Well interesting question and this is how I think it can cause disease through 2 mechanisms: 1. Hirschsprung's enterocolitis: inflammatory cytokines and other mediators affecting the absorptive capacity and permeability of gut. 2. Overflow diarrhea: In severe, untreated Hirschsprung's disease, stool can become impacted and build up in the colon. This can eventually lead to some watery stool leaking around the blockage, which might be mistaken for diarrhea.
