Thank you, Dr. Been! I’ve been following you since COVID hit the USA last year. I am a retired nurse and have had DM2 for almost 30 years. Very good material. I've been T1 diabetic for 51 years now. I have never heard an endocrinologist explain the mechanism of either insulin resistance or beta cell destruction for T2 diabetics. This is a critical point as knowledge of the mechanisms at play give us the power to do somethinig about it. A great number of the T2 diabetics I have known were insulin dependent and I submit should now be considered T1. Personally I like using T3 as a designator for the combination whether in a T1 who develops a great deal of insulin resistance or a T2 who destroys their beta cells with the process described here.

I started living KETO/LCHF 3 years ago. 64 years, 173 cm , 93 kg BMI 34, Sober glucose Hb1ac 7.1 mmol/l, blood pressure 170/105 Many chronic inflammations. KETO style max/day carbohydrate 5%, protine 20%, Fat 75%. Results Lost 35 kg (in first 6 months) , reversed DB2, Hb1ac 5.2, blood pressure 127/84 HDL up / LDL up/ Triglycerides Down. HDL/Triglycerides rato perfect. All inflammations disappeared. Daily Ketone level 1.0-1.5 mmol/l. Completely fat adapted. ketones are my major source of energy.

Thanks for the info, doc! So basically diabetics have a continous cytokine storm, no wonder they're at a high risk with covid then. Would fasting help them or at least time restricted feeding?

Thank you Dr. Been! Hoping you might talk about LADA or GAD, sometime soon! After being diagnosed with Graves’ disease in my late 20s and treated with radioablation (wish I hadn’t) now my Dr thinks I may have LADA. Still borderline, but... not many people talk about this condition. I imagine LADA is more common than is actually being picked up. Thank you again for all your teaching, very much appreciated.

It was noticed as early as 1877!! that starvation (similar metabolically to keto) impairs glucose tolerance. Claude Bernard. Dietary fats and low carb was shown to have similar effect the 1920s and 1930s by Sweeney and then H Himsworth and has been confirmed in proceeding decades. I would find it hard to ignore that.

Hello All, one small question if any one can answer here, my mother is taking Zoryl-MV 2 Tablet SR for diabetes but just to save a bit on regular medicine is it fine to take Starvog GM 2 Tablet SR. On the internet I see the salt is same but a different company. Sometimes doctor write a medicine which is profitable for them and also with this covid situation it is tough to reach the doctor for my mother. Thanks for your help.

Hi Dr been.. Thanks for the knowledge.. Have you ever found T2D with glucose range 90-130 but the ketones was high range 5 mmol, the drug only oral. Could you help to educate why, how, and treat decrease ketones in blood? Because so lack source about blood ketones meanwhile the glucose are normal. Prof Tim Spector has done several RU-vid videos on diet. He had his insulin reaction tested to various carbohydrate foods and I believe it was either bread or pasta(it wasn't mentioned whether wholewheat or non wholewheat) which caused a spike in his blood sugar levels. He just doesn't eat the food which causes the spike. Each person is different and one recommendation doesn't fit all. Diets need to be more tailored to each person's individual needs.

I am only in the first 1/4 of the video, and I have heard this about insulin levels. Why is not insulin levels checked, if at least if someone has diabetes in their family? I see A1C more ordered now than just blood glucose in a preventive panel, but never a fasting insulin levels. If it is an insurance thing, it could at least be offered to a patient. Most people do not understand about this time period prior to the diagnosis of diabetes, as if diabetes happens overnight.

Can you do a video or two on calcium oxalate kidney stones. I do have diabetes type 2 , HbA1c. Is 5.4 %. I am now 61. Had my first stone last year, nearly a year later had a second one. X-ray was done, still have. 5 mm kidney stone in my left kidney. Have been drinking extra water and adding lemon Juice. I like to know how to pass this stone, and how to prevent others.

In liver glut 2 transaportar present which are not insulin dependent it means glucose can enter in liver So why liver cells sense starvation like condition and start gluconeogenesis

Great, finally somebody see FFA as causing factor. Did you know that FFA, especially long chain, can be lowered by MCT oil supplementation?

@Geraldine Deblon FFA = free fatty acids, fats in blood intended to immediate burning in cells, if not burned, must be repacked to VLDL. MCT = medium chain triglycerides, saturated fats with 8 or 10 carbons, you can buy it as supplement.

Summary: So in other words high sugary diet results in peripheral fat around the liver, the liver ceases to be able to store sugars, resulting in increasing insulin production in beta cells. Beta cells overwork and release cytokines causing inflammation which activates the macrophages and t cells who tell the beta cells to commit hara-kiri or been eaten. The killed beta cells have to compensated by other beta cells working overtime. Once you lose 85% beta cells you have type 2 diabetes? So how do we reverse insulin resistance and remove peripheral fat? Which foods should we avoid in detail to avoid free fatty acids? What should we replace them with? How do we encourage the creation of new beta cells?

This sounds like a Randle Cycle lecture except that it does not require people to be fat to cause these conditions. Don't mix fats and sugars.....
