Being responsible for 150 patients and covering 5 pagers is exessive. Was this happening in the "before" times too or is this due to the shortage of medical staff we're experiencing in Ontario now? This is really scary. You are an exceptional doctor but doing this on all calls can only lead to burnout.

It is really lovely to see you in full action and that you are now an attending physician. You sure have a lot of responsibility. Thanks for sharing insights into your work life with us. Would be nice to also see your husband again in a video with you occiasionally.

And this is your first shift ... what a crazy one. It's not difficult to see why SO many Doctors and Medical Staff are suffering burn-out. Your dedication, intelligence and enthusiasm are amazing. Just don't forget to take care of YOU.

As a patient you don’t see the behind the scenes work the doctors put in - people think they’re just not coming to see you because they can’t be bothered. I wish everyone could see this video to see there are so many things going on, so many other patients and some are more urgent, if you’re not getting seen immediately that’s a good thing because it means you can wait whereas someone else can’t! I’m amazed by how Siobhan does it all with a smile on her face, I definitely couldn’t stay so upbeat if I was that busy!

I have been following you since your first year of residency. This episode exhibits new confidence, which also shows in your production quality. How do you look so fresh-faced after a sleepless night on call? Maybe you should develop a line of skin care products 😀

Dang, what a night shift! But it‘s amazing to see your well rounded education in action. At least on camera it seems that the jump in responsibility and taking on your new role came quite naturally to you!

Thank you for respecting the family of the dementia patient and their wishes. My father had Alzheimer’s and at the end he had a brain bleed and stroke and they wanted to do all these interventions even though he had clear directives that he did not want that and quality of life was going to be poor even after all these interventions. The original doctors were not understanding of his wishes and were not kind to me, the Health Care POA, while I was trying to honor his wishes. It made a hard decision harder, and was not helpful.

Wow, what a first shift, with two life threatening issues. Reminds me to get my advanced heathcare directive done. I had a STEMI, and a head injury with a small bleed as well, but not on the same day, or year,lol.

You have really inspired me to become a doctor. For a little while I wanted to be a nurse but my mom has convinced me I have the intelligence to be a doctor. It’s weird everytime I tell someone else they seem shocked at my decision. I’m from the U.K. and I’m in my final year of high school so I’m working really hard to get the grades that I need. I just want to thank you in 2020 I was in the hospital after trying to end my life anyway ended up in the ICU and found your videos and they made me feel better on what was happening to me in the hospital I found your videos amazing and helpful seeing what doctors and nurses do to get you well again at first I figured that they were doing things to me for no reason at all but your videos made me see a lot differently thank you for that

You're such a wonderful doctor!!❤️I lost my aunt 4 years ago from SAH😭She experienced headache and ringing in the ear but thought it would go away on its own. Then she was rushed to the A&E. Every second in an emergency matters, but the doctor that spoke to my family was very disrespectful. She told us that the CT machine nearest to the A&E is under maintenance and they would have to rush her to another CT machine but it's at another part of the hospital and she wouldn't make it... My family was begging the doctor to just rush her to the nearest available CT machine... Instead of trying her best to save her patient, she wasted time explaining why my aunt wouldn't make it. It still makes me mad and breaks my heart talking about it💔She was warded in Neuro HDU and on life support for 2 days... I held her hand and prayed with her and tears rolled down her cheeks... She left us forever😭My family was mad at the A&E doctor and wanted to report that doctor. But the neuro head came down and spoke to my family and apologised to us. My aunt was a very nice person so we just let it go because we knew she wouldn't want us reporting the doctor. Sadly no apologies can bring her back to us and we miss her very much everyday😭

So nice to see you working at a hospital I'm very familiar with. I had surgery there in April and it doesn't surprise me that you were in charge of so many patients. Sadly, due to the pandemic, we are severely unstaffed at our hospitals here (and probably everywhere to be honest). You did a great job keeping up with things!! I've been watching you for a while, and I'm looking forward to more videos from you as a full-fledged doctor!

My father passed away in 2019 from complications from a subarachnoid haemorrhage a year after the initial surgery. He never left the hospital and spent most of the rest of his life in the ICU and step down... It was a horrible thing watching him decline over the next 12 months... :(

I was a resident back in the early 80s. One weekend I wound up having to carry three pagers. I had an inordinate fear of either dropping them in the toilet or having my pants keep sliding down due to the weight of the pagers of the day. As you can tell, I am now retired but do enjoy watching young fresh faces and the future of our Medical Care keep up the good work and always find time for each other❤

Hi Dr. Siobhan, It's really amazing to see your night shift on call in the hospital and you showed me hot to give a treatment for the patient with high blood pressure. Siobhan, There are many types of headaches such as headache is coming with high blood pressure, Stroke, Head injury and migraine. Siobhan, I mostly get the headache because of migraine. It really is so important for families to have those tough conversations about their wishes if they were to become incapacitated. As an ER nurse, one of my least favorite things is having to use “extreme measures” for patients that you know will probably never regain consciousness/come off the ventilator. Quite the first night for you, but know they wouldn’t give you that responsibility if they didn’t feel you were 100% capable, which you are! Thanks for sharing!

It’s so interesting seeing the different dynamics in the hospital and how little nurses seem to be able to do autonomously. As a paramedic, I have many standing orders, and if someone was having chest pain, I would do a 12 lead and give ASA right away. Weird seeing that you as a doc have to “order” an ECG before it gets done.

This is a great vlog because it shows something you don't see on all those crazy medical TV shows: that you have to stop and think about the problem (I.e. know the relevant history) before doing anything. That Pt with the hypertension was a great example. If you hadn't see the gravity of their brain bleed before going to see them, mistakes could have been made. Probably got them to have a craniotomy in time. Many times people waiting in ED ask why nothing is happening. There is stuff that is happening in offices and in front of computer screens.

on a whim I started watching the channel when you was a second year of working in the hospital and figuring it out , I totally fall in love with the channel and have not miss a video since!!!

I’m a dispatcher for the ambulance service for a local hospital network. Coming from someone that works on the transport side of the hospital scene, we obviously don’t want transport to take awhile, and it doesn’t always but sometimes we get a lot of emergent calls that have to take priority. I don’t know how y’all do it, but our network owns their own ambulance service. Not all do, I think we’re the only ones in my area as everyone else contracts out to private agencies. Always takes longer when transferring out of network though because it takes awhile for a physician to accept and get a bed at bigger networks. A lot of people don’t know what all goes on behind the scenes in the communication center, and think we have unlimited crews just sitting around doing nothing, which is far from the truth most times. But enough of that… Omg! It’s so weird hearing you day specialist!! I’m so excited you can say that though. I have been waiting for a video as staff and was wondering how your first day went, but now the wondering is over! So exciting! Congratulations! Enjoy it. Can’t wait for more videos!

This was wonderful to see. I am an American where your pay for every step of medical care and everyone has a copay. How do you get paid for your work in Canada especially if you are moving around so much as a locum.
