Thank you so much for this interview! re: no drugs that slow progression Inosine was found to slow progression in women with PD (but not men): pubmed.ncbi.nlm.nih.gov/31484712/ " Women in the higher inosine dosing group showed a 7.0 Unified Parkinson's Disease Rating Scale (UPDRS) points/year lower rate of decline vs placebo ( = 0.01). In women, slower rates of UPDRS change were associated with greater increases in serum urate ( = -0.52; = 0.001), and with greater increases in plasma antioxidant capacity ( = -0.44; = 0.006). No significant associations were observed in men. " Given that glycine may lower uric acid [PMID: 30845731], should products high in glycine (e.g., collagen supplements) be avoided by people with PD? ~ Also, it is my understanding that dopamine replacement therapy extends life of PD patients; if it does not slow progression, then does it extend life expectancy by extending the latter stage of disease (perhaps by lessening autonomic symptoms, like swallowing problems)?

Recently some studies have implicated DHA in production of toxic alpha synuclein aggregates: www.ncbi.nlm.nih.gov/pmc/articles/PMC6099649/ pubmed.ncbi.nlm.nih.gov/23077527/ "Specifically, a diet enriched in DHA increased the accumulation of soluble and insoluble neuronal α-Syn. In accord with cytotoxic α-Syn accumulations, we detected evidence for enhanced neuritic injury and astrocytosis. *Conversely, α-Syn deleterious effects were significantly attenuated by a low-DHA (and low n-3 PUFA) diet.* " It is possible that while DHA can prevent initiation of the disease process, under conditions of high levels of a-syn accumulation, DHA may exacerbate progression. The Mischley study is interesting, but it is an observational study.

I love how you manage to keep things so clear, yet managing to tease out a detailed understanding of the field of study of these scientists! It's of course also thanks to having great guests, but excellent questions, and engagement in conversation, and the additional references really helps! To be deprived of a thing certainly makes one more apppreciative! And not only that, but managing to inspire to action through science! I'll self-experiment with more brain-body type exercise. I experimented with piracetam some years ago, and I improved my brain-body connection a lot! I never practiced sports, and was never really connected fully to my body. So that was a very pleasant experience. I will try and see if it can be achieved simply through exercise.

Most doctors speaking on RU-vid do not take into account the ignorance of the layperson audience, causing me personally to pause and open a second window to google the many acronyms and medical terms flying around. Michael Okun has a great video where he talks about the many symptoms of Parkinson's not just the tremors. For example, the early markers mentioned like constipation, and problems with normal involuntary functions (respiration, swallowing, etc), as well as the loss of smell.. which is often said to also be a deficiency in iron, Iodine, copper. Any thoughts on how Parkinson’s patients typically have very low uric acid levels, and that uric acid acts as an antioxidant/metal chelator , but if levels are too high it is pro-oxidant? I've read that SGLT2 inhibitors / allopurinol may help lower high blood pressure and bring uric acid down to below 5mg/dL Or thoughts on fat-soluble statins (lipitor/atorvastatin) and their correlation with a decreased risk of Parkinsons? Or why Nicotine seems to protect against getting Parkinson's?! BTW, there's a great Art of Manliness podcast (#562) that suggests the type of fitness exercises used for boxing (twisting, hand-eye coordination, etc) are the exact type needed to help with Parkinson's. Regarding the 19m mark, how the temperature at which the heroin was synthesized/tainted caused immediate parkinsons by affecting mptp... it sounds similar to guillain-barré where a viral infection administered during a flu-shot, may cause an autoimmune disorder that attacks nerve cells and leads to paralysis. BTW, some suggest that many common brain diseases may be viral/auto-immune in nature. For example, women seem to accumulate plaques in the brain earlier than men.. and in general suffer from more auto-immune diseases than men. On the plus side, however, an over-active immune system may be the reason why women seem more protected against cancer and infectious diseases compared to men. Braak's theory says that parkinson's originates as a mis-folded virus/bacteria/prion/alpha-synuclein that travels from the gut to the brain, and that just severing the Vagus Nerve (truncal vagotomy) appears to stop the progression of Parkinsons! Alternatively, just vestibular/vagus nerve stimulation may alleviate symptoms. Jerold Chun, has a similar theory that alzheimer is a retro-virus like disease that causes APP mutations. He cites epidemiologic studies showing those treated with reverse transcriptase blocking drugs (like those used in AIDS/HIV), appear to be get Alzheimer's at a lower rate! And Pankaj Kapahi has shown that if your body can't clear AGES (for example, when there is defect in the DJ-1 gene responsible for the glyoxalase enzyme) then you are at a 2x-4x risk for Parkinson's and Alzheimer's! in general, it seems that anything that reduces mitrochondria function (or increases metabolites like L-2GH), accelerates the signs of aging (inflammation). Some Mitochondrial mutations (PINK1, PARKIN, etc) lead to energy deficiencies and synucleinopathies (Parkinson's and Lewy bodies dementia). These RNA-binding protein (Pumilio2/PUM2) increases with age, and inhibits mitophagy (by suppressing the Mitochondrial Fission Factor/MFF protein). The Urolithin A metabolite is said to be an antagonist of the Farnesoid X-Receptor (FXR), which up-regulates TFEB/autophagy, but unfortunately as you age, the ability for bacteria in the gut to convert polyphenol Ellagic Acid (EA) and ellagitannins into urolithin A.(UA) decreases. Regarding these genes (ex. shank2, CHRNA7 ), there was a good Ted Talk by Russel Foster ('Why do we sleep?') that mentions how the genes involved with sleep, are the same as those involved with schizophrenia and perhaps schizophrenics have either higher or lower levels of synaptic pruning. Finally I can't remember where I heard it, but I recall an association that that those with exceptional memory when young, seem to be more prone at developing dementia when older. I believe its along the lines of the dopamine producing neurons burn out faster.

Inosine was found to increase serum urate and slow progression in women with PD, but not men. It caused kidney stones in 3 of the 14 women, however. pubmed.ncbi.nlm.nih.gov/31484712/ Uric acid is the main peroxynitrite scavenger; finding a well-tolerated peroxynitrite scavenger really ought to be a research priority. Dairy milk and glycine and tryptophan [PMID: 30845731] can lower uric acid/urate. Dairy consumption is associated with increased risk of PD.

@@rhyothemisprinceps1617 ​ @Rhyothemis princeps Thanks, Julie Anderson from the Buck Institute suggests Paraquat exposure, elevated levels of Iron ( or low blood anemia), may all contribute to Parkinson's. Lithium and rapamycin, may block mtor (which blocks autophagy/tfeb), and therefore these may increase autophagy expression. Apparently, as you age, autophagy/tfeb decreases , while pro-inflammatory senescence cells increase. She suggests senescent cells are needed early in development (antagonistic pleiotropy) but for some reason they increase in numbers as you age.. continuing to function, just not able to divide or die (apoptosis). However, if the cell is functioning, why are senescent cells generating inflammatory/tumor promoting SASP signals?

My understanding is that senescent cells are not functioning normally but do play a functional role in wound healing (and maybe that is why they spew out inflammatory mediators) and also to a certain extent the senescent state prevents transformation of the cell to cancer (but it's not perfect). I read something interesting yesterday on senescent cell clearance and glutaminase and intracellular pH, which shows similarity between cancer and senescent cells; notes and links to references here: rhyobrain.blogspot.com/2021/01/brief-notes-senescent-cell-clearance.html ~ Something I remember thinking about when I was a student regarding the link between inflammation and cancer - that perhaps the reason inflammation promotes cancer is that inflammation is part of injury response and the these inflammatory markers in ancestor species might promote regeneration; regeneration would require (I think) the two main features of cancer - de-differentiation and rapid proliferation. Homeotherms have lost the ability to regenerate limbs but chronic inflammation might set off ancient pathways associated with regeneration, which instead result in cancer. Perhaps this has already been discussed in the literature; I guess I should look it up.

Taiji is my practice and exercise. My teacher is in late 60’s and is a master of this lost art. He moves like a 20 yr old without effort. An ancient art that is timeless. No hocus pocus. Train body & mind will follow; it doesn’t work the other way around.

@10:00 the good Doctor guest talks about dopamine problems apropos circuitry problems. If chronic alcohol use is known to result in low vitamin B12 and that is a crucial vitamin for nervous system health, why do docs say that alcoholism isnt connected w parkinsons disease? Especially since so many brain regions she’s naming “cerebrum, basal ganglia, nigra” also are targets of alcohol abuse

So since fasting increases neuroplasticity, would that not also help in combination with exercise? I'm surprised there was no discussion on this from looking at the bookmarks in the description. Is it because she specializes in exercises effects on the disease only and it may not be her specialty?

Sure, but there is surely a compliance issue to just make sure your patients / test subjects exercise. And fasting is hugely misunderstood by the general population.

I'm dying to know about the link between those medicated for ADHD and the higher likeliness for Parkinson's? I'm recently diagnosed as an adult but my grandmother died of Parkinson's. Now I'm very scared about starting treatment for ADHD.

Hello Rhonda, I am subscribed to your channel and I am thankful that I am. I do have a question. Rhonda I am trying to get a juice and smoothie shop going. But before I move forward with this, I would like to take an accredited course so that I am certified in the make up of the drinks. So that I am able to provide my clients/ customers with the proper nutrition that compliments their symptoms. My question is is there a course, school, or mentor I can contact to achieve my goals!!!! I wonder what the other sources of tremors could be she alluded to early in the interview. My dad is 4 years in parkinsons and shakes strong in his hand and we cant get him to stop drinking alcohol :(

You look just like my daughter. Anyways good video as usual I know dopamine it linked with inflammation. I personally blame heavy metals toxicity for Parkinson's and alzhiemers. Ive experienced heavy metal poisoning myself pretty severely. Im still recovering doing much better. But I can tell you from what I felt, was similar to Parkinson's/schizophrenic symptoms. I would loose my short term memory every 4 seconds couldnt even watch a movie. And it was like all the pleasure(dopamine) was sucked away from the things I once enjoyed(fav song, fav food) once I started using multiple chelators and binders it started improving drastically

80-85% max heart treadmill exercise ? Be ready to catch quite a few Parkinson's disease patients falling off the treadmill. Unless we are talking about incipient disease here.

Would be good to have a variety of simple, jargon-free, recommended exercises for people with Parkinson’s. Rank these exercises from beginners and graduate to higher more challenging ones. This is all very interesting but mind boggling and leaves you thinking - what do I DO first? Not really very useful for people with Parkinson’s.
