In this compelling episode of Talking with Docs," the esteemed medical professionals turn their focus to the critical issue of osteoporosis, particularly in the context of hip surgery. With unparalleled expertise, the doctors shed light on the increased risk of osteoporosis following hip surgery, a topic of great concern for both patients and healthcare providers. What makes this episode even more captivating is the fact that one of the doctors featured, Dr. Zalzal, is not only a trusted member of the show's team but also a noteworthy contributor to the research paper they discuss. Dr. Zalzal's firsthand involvement in the research adds a unique perspective, making this episode a must-watch for anyone seeking a deeper understanding of osteoporosis and its implications in the context of surgical interventions. "Talking with Docs" continues to deliver insightful healthcare discussions that bridge the gap between medical expertise and patient awareness.

Physically fit, athletic 72yr old who stepped wrong during morning miles with fall.. Broke the ball off right femur, had to wait for OR and delivery of prosthetic. Surgeon came in after surgery and informed me I had osteoporosis, and has me scheduled for periodic injections of Prolia. I walked (shakily it must be said) six hours after surgery. Walked a 5k in 93 minutes six weeks after surgery. Going to do an 8k this weekend.

Thank you for putting this out to the public. My surgeon never mentioned treatment for Osteoporosis. However, My PC prescribed vit. D, I, on my own started taking a supplement called Grow Bone by Garden of Life. Calcium, yes, but also the other nutrients needed for bone growth such as strontium, boron, vanadium, vit K#, vit D, pro biotics, and so forth. PT was started in the nursing home; I had home health OT and PT and I continued at a clinic. Prior to the traumatic fall that broke my hip I was hiking and backpacking. I had completely refurbished and repaired my house (a lot of lifting, carrying, hard physical work. I was 80 years old. I was able to resume hiking, but this past winter sprained my abductor muscles and injured 3 tendons so was again in physical therapy, this time using blood flow restriction exercises which were a miraculous game changer. I am eager to get another Dexa this May. I am fairly certain it will show major improvement in bone density. I did functional therapy at home otherwise known as yard work and landscaping, again active, hard physical work. For whatever reason, (probably the fairly grim stats for folks my age with a broken hip) my surgeon's first words after telling me the hip replacement went well, were "I'm concerned you will be a non-compliant patient and resist therapy" . Please, never make such a negative statement to a surgical, or any other patient, especially not while they still have a consciousness-altering substance in their system. This was a negative hypnotic suggestion for failure. However, I am an over achiever who is goal driven and my therapists gave up trying to rein me in and now brag about my good recovery.

My mom broke her hip on her 77th bday. She received amazing care in her local hospital, Peterborough Regional Health Centre and by her GP. They did everything you talked about. She was back to her active self a few months and 4 years later still going strong. Thanks for a great video.

I love your videos! This one was especially meaningful to me because in December of 2022 I stepped backwards off the 3rd rung of a stepstool thinking that I was still on the first rung and stepping to the floor. As I fell, I did a 180 degree turn midair and then went down hard onto a concrete floor covered in vinyl flooring material fracturing the neck of my left femur. I was 70 years old at the time. Being widowed and living by myself I had to drag myself from one end of the house to the other to try to find my cell phone which was up on a counter. I knocked the phone off the counter with a grabber-getter and called 911. The surgeon(s) performed a hemiarthroplasty procedure the next morning which has done well for my femur and hip however, I ended up with a damaged peroneal nerve from the surgery which has resulted in foot drop. And yes, I watched your video from a couple of years back as to why you shouldn't put up Christmas lights so lesson learned the hard way. Thank you both again for all your great videos!

Great talk. Thank you. My own case is extra weird, since I have osteogenesis imperfecta (mild form). When I broke my hip I couldn’t put weight on it for two full months which means that recovery has been very slow. But I had a first rate surgeon and somehow I survived. I use a walker and a cane and I’m trying to get to 4 miles a day with those. I have excellent physical therapists. Here’s what I would add: Make sure your patient gets emotional care and support. It’s very easy to just give up, or to fall into depression. Remember that not only are you in charge of your own health, but you are more important than you may think.

Great video! My dad and his sister had osteoporosis. Dad broke his hip at age 73, had surgery, but sadly died from pneumonia 5+ weeks later after not being able to recover from the surgery (he had other health issues). His older sister broke her wrist at age 89 which triggered a downward slide in her health. My sisters and I are fully aware of our genetics and do the three things mentioned at the end of this video: vitamin D, diet, and exercise.

Gosh I wish I would have known the delirium piece from my dads surgeon. It was a real problem for my dad who was in his 80's. He even got out of bed, over the rails on his bed, on the first night after surgery! The delirium lasted quite a while, it was very unnerving. Thank you for this video.

Wow what a great episode. We owe alot to Dr. Zalzal for recognizing that my hubby had early onset osteoporosis (47) after a knee fracture. Dr. Khan has been an instrumental in figuring out what was causing it and treating the osteoporosis (vitamin D and Zoledronic Acid infusions).Lots of great info in this video and really reinforces how fortunate we are to have such great healthcare providers close to home. Congratulations on the article and thank you!

Great paper, Great info, Docs. Also, a big Thanks for including the research article. You guys are absolutely correct in the fact that, some people look at the healthcare field as a reaction to your declining health or emergency. When in fact it should be the other way around, the steps we as human beings should be taking, to avoid Hospitals/trauma/injuries.

Plus exercise. Safe home environment. (small rug in the shower, clean house, nothing around my bed, prevention).. and supplements are : K2 Vitamin, B12, B Complex, Zinc, Boron, Strontium, Potassium citrate, Magnesium Glycinate, viatmin A, ... I take my calcium from foods sources but if not I take calcium hydryoxyapate form....Also, daily sunshine, cold shower and breathing exercise Win Hof style... Good Luck and stay healthy!

Thorough! Thank you! 71 y/o male here, zero meds. This thought ran through my mind. -Weight bearing exercises- Yes, you mentioned several times. Direction I am coming from…12 weight bearing exercises every other day…three reps of each. Drudgery to start…absolute drudgery…3 exercise days…I looked forward to going…You change that much. Started with bench press…no weights just 17 lb bar. -12 reps 3 sets. AMAZING difference….I walk taller, fabulous inner feeling…Junk food gone. - Personal thoughts now: #1 take the time #2 do 12 weight bearing… not 3, or 5…12 #3 do 3 sets of each. Thanks for motivating me to offer my experience. Stunned (by results) and thrilled I started. Richard

Hello docs, love your videos. Can you do a video on osteonecrosis generally? I turned 31 two days ago and just had my second hip replaced today following a bilateral osteonecrosis diagnosis earlier this year. Devastating news for someone who is very active at my age, but after lots of research and having the procedures behind me, I’m feeling so much better. Also, your videos have lifted my spirits during recovery and taught me so much. Thanks for all you do!

I'm 62, got my osteoporosis diagnosis at age 61 with a t-score of -4.6 in my spine, mid -3s in hip/femoral head. Broke my wrist 4 years ago after slipping on ice, wanted to protect my back as earlier in the year I'd "done something" to it that I didn't want to repeat. And I didn't repeat it for another two years, but since then, it's been like a cascade of injury, sometimes severe pain, as of this last June, a total of 3 inches of height lost with the accompanying discomfort that "less room" brings, but if my husband hadn't fractured two vertebrae and asked for a DEXA scan, I still wouldn't know I have osteoporosis, because "we don't screen under 65", apparently even after a "fragility fracture". Trust me on this: you want to catch it early. Maybe if you're 80, it's one thing, but if you're still trying to work (I'm a hairdresser--5 minutes of holding my arms out in front of me is too much, after 4 hours, I'm nearly blind with pain,) have been active, you don't want to let it get this far along before you discover it. And yes, I was a runner, (I attempted to run again two weeks ago, 99% sure I fractured T 8, (did not feel it during the run,) but worse, somehow strained my intercostal muscles--maybe because my physiology is different with my height loss? That's absolutely driving me insane. So, I am now a former runner. Three attempts to run, three injuries to my back--that's enough;) I did strength training, maintained healthy weight, not ever skinny, never smoked, drink very little. I thought I was doing what I needed to do. 😂

Hyperparathyroidism causes osteoporosis in otherwise healthy & active people - not uncommon in postmenopausal women. Surgical removal of the over-active parathyroid gland(s) cures the hyperparathyroidism, but osteoporosis treatment continues to present a big challenge! My endocrine surgeon @ UCLA (M.Yeh) recommends NO bone-building meds for osteoporosis caused by hyperparathyroidism & has published. (I hope he's right!) Women! Get your blood checked for high calcium and/or high PTH (parathyroid hormone) - even slight elevations can be an indication... and insist on a DEXA scan! My old white male doctor didn't listen to me (so I eventually left him) and here I sit @ 56 years old, having run & exercised my whole life, with severe osteoporosis. Thanks for the fun vids, docs... gotta go for a walk!

It seems to me that once you’ve broken a hip it’s too late to start popping vitamin D and pounding back the calcium. My doctor gave me a baseline bone mineral density test when I was in my 40’s then her practice was taken over by another doctor. I’m 66 now and the new doctor continues to push me off having another BMD test saying I’m still too young. So I took matters into my own hands and fell off my bike. Landed on my right shoulder and hip but got up, dusted off and rode my bike home. None the worse for wear. The old girl still has some bounce and I didn’t break anything so I guess I’m doing ok!

I fell and broke my "hip" on July 6. It's now Oct. 10, and getting better at using the walker etc. The break was actually the top of the femur, just below the ball, of the hip socket. The surgeon put a rod all the way into the leg, all the way to the knee. Not sure how it's connected at the hip but it's a bear trying to walk 'normally' again. I have been prescribed a series of infusions of reclast. I've had one so far. What do you think of this system?

The nurse had her sit in the waiting room for a couple of hours until an er bed was available. After the x-ray, the nurse came back feeling so bad. She said she couldn’t believe my sister had broken her ball off her hip. The surgeon was excited because she hadn’t seen a break like hers. They pinned the ball where it was sitting and now she is 100%. My question after a long ramble, is calcium something she should be taking on a regular basis?

Thank you, Sirs. Another opportunity to worry about the potential onset of something horrible. 😬 So generous. I am going to take preventative tips and move on - and am hoping watching the videos doesn't jinx me into having the issues! I see Dr Zalzal has saved up all his tokens and got a brand new top. Dr Weening forgot his and wrangled one off a poor trainee whose mum hadn't brought the iron over yet to his new digs, or sewn his name on. 😊

My bones are gaining calcium after 3 years on hrt post menopause. If you’re not on hrt then you need to be on an estrogen acting drug like fosamax. It’s horrible to be watching my mom and aunts crumble due to osteoporosis. They are all very sharp brain wise but their bones are crumbling. Please look into hrt perimenopause to prevent this horrible condition. Men who have osteoporosis should be tested for celiac disease. To reduce urinary tract infection post menopause use vaginal estrogen it is very safe for aged, improves vaginal and bladder integrity, without raising blood levels of estrogen .

Plus you’re always sleeping because she pee all the time Good is drinking liquids think the only time I had a slow night I didn’t have to get up every hour and a half. I think I would drink milk in the evening. I can’t figure out why but I sure liked it because I didn’t get it 20 times. Think I got up three times. I’m so tired after three weeks.
