Basal insulin gets secreted in small quantities throughout the day to keep our blood sugar within the normal range all the time. Bolus insulin is released in a big burst from pancreas after we eat carbohydrates and sugars. People who have type-1 diabetes and do not produce any insulin, need both basal and bolus insulin injections to keep within normal range before and after meals. Basal insulin keeps 24 hour sugars normal and bolus insulin covers any carbohydrates eaten during a meal. Type-2 diabetics normally take only basal insulin mornings and evenings. If you start eating low-carb, your sugars will go close to normal with just basal insulin if you are a type-2 and still producing some insulin. However, if you are type-1 and not producing any insulin, you need to ask your doctor to give you bolus insulin to cover your food sugar spike.
