To leave the prison you will need to a sidequest that isn't associated with the prison map. Here are the steps.
Travel from the Prison to the Island:
1.) Bring up your Quest menu
2.) Select any sidequest you haven't completed.
3.) Exit the menu and pause the game
4.) Select "Load last Checkpoint" from pause menu
Travel to Prison from the Island:
1.) Use "Fast Travel" from the Island
2.) Scroll down to the Prison Category
3.) Select "Prison Canteen"
Restoring your last Prison checkpoint when traveling back:
1.) Bring up your quest menu
2.) Select your main quest (Prison Quest)
3.) Exit the menu and pause the game
4.) Select "Load last checkpoint"
If done correctly, you will respawn in the last checkpoint you reached during that quest.
