As someone who is disabled and lives near cities trying to get rid of cars, we need to make sure that those with mobility/nervous/sensory/pain disabilities don't get left behind. If we can't drive and park ourselves close to where we need to go, then we need to make sure that there are public services (busses, easily accessible rideshares, wheelchair-friendly roads/sidewalks, etc) that are convenient and available at all times. 😊

@SlavaZone My city has an extensive Dial A Ride system that assists mobility impaired, elderly and people with other challenges to move about town. We can focus on solving causes of obesity rather than just slapping a medication bandaid on it. Causes such as: poverty (they wouldn't be able to afford the medication anyway), restricted access to fresh food, poor fda/USDA regulations, literally so many other things cause this and if solved would help SO many people, not just those considered obese. Fixing the root of an issue like this helps GENERATIONS. Edit: If the country had proper ways to address MENTAL HEALTH that would also be a huge contributor to decreasing obesity. Things like eating disorders, depression, and more could be addressed while preventing health complications down the line

I work as a physical therapist. Every person should FIRST be referred for a consultation for a dietician and physical therapy to begin their weight loss journey. Research shows better weight loss and health outcomes when focusing on diet changes. It shows even better results when pairing this with physical activity. Nobody here is getting the point. We already teach kids about drugs, about smoking, about drinking, about smoking weed. And yet so many teens and young adults struggle with those addictions. There's no rationality in an addiction. It's a purely emotional response to an outside or internal stressor. Maybe even one caused by the addiction itself, i.e withdrawal. No amount of education is going to get people to take better care of themselves. We need better mental health support services, a better system to help those who are struggling and find out what it is they're trying to escape from. Once we find out what those stressors are, we should work on trying to eliminate those on a societal level as best as we can (poverty, bullying, undiagnosed mental disorders, stigma, social issues such as racism and homophobia, etc.)

These people who are taking Ozempic usually are the same ones who regain their weight after their bypass surgeries. I've witnessed it multiple times myself. I don't believe that it's uncontrollable to the point of requiring medication (in general). If it's controllable through the individual, then medication is unnecessary and shouldn't be used. If it's an uncontrollable medical issue that can't be controlled by the individual, then why is our obesity pandemic so historically sudden? It's always been people with virtually unlimited amounts of food, such as kings and wealthy Romans, who would vomit in between courses so they could keep eating. In the modern day, most people essentially eat like kings.

Lol. Ozempic makes you stop being hungry. That's as close to a root cause as there is. Overeating isn't primarily a psychological problem that you can fix, as though it were a broken bone. People have varying degrees of satiation, and food has been getting less satisfying and more calorie dense over time. Now we've passed a threshold where most people's natural inclinations no longer keep them at a normal weight. Three options: Intense societal pressure (i.e. Japan) Serious regulation of food (nobody has done this) Medication

Regaining the weight after stopping treatment is the most common outcome. Telling patients to “lose weight” is not helpful. Understand their symptoms and treat what they came for. @@Annika4470 Saturated fats and high cholesterol foods can affect your heart health but they are not the primary cause of obesity. That’s sugar. Anything that breaks down into glucose including complex carbohydrates contributes, but because complex carbs usually come with filling fiber they are not the primary cause. It is almost exclusively sugars. What I mean by sugars is, yes processed cane sugar, but first and foremost it’s high fructose corn syrup. Which is derived from corn supported by corn subsides. It’s 100% glucose so it goes directly into your body needing no digestion, and in 2009 it made up 10.2% of total daily calories for people in the US. I can only imagine that has increased in the years since. It’s in nearly every cracker, bread, soda, juice, jelly, salad dressing, etc. Certainly in everything sweet and often even in things you wouldn’t consider sweet. This adds completely empty calories to the staple foods people, especially low income people, rely on for survival. If everyone could avoid 10% of their calorie intake imagine what that would do for obesity levels. Maybe take them back to pre1980 levels? I say that year for a specific reason. This isn’t some cultural shift or miseducation about portion sizes. This was a literal government plan started in the 1930s and worsened by Reagan in the 1980s. By subsidizing corn, a crop that if grown GMO and with chemical fertilizers would grow EXTREMELY consistently from year to year, they would create such a consistent food source that food shortages would stop completely. It would make their seat of power stable, bread and circuses style. They managed to stabilize supply so that we would always have an excess. The stability they created allowed for complex food systems, like many processed foods require, and the natural processes of the system meant that they found novel uses for the corn excess, ie shoving 10% more calories into everything we eat and feeding it to all of our livestock, many of which can barely survive on corn. Most other “processed foods” are also corn derivatives. Not just fructose, but also maltose, dextrose, and most of those ingredients you can’t pronounce, all are hidden forms of corn. These uses only exist because corn is cheap and corn is only cheap because it’s subsidized. Stopping corn subsidies would instantly alter our entire food system. Likely putting an instant dent in obesity but also sacrificing the stability which is the foundation of our entire food system. I’m ready for it. Corn is poison pretending to be food. But for many low income Americans dependent on the cheap sh*tty ingredients that come from corn, that policy change would lead to starvation. In food deserts across the US fast food and corn-based nonperishables are all they have access to and can afford. Stop blaming “poor education” and individuals when these are clearly choices made by the government. Everyone knows they shouldn’t be eating sugar. This is not an education issue. It’s an issue of access and affordability because of lazy failures to control supply and demand.

I think the nutrition curriculum in schools should be updated and Physical education should be performance-based. Not the old standards which I believe are unnecessarily challenging. I think Americans should understand what they're putting in their body and have a better understanding of what they can do with their body. I spent one or two years in the gym and studying nutrition and that experience has greatly improved my quality of life forever. I have family members who are obese and don't understand what they are capable of because they don't know how carb-heavy diets impact you. They don't understand how beneficial increasing protein intake can be regarding satiety, metabolism, and the recovery increasing protein can bring in. Yes, being heavy can make your knees and ankles sore, but if many Americans ate more protein, they might not wake up the next day still sore. It's not just about changing your whole body but understanding that some simple changes can impact things that many consider a permanent fact of their life. (I was obese with knee pain and sore ankles) I am still obese by definition but I can run faster, longer, jump higher, lift more, and fight better than the average American. I was 250lbs at 5'10 with like 45% bf. I cut down to 210 ~25-30% and went back up to 225 with the same bf%. I love how I feel and my life has been greatly improved.

Shouldn't we fix our portion sizes in the first place to avoid such problems? Or am I getting it wrong? Right now I see celebs using this medication for weight loss and its just weird like why not with natural methods then use a medicine which might have some impacts if you're not obese?
@danaolsongaming
@danaolsongaming 11 месяцев назад
Because not everyone can do it through natural methods unless they're forced to. Call it laziness or whatever you want, but being able to instantly "cure" one of the largest burdens on our Healthcare system without forcing people to significantly change their lifestyles is probably the best solution we have to fix this issue. Food education hasn't worked, restricting portion sizes hasn't worked, shaming people hasn't worked. At some point we have to accept this is the world we have and go from there.
@Lau3464l
@Lau3464l 11 месяцев назад
It’s at the point where normal sized people are most likely already eating very little in their attempts to lose weight and reducing portions even further would be risky or trigger a binge-and-restrict cycle. Reducing portions isn’t sustainable for most people. Everyone who has ever tried to lose weight will have done the “natural methods” already, no one just wakes up one day and immediately decides they want to lose weight and jumps directly into medication.

The problem isn’t just the foods we eat, but how much. If you have all your balanced meals in a day and have two snacks, let’s say one is ice cream and the other is a bag of chips, then you will be ingesting so much extra sugar and sodium than you need to be. For Americans (I don’t know about other countries) most people put their entire sugar intake into their coffee in the morning, and a lot of folks drink a couple cups throughout the day! There is something to be said about companies that make horrible snacks and foods, but if the consumer stops buying them, the company crumbles. There is a level of personal education and responsibility we have to take for our bodies. Especially since we live in a time where it is easier to out source all of our meals because we’re too busy to cook them ourselves. My ex boss used to say, “the stuff we sell is for indulgence purposes. As long as it tastes good we can sell it for what its set price is.” I can’t tell you how many customers I would see everyday comes in for sandwiches and soups assuming it’s healthy because there were vegetables in them.

My only critique is that she says that people "have obesity" instead of "are obese". Most obese people do not have an underlying condition of which obesity is a symptom or side effect. We just enjoy our food, sometimes to excess. These is a difficult but important distinction between someone who cannot exercise self-control and someone who doesn't want to. Medication should be used to treat conditions where the patient is unable to make their own changes. If I could just choose to not have T1D, then my dependency on insulin would be self-imposed and I wouldn't expect my insurance to cover it. For most people, obesity is a choice, or at least the result of choices. It might not have been the goal, but it's not a great cosmic secret. Instead of medication, I might suggest therapy. A good therapist will be able to help identify any psychological causes, such as depression, anxiety, and addiction (to make but a few) that may underlie an eating disorder. Otherwise, the therapist can help a patient find ways to stay on track with health planning. Some people respond well to having a life coach. Others might prefer to have a team of nutritionists, trainers, and other sort structures for success. Yet others might need to learn positive self-talk, or try hypnosis, or some other alternative treatment. Only when these other treatment options have been exhausted or dismissed, should we start pumping obese people full of drugs..

I dont disagree. Our focus should be on creating healthy relationships with food and exercise, but some people are too heavy to start without a little help. I know I for one have gained weight from being less active due to chronic pain, and if I didn't have the extra weight it'd be much easier for me to start exercising again and result in less pressure on my joints and therefore less pain and thus more exercise. Some of us need the extra kick in the pants to get the ball rolling. Others are just looking for easy ways to do hard work.

I disagree. Obese patient perspective. I have a food addiction and an eating disorder. I was prescribed medication to help control my appetite and lose weight. I developed an allergy to said medication (localized hives) and had to stop taking it. When I stopped, my appetite came back in full force and even worse because I was essentially starving myself before. Instead of drugs, we should be teaching people how to listen to their bodies. Most people I know are in a restriction/binge cycle like I was. We are conditioned to think carbs, sugar and fats are the enemy. When in reality, we do need these things. Dr. Mike, can you interview Emily Vasquez? She is a registered nutritionist who teaches intuitive eating. She has helped me incredibly as a woman who was taught men's diet rules. I still have a long way to go, but I'm now binge free for 30 days when I used to binge almost nightly. Drugs only work while you take them. After you stop, the problem can become even worse.
@dr.rothotnik
@dr.rothotnik 9 месяцев назад
HOLY BASED- In all seriousness though, intuitive eating really is insanely underrated. It doesn’t help how there’s so much stigma around it, thanks to diet culture and the rampant fatphobia that plagues our society on the minute. These medications are literally just glorified appetite suppressants, which you can literally get the same effects with a plain cup of coffee daily, although of course it’s only temporary because as you experienced, they basically condition your body to starve itself. I know it all too well. :/ As a former person of size who also struggled with eating disorders and food addiction, IE helped me get to a healthy state better than anything I’ve ever tried, and it also helped me to restore my own relationship with food. I’d like to see an interview too tbh.

I come from a childhood where abuse was common, forced to eat what "they" thought we should, never leave food on the plate else risk a smacking, lots of carb rich, sugar and salt rich foods. That carries over. Then working as an adult in a fast paced IT job where sitting for long periods is common, and grabbing a burger is all you have time for...complicates things a lot. I am working from home now, so my diet is controllable, and I am down 13 lbs since going back onto my keto diet a month ago. I was 230lb before covid...so it is def. time I turn things around...but I will not be taking a medication for life...if I can avoid it. Especially with the side-effects and problems surrounding it.
