repeated scans in a short period of time. So a single CT scan is unlikely to cause significant harm. You should consult a pulmonologist who can assess your lung function with some basic tests. We can then decide on timing of your CT scan.
my father hed covid 19, and after 43days he recovered but later he was diogionsed with purnomanary fibrosis and we are told that his lung is damaged 75% is it possible for him to recover?
It depends on many factors to be honest. Please wait for a follow up scan and see what has been the status of disease. This will help us in giving you a long term prognosis.
Because of post covid scars in xray, can't go gcc medical. But I don't have any health issues, but after taking 9 month from covid again the scars in xray. Very disappointed due to this can't go work in gcc countries

You have to repeat the scan in 6 months and see how much of it is resolved. More than 60% of patients get really well during this time. We can only discuss about prognosis after this scan.
My mothers o2 level is between 80 to 90 without oxygen support. With 1 to 2 litres of oxygen her spo2 level remains 95 to 96. She was maintaining o2 level 90 to 92 without oxygen support.But one day she did heavy lung exercise and that evening her o2 level dropped to 68.Also her pulse rate varied from 48 to 135. So we hospitalised her.Now she is on 2 litres of o2. She was covid positive on 14th of April and on Bipap for 10 days. Can she recover now

Sir my 67 mother has posted covid fibrosis on her right lung and no symptoms other than dry cough.. no medication was given by the doctor.. now after 1 and a half year in a routine ct scan we discovered a lung mass of 3 cm in the apical segment of Rul and few lung nodules too.. the doctor did a biopsy and pet scan.. the pet scan is saying the lung mass is Metabolic active.. Very good question. In a patient who has recovered from COVID or is already known to be COVID the CO-RADS score should be avoided. This will only create confusion as lung CT changes can sometime take much longer to recover compared to clinical recovery of patient. So, please do not mention CO-RADS in these patients.
Is there any symptom of blood comes through mouth. I have only the symptom. Very small amount of blood is coming with spit when getting pressure on chest. Is there any treatment?
Hi Sir, my mom had Covid 28/may/2021 with CT score 20/25 later in hospital itself it went to 25/25 CT score 2 months of hospitalization and reached home 2 weeks before with home oxygenation with 1 litres.spo2 98. Spo2 88-92 room air it fluctuate I am scared is it related to fibrosis. Please advise
After 1 n half year of covid. During chest xray for Visa of Oman the diagnostice centre declined by medical stating that:fibrosis on right upper zone .2 times medical refusal.any solution for this .
I think it would be best for you to undergo a CT scan to assess this further to begin with and then a pulmonologist can give you an opinion on how to take things forward

Secondly, I would like to inform you that I am still suferring from Oxygen Desaturation after walking 10 steps to 75% from 93% at rest cough and chest pain. The last CT Scan showed hyperinflated lungs ggo's with probable reticulation. May I share the CT Scan with you so that you can understand better?

The most weird thing is that my HRCT scan came back normal and the doctors said the whole thing is psychogenic. I don't really know whether they are crazy or not. Initial CT 13/25..Then 30 days COVID treatment completed.Even Remidisvir and all dosages provided..D dimer and C reactive become normal.we took CT scan next day of discharge( 30 days in hospital)and results as 25/25..It's panic sign or kind of shadow of this treatment. CT score of 15 is not same as fibrosis scores. These are two different things. You may perhaps fully recover, we can only be confident about it after following you up in 3 and 6 months and seeing how your lungs are performing at that time.
Excellent Analysis sir. I recovered & my CT score was 4/25. After 6 weeks from recovery, my spo2 is 93%. Doesit mean fibrosis & should I go for CT again?
@@kamatchipourna19 No. Instead spo2 increases when I check after walking. I feel chest is heavy like catching cold. I live in Bangalore & weather is very cold.
Check it while sitting upright and place your fingers on your lap and the reading after 1 minute would be the correct one. I think during the recovery phase lung is going through the process of healing hence it leads to drop in Spo2 level. Worry not, it might also be fluctuating between 93-97 . You can never be sure. It happens with me too, but most importantly don’t stress or be anxious. Half your battle is in the mind.
I dont think you should jump to have a CT scan. The recovery may be slow so you need to have patience. You should consult a pulmonologist who can assess your lung function with some basic tests. We can then decide on the timing of your CT scan.

Doctor my mother is 25/25 corads score.... During covid she went through different complications like tachy Arrythimia, ccf, increased APTT, UTI,... She had 2 month hospital stay approx 1 month on ventilator support... From last 1 month of hospital stay she started co2 retention on ABG report... Which still not resolved... For which we started keeping her on bipap machine st level... We set up icu at home... It's about 3month total since 1st symptom.... Still her co2 retention not reduced (last abg report =68) we Intermitantly place her on nasal prong plus bipap.... On nasal prong @1-2lit/min her spo2 is b/w=90 above..can you please suggest the prognosis and further treatment line of patient... Please sir pplease.....do reply

Findings: There is a focus of chronic subpleural fibrosis adjacent to the right mid-lower thoracic vertebra secondary to osteophytosis. There is no further pulmonary opacity or other abnormality bilaterally. The trachea and central bronchi are widely patent. There are no pleural effusions. There is no mediastinal or hilar lymph node enlargement. Limited assessment of the upper abdominal viscera is unremarkable. There is no focal osseous lesion. There is moderate to severe thoracic spondylosis with osteophytosis. Comment: There is mild chronic linear fibrosis adjacent to the right mid-lower thoracic osteophytosis.
Dr.. my husband was hospitalized for 5 weeks and CT score was 24/25.. now after a month the CT score is 22/25.. observation is diffuse interlobular septal thickening bilateral with associated fibrotic changes.. can you kindly review both reports if I may share the images.. our doctor suggests to wait for another month to see progress.. he is not prescribed any medication.. room air saturation is 92-94 but hypoxia on exertion.. can't even walk few steps..
Hello Doctor, Thanks you for the detailed explanation. I have a question, My father was admitted with CTSI of 23/25 and now successfully recovered and discharged from the hospital. Clinically now his Oxygen saturation is maintained at 96 and he is asymptomatic. But from chest CT taken after 5 weeks of discharge, doctors are suspecting the initial development of Fibrosis and recommending anti fibrotic drugs. Is it really diagnosable in 5 weeks after the discharge ? Should we start anti fibrotics? Please give your inputs on this.

Yes, there are some features which can develop that fast. Some of these actually do revert back to normal. Starting on antifibrotics- especially if he is able to tolerate it is not a problem. We can follow him up in 6-12 months time and then stop these medications if the fibrosis resolves.

In usually label fibrosis if there is Traction bronchiectasis Honeycombing Architectural distortion Sir what is your opinion when we only see reticulations without honeycombing should we call them fibrosis or post covid sequelae in resorptive phase? And your opinion on BJR publications categorizing fibrosis as PCVCT 0 to PCVCT 3 ?
The answers to your question, depend on the time that the imaging has been performed. Honestly speaking we have very little evidence at the current point and we will learn as we go along.
