Join me for another day in the life of a doctor... but this time I'm shadowing a hospital pharmacist, Dr. Michelle Lui! Learn what hospital pharmacists do, how they play a vital role in patient care, interact with the medical team and allied health professionals, and find creative solutions to provide the best patient care. I can honestly say that a pharmacist saved my life by catching a drug allergy my doctor over looked. They took the time to explain to me what was going on within my body so that I understood the very real danger I had been in. Pharmacists deserve a ton of respect.

I think it would be really cool if you could shadow a pathologist, and highlight the job that clinical laboratory scientists do. The lab is often an "out if sight, out of mind" part of the healthcare team.

I was going to suggest this too! Each department has unique opportunities that can be highlighted. But I have to say Clinical Microbiology is the best but I'm biased. 😜

Lab and pharmacy are right next to each other in our hospital and that is fitting because like you said, both of our careers seem to be “out of sight, out of mind”. I appreciate the job that the lab staff does on a daily basis. Vanc troughs keep our hospital going. Haha.

@@biancadesousa Your anecdotal experience doesn’t speak for all pharmacists, not to mention you didn’t provide any context at all. At any given time there are thousands of malpractice suits ongoing involving mostly doctors. As a pharmacist myself, I sometimes catch dozens (yes, dozens!) of errors each shift that don’t make it to the patient because of my expertise and intervention. And guess what, the patient never even knows. The physicians and nurses get all the credit in healthcare. But God forbid a pharmacist makes an error, and the world comes crashing down. Nobody is perfect. Your personal experience doesn’t speak for everyone. Get a grip.

As a pharmacy student, I'm so excited to see this video and to get a view of what pharmacists do inside a hospital. Unfortunately, so many people have misinformed ideas of pharmacists as only pill counters who are soon to be replaced by automation, so I especially loved the part about how the addition of automation doesn't reduce a pharmacist or pharmacy intern/technician's role in healthcare. It's great to have videos exploring some of the other roles a pharmacist plays in helping a hospital run smoothly.

I'm a tech and I do have to say I've have many conversations with fellow techs and our pharmacists about there eventually will be no need for techs as the machines will count and patient will get it delivered. Which means we lose our jobs. I wouldn't worry too much about the pharmacist side as I'm sure there's still always going to be a final check by one but it puts us techs in fear of being replaced by robots.

I'm a hospital pharmacist who is also and onc/haem specialist. Great video! Thanks for showing what pharmacists do apart from dispensing medications. Doctors are disease specialists and we are medication specialists. Together we make an awesome team.

@@ViolinMD you are the same "doctor" that was lying to the public about covid just so you kept getting a NICE NICE PAYCHECK, so i spit on you and nurses like you that only think about the $$$ and u will say " no no i am helping people" ooooooooh really. ok do this help a person with NO medical insurance and see if u still have a job being a doctor you WONT. the only time a "hospital will give out free" medical care, is when it is a natural disaster and they will ONLY do it then because they will get paid back in the "clean up" bill.

You should definitely make this a series where you shadow different jobs in the hospital. Even the ancillary and non-clinical staff like security and housekeeping could be cool to see. There are so many people with so many skills needed to keep a hospital running.

Yes! I had baby back in May of 2021 and housekeeping came in and tidy up the room and I thanked them numerous times bc I couldn't get outta bed due to a csection. They have an important role as well as the Drs and Nurses there 🤎.

As a hospital pharmacist, this video was awesome to see. I also want to add that, depending on the hospital, pharmacists are given a level of autonomy to manage certain medications and monitor drug levels. For example, my hospital has protocols allowing pharmacists to manage medications like antibiotics, anticoagulants, anti-epileptics, and TPNs. This means that if a patient is on any of these medications, a pharmacist will be monitoring the patient, adjusting the dose for these meds, and ordering any relevant labs. We also have protocols that allow us to adjust dosing of meds based on the patients renal function and convert medications from IV to oral. Lastly, every single medication ordered by a provider is reviewed and verified by a pharmacist before it can be dispensed and given to a patient. Just wanted to share a bit more of what pharmacists do in the hospital for anyone curious. Thanks for the video!

As a retail pharmacist, I really enjoyed this video and getting to see what a day in the life of a clinical pharmacist looks like. I would love to see you do a video shadowing a retail pharmacist in the community setting as well! I feel like the majority of people do not realize just how busy retail pharmacy is and all the intricacies that we deal with on a daily basis. The big box pharmacies have gotten insanely busy and the pandemic has just magnified the problem.

@@cheesobogo3199 it’s hard to explain all of it because there is a lot of stuff that goes on behind the scenes (insurance billing, calling md’s to request/fix prescriptions, making adherence calls to patients, calling insurance companies through 1-800 numbers and waiting on hold, giving vaccinations, etc). Most of the time this is done with 2 or 3 technicians. One technician is usually at the drive thru full time, another is up front ringing out customers, and the last one answers the phone and fills prescriptions. An average cvs/Walgreens in my area does 600 prescriptions a day and some are pushing over 1,000 per day. In a 12 hour shift you would have to check at least 50 prescriptions per hour to keep up with the 600 per day. Each prescription is checked twice (once after it is typed to make sure everything was typed correctly from the written prescription. This is also when we look for drug interactions and dosing concerns. The 2nd time we check it is to make sure the correct pills are in the bottle after it is filled.) This means you are doing 100 “checks” per hour and that does not account for time to do all of the other stuff that is required. A lot of the time pharmacists get pulled away from checking prescriptions to help in workflow (typing prescriptions, ringing out customers, filling the prescriptions, answering the phone, putting up the drug order, putting in the drug order for the next day, calling insurances or doctors, etc.) My best advice would be to work in a pharmacy before you go to school for it and if you decide to do it I would go work in the hospital. I did 8 years of retail and finally transitioned to the hospital about a year ago. The life of a pharmacist in the hospital is way better and it is not even close. I hated RETAIL so much I almost quit pharmacy to get my real estate license.

​@@cheesobogo3199 it's been a year, but imma reply in case anyone else comes across it. Honestly, I highly discourage going into retail pharmacy, especially at this stage where corporate has learned to maximized profits while minimizing labor. I used to work at a moderately busy "3 letter" pharmacy. We're also next to a pretty big university, so we fill around 500 prescriptions per day during the school year and 400 during the summer. As a pharmacist you need to verify prescriptions, making sure the techs typed it up correctly and count the correct drug. You answer phone calls from doctor's office of they decided to call in the prescription instead of electronic, you also have to talk phone calls whenever someone(doctors/nurses/patients) have a clinical questions about medications. You also have to counsel on certain prescriptions, and recommend OTC drugs, you'll get a lot of "What do you recommend for this rash I got here?" You need to call the doctor's office if there's something wrong with the prescription that can be life-threatening error to the patient, but sometimes it's a weekend and the doctor doesn't answer Monday. And now you have a angry patient that demands their meds even after explaining we can't release it for their safety. You have to leave every 10minutes to do vaccines when it's flu season (typically September-February). You're also expected to do pick-up and drive-thru if the techs can't catch up. With all of these, corporate only allow our store to have max 1 pharmacist and 1 tech (some times 2) at a time. But they still expect us to complete everything in 15minutes(from data entry to filling to verify, while having to do vaccines) or else our store KPI goes down which reflects poorly on your yearly salary increase. Not only that, the 3 letter pharmacy is notorious for buying out AND closing down other pharmacies in the same area, so instead of a town of 5,000 people going to 4 different pharmacies, all 5,000 is now forced to go to the only pharmacy that is left while not increasing staffing or pay.

@@cheesobogo3199 in retail, you're adjudicating one patient event every 5-15 seconds. Every 5-15 seconds you're verifying an order, answering the phone, wringing someone up at the register, listening to someone's symptoms and telling them which OTC med to get, figuring out an alternative to a med not covered by an insurance, making a call on whether or not a doctor is prescribing opioids appropriately or not, etc. No one but other pharmacists and techs will ever understand how busy you are. If you try to explain it to someone, they either won't believe it, or they will just chalk it up to being consistent with everyone naturally complaining about their own jobs. You are chronically understaffed, and patients will take it out on you. Not every patient, but enough to give you chronic stress both at work and when not at work. Most patients and even some doctors don't know the Federal and State pharmacy laws, so you will be in constant conflict with people that want you to do something for their convenience, but which will put your license to practice at risk, and also which could put you in a position to be civilly liable, i.e. lose all the money you made working as a pharmacist. I would consider a career with a higher quality of life. Learn how to code with assistance by AI and work for Microsoft. Be an engineer at Lockeed Martin. Something with a better quality of life.

@@jupitersspell not really. Some people just get unmotivated. Especially when you see all the work you have to do to get there, and it’s hard to see when and how the good part will come. And they said it’s their dream job so I think they will do great!

@@kaitlin6184 No, I mean the career has negative outlook. It's projected at -3% for 2019-2029, meaning there are ZERO new jobs and only terminations. The only career with that low of outlook is interior design. We simply don't need more pharmacist, its worse than doing art school.

I wish people would give the same level of respect to pharmacist in the UK. Sometimes I’d rather see my pharmacist than my GP. My pharmacist also caught a bad mistake made by my GP, That would have made me sick. Let’s not forget these professionals ARE medically trained and been to university/medical school after all,

So exciting! As an RN, I’ve started dispersing tablets in water inside a syringe (when dispersion is an approved method of administration) for my delirium and dementia patients, but I’ve NEVER seen anyone else do it this way! Nice to see it backed up by a pharmacist.

Pharmacists work in shadow but they are front line to me! I was diagnosed with AML 4 years ago, went through all the process until stem cell transplant, got better then relapsed, had more chemio until the team pulled me out of treatment to go palliative. I have the same oncology pharmacist since day on and he still work on my case with my palliative care team. He is creative, he always has another plan if something do ne work out for the best. His knowlege is incalculable, he is up to date on every clinical trial that could suites me. Im so grateful to have him in my team. He his a true lifeline to me. Thanks to all of the health care worker. You a valuable and precious.

I am a pharmacy student about to graduate next March, and I LOVED that you shadowed a pharmacist in the hospital. Pharmacists play a such an important role (we do way more than just dispense drugs lol) in the inpatient setting but are often forgotten about or misunderstood. It’s awesome to see healthcare workers learn to understand each other’s roles; this leads to better patient care! Thanks again for this video 😊.

@ViolinMD I would like to say thank you for making this type of video. I am a chronic A-Fib/SVT patient and always wondered why it takes my doctor and nurse time to pull meds and they say they need the pharmacy to approve it first to get my medications ready for me. I got cardioverted in the ER and was shocked at 200, then 300 Jewels and they had to knock me out with 300mgs of depervan / propofol and the ER doctor was only going to give me 100mgs of the drug and the pharmacist knew me and recalled what, I am always given and was given the proper dose because of the Hospital Pharmacist. This video has helped me understand how things work and would just like to say thank you my friend. Thanks, Anthony C.

Inpatient and clinical pharmacy is so fascinating! Outpatient pharmacists are obviously always dealing with something new- but the full on collaboration with internal medicine that the clinical and in patient is incredible to see happen. The type of problem solving and creativity with situations like "cant crush the tablet- etc." and needing to figure out not only the medication but delivery is so cool!

Though this is a year old. I love that you were able to show behind the scene. I don't think Pharmacists get enough recognition of how much they do to become a pharmacist. They have so much knowledge, and working with Doctors, they create an all important symbiotic relationship that can benefit everyone. I also love the energy of Michelle Lui!

Dang, you expect the books and research to have the answers from being in school , but realize you’re at the top and have to make these kind of decisions. Certainly fascinating.

Wow! This was amazing! Just seeing the narcotics room reminds me of a few years back after my last surgery. I was worried when they said that I had the option to be administered Fentanyl for pain. Though luckily, it was only a very small dose while I was in the hospital. I know you must hear this all the time, but I still can’t believe that you’re a 5th year resident now, I remember the days of hearing “I’m a 1st year resident”. You are so inspiring!!

I am glad you shadowed a pharmacist. Not everyone including the public knows the behind the scenes that goes on in a hospital pharmacy. I am also glad pharmacists and pharmacy technicians got mentioned in this video. I work as a pharmacy technician in a hospital and I appreciate this video. It also leads to other healthcare workers understand pharmacy more which leads to better patient care.

This summer, when you're fully licensed, a summary of the entire medicine education "journey" (I don't like that word, but I didn't find a better one) would be cool. And maybe you and Mark could reflect on the couples' side of it, I think (know) that people are interested in what it's like to be two doctors in a relationship during education and then "just" going to work and going home.

I'm in an accelerated BsN now, but I worked as a pharmacy technician for the past year so it was so cool to see the team working together in a hospital setting, it's definitely a different beast from working in a community pharmacy! Thank you for making these videos!

As a pharmacy student, I see first hand at how underappreciated pharmacists are. Alot of people underestimate our knowledge and don’t understand that we learn anatomy and physiology as much as we learn pharmacology.THANK YOU for this video highlighting the importance of pharmacists on an internal med rounding team.

Not sure if you plan on shadowing a bunch of roles but I love this series! Our 1st year Family/IM residents are required right off the bat to shadow the front desk registration for 2 weeks, then 4 weeks shadowing clinic nurses before they see patients. Great groundwork for being a provider!

I follow a few doctors here on RU-vid and one on Nebula, but I really appreciate how she specifically does these videos where she shadows certain healthcare professionals (ie, nurses) in order to find out their role in the hospital setting (seriously summarizing when I say that!). I love it! Very informative!

Back in 1971,I was a pharmacist yup! I was 16 yrs old and working counting pills and filling bottles,I also washed bed mats,needles that you could look through, yea I know right! and the glass container that held the needles. Gosh! I did so many different things,I can't even remember everything. But it was wow! 50 yrs ago. Wouldn't get away with letting an unqualified teenager do that now,I hope. Things have changed a lot from 50 yrs ago.

One thing I learned while working in the medical field is take advantage of the pharmacists knowledge. I know when I couldn’t get into see my PCP I stopped by the pharmacy and asked the pharmacist what I could do for an issue I was having until I could be seen by my doctor. Pharmacists have so much knowledge.

Thank you so much for this video! I am a pharmacist in Germany and many people here don't see the benefit of having a pharmacist taking care of your medication. They believe in doctors and doctors only, not knowing that some of them regularly call me to talk about medication, relying on my expertise. Pharmacy is a vital part of healthcare, we're not just there to distribute. Thanks so much ❤️❤️❤️❤️

I’m an inpatient pharmacy technician in one of the hospital in Oregon. We the pharmacy technicians work so hard to make sure our patients get their medications on time but usually our hard work never been recognize as other health care worker because we work behind the scene not giving direct care to the patients.

Tip, after you put on the cap on the syringe and pull back the plunger to create a vacuum, most pills and capsules will dissolve even faster due to the pressure the vacuum is creating 🙂 saves me so much time needing most of my meds through my feeding tube.

As a nurse, the only two places I personally have close interactions with the pharmacists are rural hospital (one floor for all) and ICU on-site pharmacist. I just feel that I never actually see the pharmacists while on the floor though, mostly just the pharmacy techs. But this video really shows that how they do their work behind us floor nurses and how they also contribute to patient-centered care. Thanks pharmacists!

It’s so interesting to see a hospital setting. I work as a retail certified pharmacy technician and we do just about the same things. Not only do we count and sell medications, but I’m discussing therapies with my pharmacists, educating patients on drugs, and I do compounding as well. There’s a lot more too that we do. It’s definitely a different side to health care that not a lot of people really think about. Thanks for posting this!

I found that when I went from retail to inpatient, it’s actually really different. There’s a lot that I do at my hospital that this didn’t show, like sterile compounding for IVs, refilling med stations, delivering meds, etc. I think that’s because my hospital has different technology and I’m also in the US so that changes some things. But I was definitely surprised when I felt like I didn’t know what I was doing when I went from retail to inpatient.

I’ve had this comment from our elderly patients quite a bit “just count the (expletive) pills, that’s all you have to do, what’s taking so long”. People don’t understand how much goes into that one medication, tech typing rx, rph reviewing the script, most time consumingly- having to deal with thoseDREADED INSURANCE REJECTIONS, and finally rph checking the med against the script. I will mention to be careful Rachel because in my state, there’s a really fine line in helping the patient vs counseling the patient which is quote “beyond the techs duties”. They are real strict about pharmacist being the only to counsel in my state.

i started working as a hospital pharmacy tech in June and it's so interesting to see how other hospitals' systems. we have a robot that dispenses medications that patients (who are staying for multiple days) have to have every day and we deliver them to the rooms. however, our robot contract and warranty is expiring soon and we don't really have the space for a new one so my typical tasks are going to change when we get rid of it, which is both exciting and terrifying. thank you for including the pharmacy techs in the video!! i love my job and has made me think about going into pharmacy rather than medicine.

Being able to make ivs at my hospital makes me feel like I’m actually truly helping people as opposed to my retail pharmacy job. I work overnight with one pharmacist, it’s just us for a 125 bed hospital that due to COVID recently is pretty much at full capacity all the time now.

@@matthewthompson4611 I agree that making ivs makes it feel like I’m helping. I used to be an overnight pharmacy tech at a children’s hospital and it was just me and the pharmacist. I actually really enjoyed being on nights but man it’s hard on your body after awhile and takes a toll on your social life for sure. I hope things calm down for you guys soon!

@@dianafox2018 the hardest part for me is when we fight so hard making so many drips for our patients and then God has another plan for those patients. I imagine it would have been even harder at a pediatric hospital. 😢 I’ve been working nights for about 7 years now. Used to be 7on,7off 10 hour shifts. By that 6th day you’d about had it with everybody. Now we work a 5,2,2,5. Makes it a little more manageable. 5 days are not quite as bad. Are you still working yourself?

@@matthewthompson4611 Yes, it's really emotionally hard to work with pediatric and neonatal patients. But when you see a baby that has been in the NICU for over a year finally get to go home, that makes it worth it. It's really hard when it doesn't end up that way. The Pediatric ICU, though, is really, really rough. I've been in research for a few months but I'm getting ready to transfer back to the women's and children's hospital that I worked nights at. We do 7 on 7 off on nights and I actually really liked it. So much so that I'm going to be doing that on days now.

6:41 I remember using the nematic tubing as a nursing student and was amazed by it. I’m still amazed by it. Might be old technology from what she’s saying, but I think the tubing system is great. As for carbon copies of messy doctors’ hand writing, ya, you need an upgrade. Haven’t seen carbon copies used for orders before & doesn’t seem like a great idea. The robotic drug dispensing machine (aka Omni cell) is great, although they’re often slow and nurses have to line up to get their patients’ meds.

I loved this video. I’m a senior hospital pharmacy technician in the U.K. currently specialising in paediatrics (Evelina children’s hospital London) Whilst and after qualifying I have had the opportunity to specialise in manufacturing parenteral nutrition, chemotherapy and clinical trials, sterile and non-sterile products, Quality assurance, dermatology pharmacy, psychiatric pharmacy and now paediatrics. I take drug histories, supply medicines on demand and take pharmaceutical care of whatever is needed on the wards. I love love love my job.

It's such a diverse job and people often assume that we just massage (never do that in a hospital), not realising that we work in icu, oncology and a whole lot more.
