Also a strange comment considering malnourishment is not about how much you eat but if you are getting enough nutrients from the food you are eating. So someone over weight can still be malnourished

He really isn't though, no one gets to this sort of weight without severe mental health problems. They need serious therapy. This video is funny and it's clear Dr Now is purely reality TV entertainment but he'll never actually help anyone. These people know they're fat, they know they're lying but they're completely suffering trapped in a body AND a mind that doesn't work properly. They need time, help, therapy and attention not someone yelling what they already know.

@@BlitzkriegAP That's not his job. He's a doctor, and they're on the verge of death. Also, you know what's more rude than honestly? Lying to a dudes face about your eating habits, and demanding that you get what you want even when you didn't earn it. I've seen the show, watched full episodes, and so many of these people are in severe denial. Sometimes people need a harsh truth in order to accept reality. If they want therapy, they can go to a therapist. But they don't want that. They want an easy solution, and they refuse to listen to the doctor at every turn. So, no. A doctors job isn't to be polite, it's to let you know how healthy you are, or are not. If they get emotional over the facts, that's on them.

@@BlitzkriegAP what Nikko said, a lot of the patients just insist on making excuses, and every appointment they take up it's an appointment that could go to a different patient that actually would make an effort, because while obviously a good portion have a problem and losing weight is not easy, specially when you have reached such a level, some of his patients really try hard to lose weight and it shows in the appointments

Some people may think he's just rude, but I honestly understand him. I'm not a doctor, I'm a teacher, but fundamentally they're jobs that aim to help people in one way or another. When you want to help someone, and they avoid you, they deny the help, they make excuses, it's extremely frustrating, because you want them to do well, you know they can do it, but they themselves put obstacles to show you they can't. He's basically a father dealing with the brattiest children walking to their death.

I work in hospitality and I'm blunt. I absolutely hate sugar coating things to make people feel better about themselves when they, in fact, need to be smarter about some things. Not to mention the entitlement these people have when they speak to me like I'm not the one helping them with their events. I'm studying to be a nurse and I know my blunt nature will be more welcomed in the medical field where people need to hear the truth.

What's really impressive is that these people MAINTAIN 10k calories a day. The most I've ever tried to maintain was 4-5k and I felt like hot garbage. I was sluggish and constantly felt like I had to hurl. My body was physically rejecting the huge intake of calories

Me too, I'm 47 and always in good shape. Back in my 20s I overate to bulk (lifetime natty) but I was rejecting. I learned to eat when Im hungry and feeling good. Not when Im not and feeling bad.

The Bible is truth. Please read at least Genesis Mathew and one book of the Bible you chose yourself. The key is putting Jesus Christ’s lessons into practice in your own life. Forgiveness is extremely important to give others. To be forgiven we must forgive. Jesus Christ taught that in Mathew in a parable about a master and slave. It is a mandatory requirement. Unwillingness to look inside, process your emotions, and forgive is what keeps God at arm’s length from most people. That and the personal reading. At least those three books. I'm a bariatric patient myself, though I'm not a surgical patient, I'm prescribed ozempic. I got on it because like a lot of people I gained weight during the pandemic and I wanted my knees to work better

@@tuffmonkey6181 I understand this may be a confusing concept to some, but I'll try to explain! Body positivity stands for accepting bodies as they are. This is regardless of the health situation - even though you are unhealthy and need to lose or gain weight to be healthy, you are still worthy as a person and loveable at your recent state. It basically stands for being allowed to live, do things and be happy even if you are not at the ideal weight or shape (yet or any time). So the health aspect is not really important for body positivity, but of course it remains a fact. I hope this was understandable, English is not my first language ^^"

When that dude is like "I'm going to call the news on yiu" and he says "let me call them for you", it's funny how conversations like that literally happen all the time in medicine. I always have patients say things like "I don't care if it's too soon for my pain medication. I want it now or I'm calling the cops" and I'm like "Please do. I'd be curious to see how that conversation goes down: Yes officer, my nurse isn't letting me abuse controlled substances, please arrest him".

Im actually a patient in a nursing home (im not old, i just got too sick to go home after the hospital), and hearing the other patients threaten to call cops or lawyers because they're not getting their controlled meds whenever they want is so annoying. For the record, you can get some types malnutrition while being very fat, having a large calorie intake and body fat doesn't mean you are getting enough of specific nutrients you need. After bariatric surgery in particular people at high risk for deficiency of things like iron and B12. That said, there are specific vitamins you take to prevent that, increasing your overall food intake would not be the answer at all.

I do have a medical condition that makes losing weight difficult along with an eating disorder. But I also made sure I worked on losing the mentality that "I can't do this because..." and have dropped 30 pounds since June. It's difficult, yeah, but not impossible. It's harder to change one's mindset than to actually lose the weight.

I'm working on losing weight and have been cutting calories. I estimate I was eating 3500 calories before I started making changes, down to 1600 calories a day. Eating 3500 calories was a lot of food and I will tell you, my stomach was not happy with me most days with that much food.

College swimmer here, the whole mens team averaged between 8k and 10k calories a day, we also spent 3-6 hours swimming daily plus weights, running and calisthenics. so we burned all the calories we took in, its a sickening amount of food but you have to have the calories to be able to build muscle mass and not have your body start usings your muscle tissue for energy.

Dr Nowzaradan usually is pretty chill when these folks initially start but he doesn't sugar coat anything when it's the third or fourth visit and nothing has changed with the patient. I really like him because these patients have largely been able to talk their way out of accountability or are able to place the blame on another person and Dr Nowzaradan doesn't have that. I get the impression that the initial weight loss he requests from patients is yes to ensure they're healthy enough to go through with it but also to see if the patient is willing to make the changes they need to. Cause yes, this surgery is not a quick fix: you keep doing what you're doing and you'll be back to where you were.

You are so right! If they can't make the little changes that he asks for before he approves them for surgery, then there is no way that they will keep those changes after. The funny thing is that I know that every patient has to have a psychological eval pre-surgery to assess what, if any, the underlying issues may be... how do they pass it?

My mom got this surgery a couple years ago and since we are super close I hadn't noticed just how much weight she had lost until I seen a Christmas picture of her a year or two prior to surgery. I was absolutely shocked and she was able to get another surgery for the excess skin as well! Its really inspiring seeing the before and after pictures of people who've gotten it and taken it seriously. Seeing these people complain saying its hard or its water weight or whatever is upsetting. They are given everything they need to succeed along with a doctor who will tell them what they NEED to hear and still waste all of that.

My uncle was morbidly obese like Tammy's size, he eventually died from complications related to his obesity. His family tried to save him but in the the end you need to want to be saved, he knew the consequences but said many times he didn't care. He was a good uncle always did his best to make sure we had a good time visiting him. I’m from RHode Island and worked in the medical field…I have personally had to deal with Steven and his father! He is every bit of awful in person as he appeared on the show. We weren’t even allowed to talk to them on the phone without a witness. He posted videos about how he purposely abused the healthcare system since it was free and pretty much got kicked out of the state lol

I'm a nurse, and I'm very interested in nutrition, and I can say without any doubt that weighing 600lbs is vastly more risky than being malnourished. And here's the kicker, you get to 600lbs by eating oreos, twinkies, McDonald's, and ice cream. Do any of those foods sound like they're packed with vitamins and minerals!? Nah, most of these people are malnourished AND 600lbs.

Dr. Now has probably been doing this and seeing these patients for a long time and it is frustrating to deal with patients that don't seem to want to get better.

I'm currently on a weightloss trip. The people that blame genetics for them staying fat I despise. That just means you'll have to be more careful with what you eat, or get up and exercise more. The person at fault is yourself, you can't change your DNA but you can change your weight and mindset

I'd say valid blame is medication. Some medicines do get you to puff up like a popcorn and it's quite difficult for those who take it, otherwise i probably wouldn't stand up most excuses even those i make for myself.

@@ayf449 100% man. Some medications do make you puff up like crazy. Others have a more mild effect that can still be combated, but that is still valid. I have made many excuses and likely will still going forward, we all human. In the case of medication induced weight gain, if you are still continuing with a healthy life style there is nothing to criticise :).

I think it's the fact they come to him for help but don't actually want his help. They waste his time that could be spent with someone who does want to make the changes instead of many of them who just are looking for a magic solution. 1st step of fixing any issue is to acknowledge it which in this case you can't do if you say it's 700lb of water lol! The mindsets of the participants on this show are such a textbook example of why counseling and therapy are such a crucial part of success when it comes to both losing and keeping off the weight. Their mindsets seem delusional and skewed because they are, so that needs to be an absolute requirement. I do love this show and I know Dr Now usually requests they go to therapy too but they often don’t get that it’s just as important as the diet.

I totally get where Dr. Now is coming from it’s the same way I feel about people who smoke, get cancer, beat it, then continue to smoke, and get cancer again. I feel zero remorse, it might sound cold but I had cancer when I was 21, I believe brought on by all the fast food I used to eat, I don’t eat it anymore. He’s tired of hand holding and good bedside manner, if they aren’t going to help themselves, why should he do it for them.

So I've never been an unhealthy weight to these standards of course, but fun story about the stomach shrinking thing: When I was in treatment for Lyme disease I had a pic line and daily IV antibiotics for 9 months. In those 9 months, I threw up 10-15 times a day, and basically just wasnt getting the calories. My stomach did shrink to basically what the surgery shrinks it too and if you dont overeat, it doesnt grow back and you dont gain weight. (Of course this is just my personal experience and each person reacts to things like weight loss differently) But, long story short lol these people bullshitting. xD

I was almost 300 lbs at one point and blamed no one but myself, no I'm down to 210 and still working on it. went from being a XXL and it being snug to an XL and it being super baggy (I can fit a large but depends on the brand). I could eat 2 large pizzas and a side and still be hungry. now I can barely finish 2 slices of deep dish pizza 😆 When you're going through food addiction you have to understand that you're part of the problem and address it to move forward. I went for Bariatric surgery and that was how I found out why I couldn't lose weight. Thermodynamics has nothing to do with a hormonal imbalance, when that imbalance directly impacts metabolism. That's just ignorance.

@@ChelleTackett your metabolism, no matter what impacts it, runs on calories in calories out. You were not told you cannot lose weight, you were told its more difficult. Just because someones metabolism slows down and an only burn less than the average say, 1500 calories a day, doesn't mean they can't lose weight by eating less than 1500. It also doesn't mean physical activity doesn't have an impact on top of it. My point is suggesting that people can't, and so definitively, does harm and miseducates. Its simply not true. Unless your manifesting calories out of thin air, thermodynamics are at play like they are in everything that involves energy.

I'm over here feeling absolutely ashamed of my slight gut, and I still recognize that I'm not eating as healthy and exercise as much as I could - and then there's these people who have at least 3 times my body mass, some of them even bedridden, and they're still pulling every excuse in the book on why they're not working towards a healthier life. It's all about psychology, and these people _need_ to hear the painful truth of what they're doing to themselves more than anything. If they're to even start setting goals for themselves, they need way more than just positive reinforcement.
